INSTRUCTIONS
1. All entries must be filled in with black ballpoint at the specified place (s). Write down your name, father's name and district according to your
Matriculation Certificate. In case of any discrepancy the form will be rejected.
2. Particulars must be filled in by the candidate neatly and legibly in his / her own hand writing with the ball point.
3. Attach attested photocopies of Matric (SSC) / O-LEVEL, Intermediate (HSSC) / A-LEVEL, Bachelor Degree, Master Degree, M.Phil Degree etc. (For First
Chance only)
4. Both the Admission Form and the original receipt of the deposit of Examination Fee must reach in the Examinations Department as per schedule
notified by the Registration & Examination Department, UE Lahore.
5. No candidate is allowed to submit two admission forms to appear in the Examination of two different programs simultaneously. In case, two admission
forms of two different programs for same candidate are received, the University is fully authorized to confiscate the fee and cancel the admission in
both the programs.
6. All payments regarding Examination Fee shall be received through single Challan Form / Demand Draft deposited through Head of Institution in the
prescribed Bank.
7. A fine of Rs. 1000/- per correction will be imposed for every single mistake; hence, it is in the interest of candidates that the Admission Form be
carefully filled / completed, all entries be made according to S.S.C. / University of Education Registration Card to avoid extra fine / delay in the issuance
of Roll Numbers.
8. No Admission Form shall be entertained after the expiry of last date for submission of forms with tripple fee as already notified; however, extra
ordinary cases will be dealt accordingly.
9 Each candidate must paste three latest photographs, one on the Admission Form and the other two on Roll Number Slips (Candidate's /
Superintendent's Copy) duly attested by the Head of Institution (on front side with stamp).

INSTRUCTIONS
1.

The candidate will only be admitted in the Examination Centre on production of this ROLL NUMBER SLIP

2.

This Roll Number Slip is PROVISIONAL and in case of any wrong statement or incorrect information, the University reserves the right to cancel the admission
and confiscate the fee / result sheet / degree of the candidate.

3.

The Examination will be held on the dates notified in the DATE SHEET

4.

Every candidate must keep his / her CNIC with him / her in the Examination Centre during the Examination Hours

5.

The MOBILE PHONE / ANY ELECTRIC GADGET (Except Calculator, where allowed) is strictly prohibited in the Examination Centre

6.

Every candidate must make his / her EARS VISIBLE during the Examination Hours

7.

The UNFAIR MEANS CASE (UMC) will be registered against:

a)

The candidate who creates any DISTURBANCE or FUSS in the Examination Centre

b)

The candidate who MISBEHAVES or MISCONDUCTS with the Invigilation Staff

c)

The candidate from whom any kind of MATERIAL is recovered (either relevant or irrelevant)

d) The candidate who gives any MARK OF IDENTIFICATION on the answer sheet (e.g. his / her Roll Number, Name, Institute Name etc.)
e)

The candidate who changes his / her Examination Centre at his / her own (i.e. UNAUTHORIZED CHANGE OF CENTRE)

8.

Bring your own BALL POINT to the Examination Centre. Each candidate must fill all the entries on the title page of his / her Answer Sheet, before starting his
/ her answer.

9.

No candidate shall be permitted to leave the examination centre before HALF THE TIME IS OVER

10. When the time allowed has expired, THE ANSWER SHEET MUST BE HANDED OVER to the Centre Superintendent even though the candidate has not
attempted any question.
11. In case any student writes WRONG ROLL NUMBER on any paper, his/her result will be withheld. A fine of Rs. 1000/- (Per Paper) will be imposed on the said
student (As wrong Roll Number fee). The result will be declared on deposit of wrong Roll Number Fee.
