Area: Emergency Room

Date: September 09, 2015

I. Objectives

To be able to familiarize ourselves in Emergency Room


To be able to learn the role of an Emergency Nurse
To be able to learn how to use ECG

II. Clinical Learning Experience


Today, we gain knowledge on how to use the ECG machine with the help of Sir Ronnel
our clinical instructor in this ward. He also orient and help us to familiarize in Emergency Room,
he also taught us on how to be an emergency nurse, how can we assess patients upon entering the
emergency room and how to use the beds and the other things in emergency room. He also
educate us on how we can assist nurses in their jobs. I also learn the different emergency drugs
and the classification and mechanism of actions of each.

III. Values Learned


This is our first day in Emergency Room and similar into other wards I feel a little bit
uneasy because this is our first time and nobody knows what is coming or what is going to
happen, good thing Sir Ronnel taught us how to be stay calm all the time and at the same time
how to work calmly and we don't have to fright. In our first day, I forgot my cap I'm lucky that
sir gave me a chance to get it from my brother in law. As a nursing student this is the values I am
lucky I learned first is I learn how to be ready all the time second is how to keep calm and at the
same time have presence of mind.

III. Evaluation
As I evaluate my day today. I think I achieved my objectives because this is our first day
and we need to learn from this ward to help our patients.

Area: Emergency Room

Date: September 14, 2015

I. Objectives

To be able to develop interpersonal skills by listening effectively


To be able to apply holistic care
To be able to respect patients decision

II. Clinical Learning Experience


We have a lot of patients today that come and go. We assist nurses, take the vital signs of
our patients, perform ecg, weight the other clients, assessed pregnant women who were going
into the delivery, get the fht and fundic height.

III. Values Learned


I was the one who need to assist one of our pregnant client today but she don't want me to
assist her in wearing diaper so I respect that. We need to respect patients decisions, we need to
respect their feelings or what they might feel. As a future nurse, I need to listen effectively too.

IV. Evaluation
Some of my objectives are unmet today because patients here in emergency room come
and go and some in my objectives need long period of time to be met.

Area: Emergency Room

Date: September 15, 2015

I. Objectives

To be able to identify the appropriate action for the emergency situation


To prioritize action according to the needs of the situation

II. Clinical Learning Experience


Today, we learn how to prioritize action in our patients. To assess them if the pain is true
or not.

III. Values Learned


We have patient today that rushed into the hospital, we can see that her relatives too much
but when she assessed by the doctor nothing is wrong with her. I learn assessed first the patient
before making an action because it can cause some effects if we are going to have wrong
assessment.

IV. Evaluation
Today my goal is unmet because upon arriving of the patient I thought she was in seizure.
I was not able to identify what her true condition.

Area: Emergency Room

Date: September 16, 2015

I. Objectives

To be able to establish rapport into our patients


To be able to help a frightened child

II. Clinical Learning Experience


We take vital signs of our patients today. We just wait wait and wait for the other patients
to come.

III. Values Learned


This is our second to the last duty in Emergency Room for this semester, time flies fast
when you're enjoying what you're doing, in four days I learned a lot. Today we have patient who
was a child that have big fear in the hospital and for who was around the hospital premises, I am
lucky that I gained his trust that he let me touch him to take his vital signs. I just love kids and I
can endure their craziness. To become a good and effective nurse we need to have long patience
and a good attitude.

IV. Evaluation
My goal was met to establish rapport into the child I tell what I will do to him and give a
demo to the child.

Day: 5

Ward: Emergency Room

Date: September 28, 2015

I. Objectives

To be able to assist in performing prompt and effective action for seriously ill or injured
patients
To be able to interact with patients and their relatives
To be able to identify risk factors with their illnesses

II. Clinical Learning Experience


We observed the minor operation in a child who has open wound in his right foot that
needs to suture because of the motorcycle that falls to him and I assisted to clean his wound.

III. Values Learned


This is our last day in Emergency Room, for the past 5 days of duty as a nursing student I
learned so much not just about Emergency Nurse but more about life. I learned to be patient,
hardworking and flexible. I find ER as a not so toxic area wherein patients come and also go
after they are admitted or just they can go home. It is also our role to interact into their relatives
to listen for their stories or what they want to tell us also we also need to think the risk factors for
example the child who has open wound in right foot was risk to fall. I enjoyed our 5 days of
opportunities here in ER a lot.

IV. Evaluation
Today, my goal was met. To finished my duty in emergency room in this semester is
already a big achievement for me. ER is an amazing place for me.
