Bids and Awards Committee (BAC) Resolution recommending to the Head of

Procuring Entity (HOPE) the renewal of the Dietary Services for DOH TRC
pursuant to ITB 2013-39

WHEREAS, the Rizal Medical Center Bids and Awards Committee (RMC-BAC), acting
as procurement agent of DOH TRC, advertised and posted the procurement multi year dietary
services on 27 November 2014;
WHEREAS, the approved budget for the contract was Forty Three Million Pesos (PhP
43,000,000.00);
WHEREAS, in response to the advertisement and posting of the RMC BAC, only DOHTRC Bicutan, Multi Purpose Cooperative (DOH-TRC MPC) secured bidding documents and
consequently solely submitted its bid for the project;
WHEREAS, after a thorough post qualification process, the RMC BAC issued a
resolution recommending award in favor of DOH TRC MPC to provide dietary services to DOH
TRC;
WHEREAS, in compliance with the pertinent provisions of R.A 9184 and its Revised
Implementing Rules and Regulations, the corresponding Notice of Award, Contract and Notice to
Proceed were issued by RMC in favor of DOH TRC MPC;
WHEREAS, on 1 December 2014, the Office of the Director of DOH TRC received a
letter from DOH TRC MPC requesting for renewal of its contract under the terms and conditions
of the bidding documents, particularly under ITB 2013-39;
WHEREAS, the aforementioned request was indorsed to the RMC BAC with a
recommendation from the Head of the Procuring Entity of DOH TRC for the renewal of dietary
services of DOH TRC MPC;
WHEREAS, the BAC has determined that DOH TRC MPC has satisfactorily complied
with its contractual obligations with DOH TRC pursuant to its dietary agreement entered into on
7 February 2014;
NOW THEREFORE, We, the members of the RMC BAC hereby RESOLVE, as it hereby
RESOLVED:
1. To recommend to the Head of the Procuring Entity of DOH TRC the renewal of
dietary services of DOH TRC MPC with the contract price of FORTY TWO
MILLION SEVEN HUNDRED THIRTY THOUSAND THREE HUNDRED
SEVENTY FIVE PESOS (42, 732, 375.00);
2. To recommend t to the Head of the Procuring Entity of DOH TRC that the terms and
conditions of the original contract shall apply to the second contract;
3. To recommend t to the Head of the Procuring Entity of DOH TRC that any changes
in the quantity of the contract shall be based on the unit cost of the original contract;
4. Any changes in the contract be mutually agreed upon by the parties, subject to
existing laws, accounting and auditing rules and regulations;
5. To recommend t to the Head of the Procuring Entity of DOH TRC the approval of the
same.
Resolved this 15th day of December 2014 at the RMC BAC Office, Pasig Blvd., Pasig
City, Metro Manila.
