CHAPTER I: PROBLEM AND ITS SETTINGS

1.1 INTRODUCTION
The loss of health is loss of all happiness. Mahatma Gandhi also says, It is
health which is real wealth, and not pieces of gold and silver.Health represents a
state of physical and mental well-being. Without good health, we will not be able to
enjoy the good things in life. It is also not possible to earn a good living when one
is not healthy. In general it can be said that if we have healthy people in the world,
the efficiency and productivity on the whole will increase.
For long-term healthcare, the Philippines is an irresistible destination, with its
world-class medical facilities, low cost, and most importantly, compassionate
healthcare professionals who can assist you in every step of the wayliterally and
figuratively.Elderly people with some diseases and ailments require not just
medication but also rehabilitation. That's why rehabilitation centers in the
Philippines focus on giving mental and physical help, with the distinct friendly and
caring touch that Filipinos are known for.
Rehabilitation centers in the Philippines provide a comprehensive range of
evaluative, preventative and rehabilitative services to those with physical
impairments. With the latest state-of-the-art equipment and under the care of
world-class doctors, physical therapists and occupational therapists, you can be
confident of receiving proper healthcare and treatment. Services include physical,
occupational and substance rehabilitation programs to enable patients to fully

recover gain their health back and live a normal, healthy lifestyle after being
treated.Efficient and appropriate community services must be developed so as to
support the elderly's stay in the community.
Hospital is an institution for diagnosing and treating the sick or injured, housing
them during treatment, examining patients, and managing childbirth. Outpatients,
who can leave after treatment, come in for emergency care or are referred for
services not available in a private doctor's office. These hospital may be public
(government-owned) or private (profit-making or not-for-profit); but most are public.
They may also be general, accepting all types of medical or surgical cases, or
special, limiting service to a single type of patient or illness. However, general
hospitals usually also have specialized departments, and special hospitals tend to
become affiliated with general hospitals.
A community hospital should be an integral part of thehealth care system in a
district, providing an extension toprimary care services specially for elderly people
and a source of referral to and fromacute hospital services. Patients may be under
the care ofconsultants or nursing staff. The range and mix of services provided for
each caregroup will vary according to local needs and the availabilityof other
services. The essence of community hospitals istheir diversity, versatility and
flexible response to localrequirements.Having a landscape may help patients to
reduce stress, re-humanize health care and help the patients faster recovery. It
will not only focus on the medical needs, but also the emotional, social and
psychological needs of all the users of the facility.

1.2 BACKGROUND OF THE STUDY

Santa

The City of Santa Rosa (In Filipino, Lungsod ng Santa Rosa) is a first class city
in the province of Laguna, Philippines. It is also known as "The Investment Capital
of South Luzon" since 1994. The city lies 38 kilometers south of Manila via the
South Luzon Expressway, making it a suburban residential community of Metro
Manila. This quiet town of Laguna started its progress initially from the
establishment of Filsyn, CIGI and couple of other small multinational companies
there when the government BOI provided for incentives to companies located at
not less than 40 kilometers away from Manila. According to the latest census, City
of Santa Rosa has a population of 266,943 inhabitants in 43,625 households, but
in 2005, it surpassed San Pablo City in terms of population, making it the fourth
largest town or city in Laguna after Calamba City, San Pedro and Binan City. It is
also the densest city in the province with more than 6,000 people per square
kilometer.
Santa Rosa has a total of seven hospitals with hospital bed-population ratio
of 2 beds for every 1,000 population. These hospitals generate 1,573 employees
both medical and non-medical staff as shown. Majority of these hospitals are
categorized as Level III (57%) which means a departmentalized hospital. These
hospitals rendered services such as general medicine, pediatrics, surgery,
anesthesia, obstetrics and gynecology, first and second level radiology, second
and tertiary clinical laboratory, pharmacy, specialty clinical care, and nursing care
for patients needing intermediate supervised, total, and intensive care. The

