Conference Registration
Registration paid on or before April 1 is $125.00. After April 1 the fee will increase to $135.00. A nonrefundable $25.00 deposit is due by
February 28. A minimum deposit of $25.00 is required to make payments by installment. All monies are nonrefundable after April
1. Please note: Hotel accommodations are not included in the registration fee. (See hotel information below)

your payment to: The National Women’s Forum, P.O. Box 44468, Detroit, MI 48244. All checks should be made payable to The
National Women’s Forum. A confirmation receipt will be sent to you via mail or e-mail within two weeks of your registration. If you do not
receive a confirmation receipt please contact us at nwf-pcaf@sbcglobal.net. A complete itinerary will be sent to you three weeks prior to the

Hotel Information
The hotel rate per night, per room is $99.00. This rate includes single, double, triple and quadruple occupancies. This rate is not
guaranteed for reservations made after April 8. The hotel parking rate is $12.50 per day for overnight guests. Reservations may be
made by visiting the Hyatt/NWF webpage @ https://resweb.passkey.com/go/NWF10 or by calling the Hyatt Regency Crystal City toll-free at

All events will be held at the Hyatt Regency Crystal City. The hotel provides complimentary shuttle to the Reagan National Airport
and Pentagon City Mall. The shuttle runs every 20 minutes. The Pentagon City Mall, located one mile from the hotel, is anchored
by Nordstrom and Macy's and includes 130 additional stores.

Service on Friday evening is open to ALL at no cost!

Vendor Application

The National Women’s Forum will have space available for table displays and vending during the 2010 Spring Conference in Washington,
D.C. If you would like to rent a table, please complete the application and submit it along with your rental fee to the address below.
Vending guidelines are as follows:

- The cost per table is $100.00 for the weekend.

- You will be provided with one standard 6 foot table, and 2 chairs.
- You may set up your table on Thursday April 29, no earlier than 5:00 p.m.
- A storage room will be available to secure your items overnight.
- All tables must be disassembled on Saturday, May 1 by 2:00 p.m.
- Vending is not permitted during evening worship.
- The deadline to submit your application is April 1.
- If we are unable to accommodate your request for rental space your fee will be fully refunded.

Renaissance Award Nomination Form

The National Women’s Forum will recognize two women during our Celebration Brunch who have gone above and beyond the call of
duty to meet the needs of others. If you would like to nominate someone whose life exemplifies selflessness, please complete the form
below and submit it to nwf-pcaf@sbcglobal.net . You may also mail your nomination to: The National Women’s Forum, P.O. 44468,
Detroit, MI 48244.

General guidelines for submission:

- Nominee must be affiliated with the PCAF
- Nominee must be at least 18 years of age
- You must be at least 18 years of age to submit a nomination
- Your name and contact information must be included for your nominee to be considered. Please indicate if you would like to remain

Personal Achievement Award Application

If you reached a personal goal between April 1, 2009 and April 1, 2010, the National Women’s Forum would like to celebrate your
achievement. Perhaps you started a business, finished school, wrote a book, received a promotion, became or ordained minister, or
completed a vocational or training course. Any significant achievement will be honored during our Celebration Brunch which will take
place on Saturday May 1, 2010.
