GME/IME Medicare Reimb
Childbirth and Family Education: Classes for pregnant women and family on childbirth and parenting.
Medical Respite Center: With a collaborative it provides 15 beds for homeless patients who are being discharged to continue recuperation and self
care. It also offers social service resources.
New Directions Case Management: with collaborative, it aims to reduce frequent use of ER by those with complex medical and psychosocial
needs. It provides case management and links to community resources.
Career Academy: Provides high school students with continuing education while providing them with internship, mentorship in health care related
fields.
Family Medicine Residency Program: Hospital residency for doctors in Family Medicine.
Nursing Students Clinical and Preceptorship: Preceptorship for student nurses from community college.
Living Well Community Health Education: Series of education programs for the community on preventive care, physical activity, nutrition, end of
life decisions, childbirth, etc.
Health Benefits Resource Center: A one-stop venue providing enrollment assistance, information and referrals for uninsured low-income
residents, helping families gain access to public and private health insurance, social services and nutrition programs. It also educates residents
about the Covered California program.
Parish Nursing: A nurse worked with local churches to bring health education and provide community resources to members of the congregation.
Laboratory, Ultrasound and Radiology Internships: Provided internships for students in these fields.
Meals on Wheels: Meals prepared for needy in collaboration with Health Trust.
Community Building Sponsorships: Cooperating with others, participation in health fairs and screenings, flu shots, glucose and cholesterol and
blood pressure screening.
Support Group Administration: Persons and facilities provided for medical related community groups to bring health education to the community.
Palliative Care Services: Provides for a director for palliative care services provided to patients.