remaining hospitals (43%) are categorized as Level II which means a nondepartmentalized hospital. These hospitals render all services of Level III hospitals
except for specialty clinical care, secondary level radiology, tertiary clinical
laboratory, and total & intensive nursing care for patients. There are also lying-ins
of the same number as the hospitals in the city. In 2012, the total registered lyingins in Santa Rosa are eight (8). These are all privately-owned lying-ins with 21
total bed capacities. This facility had a total of 18 personnel of which thirteen (13)
are midwives and five (5) are nurses. Knowing that there have private hospitals,
many people cannot afford quality medical care because of limited incomes and
high cost .Providing the needs of growing population of the city in terms of
affordable health facilities, establishing an institution whose mission is to provide
high quality health care that will exceeds the expectation of the people and its
neighboring towns is an immerse step to amend the towns prolonged
contemplation for innovation.

1.3 THEORETICAL FRAMEWORK

Design is not just what it looks like and feels like. Design is how it works.
-Steve Jobs
The proponent would like to point out thatplanning for which in any case is
said to be effective if the function of a certain structure is properly utilized. Every
structure should be carefully planned in order to achieve the outmost function in

each space. What is aesthetics if it doesnt have function? In public buildings,
peoples activities are interconnected and happen in different offices or spaces. In
this case, plans should be carefully developed to set up a functional space for the
users.

A single failure or commotion in Office 1 for example, may affect the

transaction in Office 2 and so on. A single activity may limit the capacity of the
entire complex. This is how function takes effect on public buildings.

1.4 CONCEPTUAL FRAMEWORK

The design will be based on the location of the project and volume of the
population especially in the Region which have higher infection rate and mortality
who will be using the project in the near future. Principles of Modern architecture
will approach this project.
Hospitals are dynamic organizations. They have been changing in shape, size
and functions to adopt themselves to the advancements in the medical field and
socio economic changes around them. Advances in medical fields like discovery of
ether in 1846, Lister's use of carbolic spray for antiseptic surgery, Roentengen's
discovery of X-ray in 1895, introduction of CT scanners, ultrasound and MRI and
socio-economic changes like Public Health Act, national health programmes, and
healthcare financing all had profound impact on the structure and functions of
hospitals. Hospitals originated as a result of sympathy for the sick and suffering
and were initially a part of religious institutions.

More often than not, the hospital planning process concentrate more on the
designing of buildings and their architectural appearance, and devotes inadequate
attention to the planning of organization and equipment as well as accommodating
them and generating spaces to meet policies. The concern of hospital planning is
the quality of medical care and the improvement of its facilities.

1.5 STATEMENT OF THE PROBLEM

This proposal was consider the needs of the people in Sta. Rosa City.
Redevelopment is something the community and hospital staff has been
eagerly anticipating and very close to realising the dreams of a new and improved
health service. Community hospital has a long and proud history of delivering the
best health service available for the community and its surrounding areas.
However, in order to meet the changing health demands in our community and our
growing population, we need a health service that can adapt and grow with us.
The Sta. Rosa community now has more elderly people, more Aboriginal people
and

more

people

living with

chronic and

complex conditions such

as

heart disease, diabetes and cancer. The redevelopment will allow delivering the
clinical services and level of care that everyone in our community deserves. As a
result of conducting research of the existing hospital, the following problems were
observe.
Major Problem:

To have a sufficient health facilities and rehabilitation for the elder people in
Santa Rosa City. What should be the appropriate design concept and
design solution for the project?
Minor Problems:
What are the strategic solution to maintain the cleanliness and sanitation of
the project?
What are the considerations in designing a building more accessible?
What will be the materials that must be used to make the structure
sustainable?

1.6 SIGNIFICANT OF THE STUDY

Each hospital is comprised of a wide range of services and functional units.
These include diagnostic and treatment functions, such as clinical laboratories,
imaging, emergency rooms, and surgery; hospitality functions, such as food
service and housekeeping; and the fundamental inpatient care or bed-related
function. No one person can reasonably have complete knowledge, which is why
specialized consultants play an important role in hospital planning and design. The
functional units within the hospital can have competing needs and priorities.
Idealized scenarios and strongly-held individual preferences must be balanced
against mandatory requirements, actual functional needs (internal traffic and
relationship to other departments), and the financial status of the organization.
