ORIENTATION PHASE

DESCRIPTION OF PHASE
Begins when the nurse goes to the patient, introduces himself, and gets
information about the patient. During this phase that basic information about the nurse
and patient are exchanged between one another in order for them to get acquainted.
The main goal of this phase is to establish trust and rapport between the two
individuals.
OBJECTIVES:
1. To build trust and rapport between the nurse and patient.
2. To establish a therapeutic environment ensuring safety and privacy.
3. To initiate a therapeutic contract by establishing a time, place and duration of each
meeting as well as the length of time the relationship will be in effect.
4. To obtain basic information of the patient.
5. To help the patient communicate effectively through the therapeutic use of self.

NURSE-PATIENT
INTERACTION
Nurse: Good afternoon,
my name is Anam, I am
Indonesian, I am a student
nurse from CEU and I will
be your student nurse
today, tomorrow, and the
next week on Wednesday,
Thursday, and Friday. After
that times I will not around
as your student nurse
because I have another
duty, not in this NCMH.
May I know, what is your
name?
Patient: Good afternoon
Mr. Anam. Of course, my
name is Akiles
Nurse: How are you?
Patient: I am fine and
happy.
Nurse: before we talk
further, I would like to
make contract with you.
are you happy and like to
be my patient for today,
tomorrow, next week on
Wednesday, Thursday, and
Friday?

PATIENTS RESPONSE
General Appearance:
The patient is appropriately
dressed and well groomed,
body odor is noted. He has
normal facial expression
with an upright posture.
The patient can keep eyes
contact but sometimes the
patient look at the back,
side, and around while
communicate.
Cognition:
Patient is coherent and
alert, oriented to date, time
and place.

feel?
Patient: Okay. Im fine, I
feel good and better, this
day just like other day.

Psychoanalytic Theory:
According to Jean Piaget,
in the formal operations
stage, the structures of
development become the
abstract,
logically
organized system of adult
intelligence. When faced
with a complex problem,
the adolescent speculates
about all possible solutions
before trying them out in
the real world.
Cognition:
Patient is alert and can
recall past histories

Patient: I mean today I am
happy, because every day

comfortable. And I also like
to communicate outside
the ward, because I feel
more fresh. You are

admitted to Medical center,
but after that I transfer
here for continue my
medication.
Nurse: Do you know the
reason of being admitted
to this hospital?
Patient: I am drinker, I
always drunk every day. I
always drunk alcohol. I
think alcohol is not so bad,
but if it too much or abuse
the substance make our
body broke. Except drunk
alcohol, I also use drugs
like shabu. I start to drunk
and use shabu when I am
in high school. I influenced
by my friends.
Nurse: Were your friends
really
give
you
bad
influence? How it is?
Patient: My friends at the
first time give me, and then
they always invited me
when drinking until I
become addict.
Nurse: What is your job
before you admit? How is
it?
Patient: I work at Makati
as call center. I have many
client and friends from US.
Nurse: Ok Po, our time for
today is finished. I would
like to remind you that
tomorrow we will meet,
communicate, and have
some activity. Thank you
very
much
for
your
cooperation
