The Partnership Portfolio

Purpose of this assignment:

This portfolio will demonstrate your understanding of working in partnership within the health
and social care sector. It will show that you understand:
(LO1) partnership philosophies and relationships in health and social care services
(LO2) how to promote positive partnership working with users of services, professionals and
organisations in health and social care services
(LO3) that you can evaluate the outcomes of partnership working for users of services,
professionals and organisations in health and social care services.
Task 1 (LO 1,2,3, M1, M2, M3, D1, D2, D3)
You must collate a portfolio of research and evidence to cover each criteria. Each portfolio should be
individual and the way you present it is up to you. Be as creative as you like. You may choose to
produce the portfolio electronically, by hand, using a scrapbook approach or a range of methods.
The important point is you are responsible for your learning and independent research is
required.
The portfolio will include current and relevant research, information and materials from a variety
of sources. You must identify the relevance of current legislation and policy and show evidence that
links to your own work placement where appropriate. All evidence must be referenced accurately
within the portfolio and in the bibliography. Each piece of evidence requires a piece of your own
writing which explains why you have added it i.e. how it meets the criteria. It is your writing that will
provide the required explanations, analysis and evaluations.
Use headings to support presentation, structure and clarity. Ensure you clearly identify where each
criteria is covered.
N.B. You should refer to the assessment criteria when planning and working on your portfolio
to ensure you cover all the required elements.

Understand partnership philosophies and relationships in health and social care services
You may choose different evidence. Some ideas to get you started:
1.1 explain (make clear by giving detail/ facts)
Create a mindmap to show the various philosophies that underpin working in partnership in H &
SC. Explain some of these philosophies.
Outline the relevance of legislation and policy to philosophy e.g. Children Act 2004, NHS and
Community Care Act 1990, Working Together 1999
Add the mission statement from your work place and somewhere else then discuss how these
correlate to the setting philosophy.
Add relevant policies from your setting - highlight key points explain how the policy relates to
the philosophy to show your understanding.
Add a relevant section of text highlight key points explain the philosophy to show your
understanding of it. You could relate it to your setting.
Add a link to a programme have key relevant notes about the programme and explain the
philosophy within it.
