Demand for
petrochemicals is also being
affected by government policy
to reduce environmental
pollution and pursue a longterm target of developing a
modern industrial base with a
core of large-scale industrial
units replacing the former
industrial base of small-scale
local factories that are both
energy inefficient and polluting.
China has recently seen an
expansion of refining capacity
with the result that output of
some products now exceeds
domestic demand, forcing the
state-run refining sector to seek
export markets for its excess
capacity. It also means the
completion of some refinery
expansion schemes will be
postponed from 2015 to 2020.
Exports of petrol and diesel
currently total around 5 million
tonnes a year combined
and are shipped mostly to
Asian markets including South
Korea, Singapore and India.
The petroleum exporters
are mainly Sinopec and
China National Petroleum

they have export rights,

Relative to the rest of

2. Many Australian refineries

by increasing Caltexs fuel
reserves above its safety stock
in Adelaide from five days to
32 days when the facility is full.
On the decision for
the new location, national
planning and optimisation
manager for Caltex Terminals,
Paul OLoughlin, says: With
increased storage capacity
and the supply chain now
open to deeper berthing
facilities, constraints that
contributed to fuel shortages
across Adelaide and South
Australia over recent years
have been eliminated
and the fuel reliability into
the state improved.
The Outer Harbor location
also removes the facility from
the suburbs to an industrial
precinct, thereby reducing
risks to the community.
The new terminal has
facilities to load out to road
tankwagons in three bays,
with plans to add loadout to
rail tankwagons in the future,
and room to add one more
road tankwagon bay.
Increasing efficiencies
The purpose-built import
fuel terminal, at Adelaides
Outer Harbor, is markedly
more efficient than Caltexs
existing Birkenhead terminal
in Port Adelaide. The historic
facility has now received
its last tankship delivery.
Each of the three bays
at the new terminal has
five loading arms, with
each arm able to deliver
at 2,400 litres per minute. To
date this accommodates
about 48 trucks per day,
allowing 63 million litres to be
loaded out in one month.
The terminal is fed via
marine loading arms, and
two 14 wharflines. This allows
tankship delivery at 3 million
litres per hour, thus reducing
tankship turnaround times.

Shift work
and fatigue
keeping it simple
This article uses the 2005
Buncefield explosion and fires
incident as a springboard
to consider some of the
key practical factors that
operations and HSSE
managers, safety reps and
others in the major hazard side
of the tank storage industry
should look for in managing
shift work and fatigue. Why
use Buncefield as an example
again? There is always
more to learn, and finding
practical help is not always
easy, despite (or perhaps
because of) the multiple
reports. Buncefield was
unusually well investigated;
The investigation
Human factors and other
specialists were involved
early on in helping assess
documents and other
evidence, and in preparing
the questions for interviews.
This meant that human
factor topics were identified
early on in the process and
appropriate evidence was
captured and professionally
assessed. For fatigue and
55
safety
shiftwork the authors early
involvement included
recovery of the shift pattern,
hours worked, and related
safety management system
documents. The HSEs Health
and Safety Laboratories (HSL)
provided specialist support.
Looking for potential
fatigue and shift-work issues
in incidents is not a given
unfortunately and is often
neglected. To find something
you have to be looking for
it. This incident happened at
night and towards the end
of a 12-hour shift the classic
small hours period. Up to
one in five UK traffic accidents
on major roads have been
attributed to fatigue with a
significant number occurring
in the early morning period
(HSE 2006 p6). First sight of the
base shift pattern and the
actual shifts worked (found
pinned to the wall of the
control room) also suggested
that there could be an issue.
Analysis of the shift pattern
as planned and as worked
The interviews confirmed that
the then-current eight-week
shift pattern was designed for
10 supervisors but was being
worked by eight because of
on-going recruitment and
retention difficulties. This
resulted in e.g. supervisors
working seven 12-hour shifts
continuously in weeks four to
five, five 12-hour night shifts
and increased overtime. They
were left to arrange their
own holiday and other relief
cover. Overtime was also
required over a sustained
period for on-going project
and (ironically) safety
improvement work. While
individual levels varied, the
table above shows the scale.
There was also no
structured arrangement for
within-shift rest and meal
breaks so that, in practice, the
supervisors would usually eat
or take time out at their desk
in the control room. Clearly
these were not adequate
breaks away from their work

