PETITION
That petitioner is of legal age, Filipino citizen, married and with residence and postal
address at No. IB-47, Betag, La Trinidad, Benguet.
The newspaper NORTHERN PHILIPPINE TIMES is a duly registered newspaper with
contact number: 09193676436, email address: northphiltimes@yahoo.com, website link:
northphiltimes.blogspot.com, and is holding office at IB-47, Betag, La Trinidad, Benguet.
1. Petitioner, ALFRED P. DIZON, is the publisher and editor of the said newspaper which is
circulating in Baguio-Benguet;
2. The newspaper comes out once a week or every Sunday and a total of 10,000 copies is
printed every issue for distribution.
3. That the said newspaper has its principal office in the address registered with the
Department of Trade and Industry.
4. That the newspaper is not participating in the raffle of legal and judicial notices in other
provinces and/or cities.
5. That the newspaper does not merely reproduce or reprint news pages of other newspapers
or periodicals within the region of Baguio City where the Regional Trial Court is
involved.
6. That the said newspaper is already circulating for almost 16 years.
7. The first issue was published last May, 2001 which is Vol. l, Number 01.The paper has
been then running uninterrupted since May, 2001 until the filing of this petition. The
latest edition is Volume 16 Number 04 which is dated February 7, 2016.
8. The paper is edited by the Publisher himself or the Petitioner of THE NORTHERN
PHILIPPINE TIMES and it is published mainly at Baguio City and Benguet Province.
9. The NORTHERN PHILIPPINE TIMES is filing this petition for accreditation in Baguio
City.
10. The newspaper carries news releases, articles, public notices, and commercial display
advertising.
11. To support this petition, the following are likewise attached.
(a.) A photocopy of its business registration with the Department of Trade and Industry as
Annex 1,
(b.)A Photocopy of its Permit to Engage in Business, as Annex 2;
(c) A copy of its contract form showing its commercial display advertising rates as
Annex 3;
(d) Fifty two (52) copies of the newspaper printed for 1 year without interruption.
(e) Three (3) sample copies of the newspapers format for legal and judicial notices
following the requisites prescribed by Sec. 4 of Pres. Decree. No. 1079 as Annex 4.
(f) A certified true copy of the newspapers official receipt showing payment per
contracted advertising rate.

12. The petitioner believes that it has all the qualifications for accreditation after having fully
complied with the requirements of law.
WHEREFORE, premises considered, petitioner most respectfully prays of this
Honorable Court that after due notice and hearing, an Order be issued granting this
Petition for the accreditation to participate in raffle to publish judicial notices,
advertisements and/or announcements which is required by Presidential decree No. 1079
that is emanating from this Honorable Court.

3. That if I should thereafter learn that the same or similar action or claim have been
filed or is pending, I shall undertake to report that fact within five (5) days to this
