County and the one of the busiest public hospitals in the Western United States, has begun a pilot program that utilizes a new version of WorldDoc Tx in efforts to assist uninsured individuals find stable medical homes, an effort that would curb repeated, unnecessary use of the hospital. The WorldDoc Tx care management platform is currently undergoing enhancement to include the use of Care Navigators to figure in medical, financial, and social factors to help identify gaps in care for the high-risk uninsured population. COPE (Community Outreach for Prevention and Education) is a partner in this endeavor. The success of this project is creating more opportunities for the Foundation. All of these contracts add value to WorldDocs image and capacity in addition to touching individuals who can benefit from having WorldDoc tools, says WorldDoc Foundation CEO, Garth Winckler.
