This is a reference guide containing the highlights of your
employee benefit plans. It has been designed for simplicity
and should not be relied upon in any way as a legal
document.

Always refer to the Plan Document of each benefit plan for


accurate and detailed explanations. Plan Documents may
be found online, or you may request one from your HR
office or from Big Oak Benefit Advisors.

Your Medical Benefits
You will probably use your medical benefits more than any other
benefit. If you elect medical coverage, this plan will protect your
financial security and your access to quality medical care.

Health Savings Account Plan
Here’s how the medical plan works
Single Coverage:
Wilo USA pays the first $1,000 of your medical expenses.
Then you pay up to the next $1,500.
Then insurance pays 100%.

Family Coverage:
Wilo USA pays the first $2,000 of your family’s medical expenses.
Then you pay up to the next $3,000.
Then insurance pays 100%.

Here’s how the bank account works


This plan is called a Health Savings Account (HSA) because you
set up a special tax-free bank account to pay for your expenses.
You receive a VISA debit card; when you incur a doctor’s bill or
buy a prescription or pay a hospital, you pay with the HSA card.
• Wilo USA contributes $1,000 for an employee (Single).
• Wilo USA contributes $2,000 for employee+ (Family).
• You may also contribute your own dollars through payroll
deduction, to the limit: $3,050 for Single and $6,150 for Family.
• Your contributions avoid Federal, State and FICA taxes.
• All contributions are tax-free. The money is yours.
• Your unused money rolls over to next year, still tax-free.
Copay Plan
Please refer to the Health Plan Document for Details

This plan has copays for doctor visits and prescriptions, and a
deductible for the major expenses.

Here’s how the Copay plan works


• You pay a $20 copay for exam and tests at doctor’s office.
• You pay a $20 copay for Preventive Care.
• You pay a $15 copay for generic drugs.
• You pay a $35 or $50 copay for brand name drugs.

• You pay a $500 deductible for other expenses such as:


major tests, hospitalization or surgical procedure.
• After the deductible is met you pay 20% of the remaining bill
up to your total out-of-pocket. Then insurance pays 100%.
• Your total out-of-pocket is $2,000 Single or $4,000 family
★ plus all your copays, which can be substantial.
Wellness Plan

WILO is committed to the well-being of all employees; from time


to time various wellness programs may be instituted.

Here are the details of the Principal Wellness Plan for those who
are covered by the Wilo USA group medical insurance:
• A personal, engaging approach to health improvement.
• Health screenings conducted on-site or by your own doctor.
• Comprehensive Health Risk assessment.
• Confidential reports.
• Wellness consultants.
• Targeted intervention and available behavior-change coaches
for certain high risk health conditions.
Your Retirement Plan
We offer a 401(k) Defined Contribution Plan with matching. It is
designed to respond well to your changing needs during your
working career.

• 401(k): your benefit will vary at retirement according to your


investment choices and results.
• Defined Contribution Plan: Wilo USA will match your con-
tribution 50%, up to 7% of salary. You may contribute more
than 7%; please call for details. See Contacts.
• Eligibility: You may join the plan at any time. You are eligible
for the match after one year of service.
• This plan is managed by the Wachovia Securities.
• Investments: Your account is self-directed. You may select
from a wide choice of options offered through Hartford.
Your Dental Plan
Most people will utilize their dental plan at least twice a year; it
is designed to minimize your out-of-pocket expenses related to
good oral care.

• You may choose any dentist. No network.

• Preventive Care: The plan pays 100% of the cost of cleaning


twice per year, plus the periodic x-rays. No Deductible.

• Deductible: There is one annual deductible of $50 for Basic or


Major Services.

• Basic Services: The plan pays 80% for most services, including
fillings, extractions, root canals and oral surgery.

• Major Services: The plan pays 50% for crowns, bridges, den-
tures and periodontics.

• Annual Maximum: $1,000 per person, per calendar year.

• Maximum Rollover: $250 per year up to add’l $1,000 coverage.

• Orthodontia: The plan pays 50% of the cost of child orthodon-


tia up to the lifetime maximum of $1,000.
Your Vision Plan
Eye care protection for you and your family.
Please refer to the Vision Plan Document for Details

• If you use a VSP provider*:


✓ $20 copay for exam (per 12 months)
✓ $20 copay for lenses (per 24 months)
✓ $20 copay for frames (per 24 months)

• Using a Non-VSP provider**:


‣ Guardian reimburses $46 for exam;
‣ Guardian reimburses $47 to $125 for lenses;
‣ Guardian reimburses $47 for frames.

