This Procedure describes PT Gorontalo Minerals (GM) requirements for personal protective
equipment (PPE).

RuangLingkup / Scope

This Procedure addresses the mandatory use and general application of PPE for PT Gorontalo
Minerals staff and contractors.
This Procedure does not address PPE required for specialized activities such as certain offshore
operations including life saving appliances (immersion suits, life jackets, work vests etc.) and
emergency response equipment (emergency grab bags, fire fighting turnout gear, helicopter crash
response equipment etc.) which should be covered by the operational procedures from the relevant
Asset / Project.
This procedure applies to all staff and contractors, including visitors to sites where PT Gorontalo
Minerals activities are taking place.

5.1 Records
Records shall be maintained to document

PPE is designed to protect against work related

worker and the hazard. However, it will not
prevent incidents and does not eliminate

If the protective equipment is


defective, or becomes ineffective
while being worn the wearer may
become exposed to the hazard
without necessarily being aware of
it
PPE provides protection for the
wearer only, whereas measures
that control the hazard at source
can protect everyone in the
workplace and nearby areas
The use of PPE may introduce
additional hazards to the wearer,
such as reduced field of vision
while wearing goggles.
PPE may transfer the hazard to
another location, such as toxic
chemicals being absorbed on
shoes and clothing and transferred
to offices, dining rooms or the
home.
PPE, particularly respiratory
protection (RPE), may not be
suitable for continuous use, and
PPE might be worn incorrectly.
For example, safety boots/shoes

Equipment (PPE) is required, signs shall be
posted.
PPE shall be worn in work locations or for
activity situations where the wearing of PPE
will reduce the risk of injury.
PPE shall:
Give protection against hazard(s) without
leading to any increased risk in itself

correct fitting, proper type and model, and
Be compatible with any other types of
PPE being worn.
PT GorontaloMinerals shall provide PPE for
their staff and visitors. Contractors shall

The head is the part of the human body most
susceptible to disabling injury from an impact.
Protective headgears such as safety helmets or
hard hats are designed to provide protection
from specific hazards.

Hearing Protection

Short periods of exposure to excessive noise


levels can produce a temporary hearing loss
that may initially be reversible. Permanent
damage known as noise induced hearing loss
occurs when exposure to excessive noise
continues over a longer period.
In areas where high noise producing equipment
is being operated and / or where signs are
posted that warn of excessive noise levels.
Supervisors shall ensure that correctly fitting
hearing protection is being worn, appropriate to
the noise level and the work being done.
Protective Footwear

Protective footwear can protect the wearer


against a range of hazards including contact
with chemicals, electrical hazards, extremes of
cold and heat, slippery surfaces and punctures
from sharp objects such as nails.
In areas where there is a
danger to a workers feet,

Special precautions should be
taken in wet conditions. Such
precautions include the use of

Hand and Arm Protection

Fire Resistant (FR) Clothing

Supervisors shall ensure FR clothing is worn as


necessary, appropriate to the work being
performed. FR clothing shall cover the entire
body (neck to ankles, shoulders to wrists).
Sleeves for coveralls shall not be cut off.
FR clothing shall be worn as the outer garment
except when other personal protective clothing
is more appropriate and is required.
For
example: chemical resistant suits, rain slickers,
welders leathers, disposable protective suits.
All persons who has activities in plant area and
offshore area shall use fire resistant clothing
Respiratory Protective Equipment (RPE)

RPE is required to protect workers where


respiratory or inhalation hazards exist in the
workplace at or above established permissible
exposure limits.
Hazards include oxygen
deficiency, gas and vapor contaminants and
particulate contaminants such as harmful dust,
fumes, chemical mist or fog, smoke and spray.

RPE is typically used:


To
reduce
exposure
until
engineering controls for the specific
hazard are installed; e.g. tie-in of
new equipment.
To supplement engineering controls
and work practices which fail to
reduce the hazard to an acceptable
level; e.g. confined space activities.

