availability at work place. Regular inspection record for equipment,
Fire Extinguisher with different type availability within calibration
tags, Fire Hose installation, water/sand bucket, First Aid box.

3) LMS PK has recently got a new customer on-board. The facility has been
acquired and you have been asked to develop layout for fire extinguishers.
What factors will you consider while deciding locations of fire-extinguishers.
The Fire extinguishers should be strategically placed according to
hazards, type of fire, material stored and as recommended in MSDS of
hazardous material nearby. All type of Fire Extinguisher available to
extinguish A,B,C class Fire.
The placement of Fire Extinguisher should be 15 meter reach inside
warehouse if Hydrant system is not available, If Hydrant system is
installed then it should be placed 25 meter reach/400 sq ft area on
prominent locations with easy access.

Accident & Incident Investigation with competent person for said

6) Today due to extra workload in C shift, you have decided to stay in warehouse
overnight. It is 08:00 PM and every thin is working fine till all lights went of
due to short circuit in one of the electric main boards. The situation is not
that bad and it only needs a minor repair to resume electric supply again. You
called the vendor and luckily he was nearby. He has reported at security post,

but has been stopped by security to work as he does not have necessary
PPEs available. How will you deal with this situation? Explain briefly.

Call to concerned to switch off Main Breaker/supply

Switch on all Emergency lights in warehouse
Clear the walk ways
Ask to vendor to arrange necessary PPEs first/ or contact with
nearby DHL warehouse for PPEs availability and arrange.
Assess the work load and importance of tasks to be executed
and consult with Management/Customer (if required),in case of
any delay in operations
Resume operations once all necessary repair done with safety
precautions.