*Certain limits apply. Please check with your VSP Provider when purchasing materials.
**When using a non-network provider you will need to attach a copy of your receipt to the reimburse-
ment form and mail to Guardian.
Your Group Life Plan
Your group life plan can provide financial security for your survi-
vors if you die while an employee. Your beneficiary will receive
the amount specified below.

• The death benefit is one-times annual salary.


• Additional one-times annual salary for accidental death.
• The plan is paid for entirely by Wilo USA.
Your Voluntary Life Plan
This is a voluntary plan in which you may elect to purchase ad-
ditional life insurance coverage on yourself, your spouse and your
children. This is term insurance: the premiums are low and the
plan is portable.

for Details

• Employee: You may elect to purchase life insurance in in-


crements of $25,000 up to the maximum of $150,000.
• New Employee Guaranteed Issue: At your initial enrollment
only you may elect up to $100,000 of life insurance with no
health questions.
• Spouse: You may elect 50% of Employee amount up to
$50,000 for your spouse in increments of $25,000.
• Spouse Guaranteed Issue: At your initial enrollment only the
first $25,000 is guaranteed issue for a spouse.
• Children: You may elect 10% of Employee amount up to
$10,000 coverage per child. The premium is a flat rate, not per
child.

Cost is based on age and amount.


Your Short Term Disability
Income Plan
If you are enrolled and are unable to work because of an illness or
injury, this plan continues to pay part of your salary. The benefit
plan is designed to help protect your financial security while you
are unable to work.

tails

• The plan pays 50% of your salary, up to $800 per week.


• Benefits begin on the 1st day if you are unable to work if due
to injury, or on the 8th day if you are unable to work because of
illness.
• Benefits are payable for as long as you remain disabled, up
to the maximum of 90 days. If you remain disabled longer than
ninety days the LTD plan takes over.
• This benefit also covers you when you are partially disabled.
• This benefit is paid for by Wilo USA.
Your Long Term Disability
Income Plan
If you are enrolled and are unable to work because of an illness or
injury, this plan continues to pay part of your salary. The benefit
plan is designed to help protect your financial security while you
are unable to work.

• The plan pays 60% of your salary.


• Benefits begin on the 91st day, the day the Short Term ends.
• Benefits are payable for as long as you remain disabled, up
to normal retirement age, usually age 67.
• This benefit also covers you when you are partially disabled.
• This benefit is paid for by Wilo USA.
Long Term Care
This group benefit is designed to help protect your financial secu-
rity in the event that you should find yourself in need of Home
Care, Assisted Living or Nursing Home services.

• Long Term Care is different from Long Term Disability. The


Disability plan pays a percentage of salary while you are out of
work, but Long Term Care pays for assistance that you need at
home or in a facility when you are unable to perform normal
Activities of Daily Living.
• Wilo USA pays for a benefit of $2,000 per month and you
may purchase additional coverage up to $6,000 of total
monthly benefit.
• This plan is portable. You may elect to keep it as long as you
wish, even throughout retirement.
• The plan is guaranteed issue for new hires and the premi-
ums lock in at your age at time of purchase.
Supplemental Plans: Voluntary
We offer several plans to our full-time employees which are de-
signed to “supplement” your core benefits such as major medical
and disability insurance. You pay for these plans if you elect the
coverage, therefore the benefits, if received, are tax-free.

Cancer Plan - AFLAC


• Plan pays you an initial lump sum benefit when a cancer
is diagnosed, and various amounts for each procedure.
Specified Health Event - AFLAC
• Plan pays you a cash benefit if you incur a condition such
as Coma, Stroke, Paralysis, Heart Attack, End Stage Renal
Failure and several more.
Accident Plan - AFLAC
• Plan pays you a cash benefit if you are injured or die due
to a covered accident.
Your Section 125 Pre-Tax Plan
The pre-tax plan gives you the opportunity to reduce your taxable
income by using before-tax dollars to pay for certain expenses.
This saves you money by reducing your taxes.

• Pre-Tax Plan: Premiums - Any premium dollars that you


pay toward your Medical, Dental and certain Supplemental
plans may be “pre-taxed”. This means that those dollars avoid
Federal and State Income taxes and FICA taxes. Most people
will save 25% to 40% or more on their premiums due to the re-
duction in taxes.

The Pre-Tax plan is free.

Go to your custom website to make a change to your benefits or


beneficiaries, or to view your current coverages or premiums.

Vacation
Wilo USA provides Time-off Benefits for rest, recuperation and
family time. Time-off is requested and approved only through
the online Leave system.

Sick Leave
Six days at the start of each calendar year.

Absence of over three days requires signed note from doctor .


• If not excused, absences will not be compensated
• and will count against the employee’s attendance record.

Sick Leave cannot be used toward Vacation time.
