15 years prior to admission, patient was diagnosed with Diabetes mellitus with
symptoms of easy fatigability, polydipsia, polyuria, and polyphagia. She was
prescribed oral metformin (dose, frequency, duration????) to which she was noncompliant. 2 years prior to admission, patient experienced easy fatigability and progressive coughing. She was diagnosed with tuberculosis after confirmation with X-ray results. She was prescribed with _____ to which she was compliant and TB was resolved after 8 months of treatment. She was also prescribed insulin glargine (Lantus) to which she claimed to be compliant. This was later on shifted to biphasic insulin aspart (NovoMix). (to which she was also compliant????) 1 year prior to admission, patient experienced polydipsia, polyuria, and polyphagia accompanied by numbness and tingling on both feet, specifically the dorsum, soles, and toes not radiating to any other parts of the body. This is precipitated by ____ and relieved by ______. The patient also complained of being unable to flex and extend her left ring finger and she claimed to notice a gradual unintentional weight loss of 6 kg in __ months, which prompted hospital admission (aling ospital? Anong ginawa?). 5 months prior to admission, patient experienced blurring of both eyes, more intense on the right. It was accompanied by itchiness and a feeling that something was floating in the middle of the eye (parang may muta sa gitna na hindi natatanggal), with no pain and no erythema. She self-medicated with unrecalled eye drops which did not provide relief of symptoms. 4 months prior to admission, the blurring of vision progressed and was described as parang may nakaharang. This prompted consult at Philippine Heart Center and Xray was done. According to the patient, results revealed ruptured blood vessels? (pumutok na ugat) and she was advised to undergo laser treatment. Patient was not able to comply due to financial constraints. This prompted consult to our institutions charity division and laser treatment was scheduled. 2 months prior to admission, on December 11 and 14 on the left and right?? eye, respectively, patient undertook the scheduled laser treatment. This provided relief of symptoms on the left eye but there was persistence of symptoms on the right eye. Additionally, patient was diagnosed with hypertension with an unrecalled but elevated BP. 1 month prior to admission, there was still persistence of symptoms on the right eye, which prompted consult to our institution. _______ was done and results revealed that there was retinal detachment. Surgery was scheduled. On the day of admission, patient underwent scheduled surgery for retinal detachment.