During activities such as


maintenance and repairs when
engineering controls are not
available; e.g. painting, asbestos
removal.

During emergencies; e.g. H2S


release.
In areas where there is a breathing hazard,
Supervisors shall ensure that appropriate RPE
is worn.

Where activities require wearing RPE, fitness


testing, medical approval and training shall be
carried out before Supervisors allow use of the
equipment.
The limitations of respirators,
including their impact on verbal communication
abilities, shall be understood before work
activities commence.
Mixing components from different types of
respirators or using unapproved components is
prohibited.
Before donning a full facemask respirator, any
head covering, and protective glasses and
foreign items in the mouth shall be removed.
Where prescription glasses are worn, and full
facemask respirators are required, a means of
attaching the prescription lenses to the
facemask shall be provided.

In addition to regular inspection and


maintenance, RPE shall be inspected before
each use and inspected, cleaned, disinfected
and properly stored after each use.
The
maintenance of respiratory equipment and
refilling of cylinders for air-supplied respirators
shall be performed only by qualified persons.
Fall Protection

The risk of falls arises in situations such as
working on any high structure (e.g. processing
plants, drilling rigs) without a proper working
platform equipped with guardrails, knee and toe
boards, whether in construction, maintenance
or working over water.

In areas where there is a risk of


a fall, Supervisors shall ensure fall
protection equipment is provided
that is appropriate to the work being
performed.

A safety helmet, full body


harness and lanyard shall be worn
whenever fall protection is required
at unguarded elevations, where
guardrails do not provide adequate
protection and where work activities
occur at more than 2 meters above
the ground or any adjacent platform
or work surface.

Safety belts shall not be used


for fall protection

Harnesses, lanyards and other


fall protection components shall be
used in accordance with the
manufacturers recommendations
and shall never be modified.

Personal fall arrest systems


and components (except lanyards)
subject to a significant fall or full
load shall be immediately tagged
out of service, removed from
service and not used again until the
equipment has been inspected and
determined
fit
for
service.
Lanyards subjected to a fall or load
shall be immediately tagged,
removed permanently from service
and destroyed.
Blast Cleaning

require the wearing of a specifically designed
blasting helmet. This consists of a helmet
enclosing the entire head and neck and
extending down both the back and chest area

sufficient distance to prevent the entry of


abrasive materials. It shall be fitted with a wideangle transparent visor, specific to the particular
design of helmet and easily replaced when
damaged. It shall be supplied with fresh air
operating under a positive pressure system to
prevent entry of dust and spent abrasive.
Supervisors shall ensure that in grit blasting,
suitable long-armed gauntlets, a long apron and
high boots for body protection are worn.
Fresh air supply to the blasting helmet shall be
supplied from a clean air source. The flow rate,
pressure and filtration of air shall meet the
helmet manufacturers recommendations.
In addition to regular inspection and
maintenance, all PPE for blast cleaning
operations shall be inspected by the operator at
the beginning and end of every shift or operator
change over. Defective parts shall be replaced
to ensure adequate performance as described
above. Items requiring specific attention are the
helmet visor, the air filtering system and hoses.
Welding, Grinding and Cutting Operations

As a minimum the following personal protective


equipment shall be worn:

Safety helmets and safety


shoes or boots (if necessary, safety
helmets may be removed when
wearing welding helmets or hoods).

Flame resistant clothing that is


free from grease, oil, and other
flammable material.

Sufficient additional protective


clothing to protect all parts of the
body from air-borne sparks and hot
metal slag.

Specialized welding helmets,


welding hoods or welding goggles
with shaded lenses (as specified in
Appendix 2) shall be worn by the
welder, helper and firewatcher
during
welding
and
cutting
operations.
In addition, during
welding, the welder shall wear fire
resistant gauntlets and apron.

During grinding operations,


gloves and goggles and face shield
shall be worn.