Average overtime worked by the Buncefield supervisors in the 5 months before the incident (HSL 2007)
HSL also carried out
an assessment of the shift
pattern against the HSE shiftwork guidance (HSE 2006).
The report concluded that:
the continuous periods of
night shifts and 12-hour shifts
were cause for concern;
accumulated fatigue was likely
present in the supervisors and
others; there were inadequate
rest days; rest/meal breaks
were irregular; and that the
overtime levels increased
the likelihood of fatigue.
A monitoring task like that
carried out by the Buncefield
supervisors over long periods
(and with workload peaks
and troughs) is also known to
increase the likelihood of error
(HSL 2007; HSE 2006). Another
significant factor was the
absence of a formal policy
or procedure for managing
fatigue and shift-work. As
with looking for fatigue and
shift-work in an investigation, if
there is no safety management
system focus then effective
arrangements are unlikely.
But people get it right
most of the time
The fatigue and shift-work
aspects were important at
Buncefield but were not of
course the only factors. It is
never that simple. But they
did colour staff judgements,
decision-making, behaviours,
effectiveness, vigilance
and much more over a
sustained period. So fatigue
and shift-work was more of
a longer-term influencing
factor, harder to assign direct
consequences to but they
were clearly there. Other
human factors contributed
including shift handover,
approach to managing
fatigue and shift-work is a
good start, and the HSE
guidance on this approach
is simple and accessible
(HSE 2006). Avoid overcomplicating things though.
It is vital to gather feedback
from the real operating
experience through auditing
and monitoring, and from
those involved in managing,
supervising and working
the shift system. And of
course being able to record
and analyse the actual
individual hours and shifts
worked, not just the averages
or the planned shifts.
And yes, you do need a
policy and a procedure, but
keep them short and accept
they will develop as you
engage with the issue and
gain experience. Equally a
shift system needs managing
it is no good leaving those
working the pattern (or an
overloaded supervisor) to deal
with planned and unplanned
absences on an ad-hoc
basis. While inevitably there
will be exceptions, it is how
these are managed after
the exception event that
really matters so that tired
people are not then returned
to work without at least
some effective mitigation.
There is no point in tinkering
with an existing shift pattern
if it is not being worked as
planned. The underlying
organisational factors are
usually simple, e.g. not enough
staff to work the rota, or not
enough people in some roles,
or insufficient flexibility in the
pattern to cope with real work
demands. Workloads and work
planning may also need fixing.
So these factors need to be
safety
addressed first, then check out
the shift pattern and review
it once you have experience
of it working as planned.
Finding a reasonable and
workable shift pattern also
requires early, active and
informed user involvement,
i.e. those who work the shifts,
supervise and manage them.
But there is no magic or
perfect shift system out there.
They are all compromises
because we are day animals
nature has not wired us for
shift working and especially
(but not just) night work. This
leads to another point: the
range of shift pattern choices
is in practice small because
of simple arithmetical facts
about dividing the available
time and days into workable
patterns over a calendar
year (see Miller 2006 for a
full explanation of this).
Training and awareness
play a part, particularly
in focusing manager and

Concrete Canvas (CC) is a
flexible cement impregnated
geotextile that hardens on
hydration to form a durable,
fibre reinforced, fire resistant
and waterproof concrete
layer. Essentially concrete on
a roll, it has rapidly gained
market acceptance in a
number of applications in and
around refinery and terminal
infrastructure. Primarily, CC is
used as a means of rapidly
lining clay and earth secondary
containment bunds.
CC provides a hard
armour surface protection
preventing weathering
erosion of the bund,
negating the need of
costly maintenance or
remediation by securing
the height and integrity
of the structure.
CC is fibre-reinforced
and has an abrasion
resistance twice that of
ordinary Portland cement.
It can be installed at rates
of up to 800m2 per day,
minimising time and H&S
permit requirements on site.
BTE manufactures
equipment for the
aboveground storage
tank industry, including its
Geodesic FlowDome roof
It acts as an effective
weed suppressant,
reducing maintenance
costs associated with
grass cutting of top soils
and negating the risk of
fire from dried grasses.
CC protects against
animal damage.
CCs ability to be installed
in wet conditions reduces
programme disruption due
to inclement weather.
CC can also be supplied
in man portable batched
rolls, which do not require
and plant or machinery
for installation.
CC is very efficient in terms
of material transport and
reduces vehicle movements
on site. 200m2 of CC can be
supplied on a single pallet.
Finally, due to its flexible
state prior to hydration, CC
will easily conform in and
around pipe extrusions and
sensitive infrastructure.
As well as bund lining, the
inherent advantages of CC
also lend the material to
lining tertiary containment
lagoons, general weed
suppression under pipe tracks
and fencing, as a means of
replacing concrete half pipes
for ditch lining, pipe protection,
and cable protection.

Mascoat is the first insulating
coating company to prove that
its product can help protect
personnel from burns with as
little as 1mm on a 135C surface.
Storage facilities dealing
with #6 crude and heavy oils
can use Mascoat to insulate
condensate return tanks,
boilers and steamlines. For oils
and chemicals that require
heating, Mascoat prevents
tank corrosion while stabilising
the tank temperatures
required for product quality.

NDT Global Services has
been trading since 1990. The
company provides inspection
and test equipment for
inspection companies, process
plant, manufacturing and
service industries for both
UK and overseas markets.
NDT also specialises
in guided wave pipe
inspection service, which
allows long lengths of pipes
in difficult-to-access areas
to be quickly screened for
corrosion or other defects.
Operating on pipes from
2 diameter, the wavemaker
system employs lightweight
transducer rings to send guidedultrasonic waves up to 100m in
each direction along the pipe.
Sophisticated software
routines help categorise
reflections from changes in the
cross section of the pipe, giving
100% volumetric screening of
up to 100m of pipe from the
transducer ring location.
The wavemaker system
can use both torsional and
longitudinal waves. This means
that the inspection can be
performed on both gas and
liquid filled pipes without taking
them out of service and with
minimal surface preparation.
The system is most
commonly used for road
crossings, wall penetrations,
straight runs of pipe where
100% of the volume must be
inspected, inspecting under
many types of supports,

The construction industry
has advocated off-site

order to control the quality

these principles Hawkes Fire
was able to crane both units
into place onto prepared
concrete bases. This saved
money by avoiding block
built construction. It has the
additional benefit of being
useable on another site should
the operators needs change.
