Introduction
As part of the Managing safely course, you have to complete a project and
have it assessed. Youll be expected to carry out a risk assessment based on
your own work environment. You need to record the results on the IOSH risk
assessment forms provided and submit them for marking. Do not submit any
additional material in support of your project work, because only the three
sections of the A4 project will be assessed. You can submit the project either
by handwriting it onto a printed copy or by typing into the PDF.
Before returning your project, check that your name, the end-of-course date
and the name of your training provider are recorded on the top of the first
sheet. By entering your name here, you are confirming that this is your own
work.

Please read all the instructions before starting work on your project.
The Risk Assessment Project is broken down into a number of stages, which
should flow from start to finish, telling a complete story. Begin with describing the
process, activity or task to be risk assessed. Then identify three hazards, assessing
the risks associated with those hazards and recommending appropriate risk
controls.
Weve provided you with:
- a two-page Risk Assessment form on which you should record your project.
- a guidance page, including a Risk Rating Calculator, Action Level Table and an
example Risk Assessment (for you to refer to when completing your project).
Please complete each section of the Risk Assessment form as detailed below:
1. The risk assessors name and the date of the risk assessment
The space at the top of the form on the first page is for your name and the date
you are completing the project.
2. Description of the process, activity or task to be risk assessed
On this first page, record a brief description of:
- the location where the activities are taking place
- the activities being performed
- the equipment and/or substances being used
- the people involved in these activities (and others who may be affected).
3. Risk Assessment
You must complete all three rows on the second page. Begin in the first column by
recording a brief description of three hazards and the hazardous events that may
arise in relation to the process, activity or task youve just described on the previous
page. Here you must describe:
