SUBJECT: IMPLEMENTING GUIDELINES FOR THE ISSUANCE OF A

CERTIFICATE OF PUBLIC CONVENIENCE TO OPERATE A
TRANSPORTATION NETWORK VEHICLE SERVICE

Pursuant to Department Order No. ____ (Re: Transportation Network Vehicle

Service As A New Mode Of Service For Public Land Transportation And
Accreditation Of Transportation Network Companies), the following guidelines for
the Acceptance of Application for the Issuance of a Certificate of Public
Convenience (CPC)to Operate a Transportation Network Vehicle Service is
hereby promulgated:
A) REQUIREMENTS
No Application shall be allowed acceptance, unless all the requirements
prescribed below are submitted:
1) 5 copies of Verified Application;
2) Photocopy of Proof of Filipino Citizenship;
3) Proof of Accreditation of the Vehicle by a Transportation Network
Company (TNC);
4) Proof of Accreditation of the Driver by a TNC;
5) Photocopy of Comprehensive Insurance Policy of the vehicle;
6) Photocopy of Passenger Insurance Policy issued by the Boards
accredited insurance providers;
7) List of Drivers;
8) Clearance of the driver/s issued by the National Bureau of
Investigation;
9) Clearance of the driver/s issued by the Philippine National Police;
10) 2 Copies of Operators Data Sheet with recent 2x2 picture;
11) Accomplished Copy of Statement of Financial Capability Form and
proofs of entries thereof;
12) Income Tax Return duly filed with the Bureau of Internal Revenue
(BIR) or BIR Certificate of Registration;
13) Certificate of Business Name issued by the Department of Trade and
Industry;
14) Certificate of Registration and Official Receipt of proposed unit/s with
year model issued by the Land Transportation Office (LTO) or Delivery
Receipt/Sales Invoice; and,
15) Location Map of garage or address of operator.

Photocopies of the required documents may be accepted at the time of

filing of the Application; but original copies thereof shall be presented at
the time of the hearing for verification and authentication purposes.
B) FILING FEE
The Filing Fee of each Application shall be Five Hundred Ten Pesos (Php
510.00) for the two (2) applied units and Ten Pesos (Php 10.00) as Legal
Research Fee. An additional Seventy Pesos (Php 70.00) per unit shall be
paid in excess of 2 applied units.

C) VALIDITY OF THE CPC

The CPC shall be valid for one year and may be renewed. Its validity and
renewal shall be dependent on the good standing of the operator or driver
with the TNC. In the event that the operator or driver is certified as not
anymore in good standing by the TNCduring the validity of the CPC, the
grantee shall be issued Show Cause, and Cease and Desist Orders.
D) FOR-HIRE REGISTRATION
All grantees shall be required to register with the LTO for the reclassification of their authorized units pursuant to its new guidelines.
E) ACCEPTANCE BY THE REGIONAL OFFICES
The Regional Offices of this Board may accept and approve Applications
of this type, subject to compliance with jurisdictional authority.
Any motor vehicle owner found violating Department Order No. ____ and this
Circular shall be penalized in accordance with the provisions of Joint
Administrative Order No. 2014-001 and other existing issuances.
This Circular amends and supersedes all issuances inconsistent herewith, and
shall take effect immediately following its publication in a newspaper of general
circulation.