When welding or cutting is

PPE clothing and respirators necessary against


the hazards of asbestos depend upon the
degree of exposure. For detailed information
on the hazards and disposal of asbestos, refer
to local authority guidelines.
Three levels of exposure are defined:

Work in designated Respirator


Zones

Work in designated Asbestos


Areas

Work with asbestos not in a


designated area.
Respirator Zones

For work in Respirator Zones, street clothing


shall be removed and protective clothing that
completely covers all parts of the body shall be
worn. This shall include:

High efficiency dust respirator


fitted with a full face mask
Plastic
or
finely
woven
coveralls close fitting at the neck,
wrists and ankles, with no external
pockets. Coveralls should include
a combined head covering or a
dust hood

5.6 Procedures for Replacement of

Coverall, and Safety Goggles shall be provided


by the Company every year, for field based on
schedule 2-1 or 2-2, meanwhile for field Adm.
based on schedule 5-2, the replacement shall
be every 3 years and for Safety Helmet every 5
years in compliance with the manufactures
recommendation.
a. Replacement Mechanism
Safety Shoes
The Safety Shoes shall meet GM Procedures.
The number of required shoes shall be counted
when they are distributed on field or each area,
and the Field Safety Inspector shall take the
record and submit the list of acceptance to HSE
Gresik, as receipt and three month before due
date, HSE shall make W/O for shoes request.
The Company shall replace any damaged
shoes before the life time, one year for field
offshore & onshore, and three years for field
administration. The Company shall also replace
in case of loss, however it subject to field
investigation that in case of negligence or by
intention the Company shall not be responsible
for replacement (the said person shall bear the
replacement cost).
Shoes replacement must be attached with
Material Request Ticket For PPE that is signed
by GSB Ware House, Head of Gen. Adm. and
HSE Dept. and the old shoes shall be returned
for evidence and disposal accordingly.
To ensure the progress of works, the Company
shall provide spares at least 25% of the total
shoes required to anticipate replacement for
any damages or losses.
Coverall
Coverall, according to the Procedures, the
procurement shall be made every year for field
offshore & onshore based on schedule 2-1 or 22, and every 3 years for Field Adm. based on
schedule 5-2.
The Company shall provide the replacement for
any damages (torn and too much exposed with
oil) during the life time, including in any losses

when the investigation on field finds no


negligence or by intention and the damaged
coverall shall be returned to the Company by
enclosing signed minutes of loss by the said
person describing the reasons.
To ensure the progress of works, the Company
shall provide spares at least 30% of the total
coverall required to anticipate replacement for
any damages or losses.
Safety Helmet
The replacement of safety helmet shall be
made in compliance with the Company
regulation, as follows:
- The life time of 5 years is
expired and the record shall be
taken when it is received. FSI
shall take the record and
submit the list of employees or
contractors who receive them
to HSE Gresik.
- The shells color changes by
long exposure to sunlight or
other chemicals.
- Broken or cracked.
- Calcified surface.
- Any losses are subject to
investigation, and in case of
negligence or by intention the
Company
shall
not
be
responsible for replacement
(the said person shall bear the
replacement cost).
Safety Goggles
The replacement of safety goggles shall be
made in compliance with the Company
regulation, as follows:
- Scratches on the glass that
prevent the user to see clearly
- The glassgetblurred
- Crackedorbroken
- Any losses are subject to
investigation, and in case of
negligence or by intention the
Company
shall
not
be
responsible for replacement
(the said person shall bear the
replacement cost).

APPENDIX 1: LEGAL REFERENCES


Legal and Regulatory requirements for safety in relation to personal protective equipment
are derived from a number of ministerial decrees, Procedures and Regulations. However,
like most workplace health and safety Regulations, requirements for personal protective
equipment in the workplace are defined at a federal level.
Under legislation workers are entitled to:
A safe workplace protected against harmful and dangerous effects that can
result in occupational injuries, occupational diseases or reduced working
capacities.
The right to refuse to do an activity in case of direct danger to the workers life
and health.
Instruction on safe methods, means and skills, provided at the employers
expense
