GUIDELINES IN THE CONDUCT OF PERFORMANCE PROGRESS MONITORING, EVALUATION,


AND ADJUSTMENT FOR JUNE-AUGUST 2015

Pursuant to Regional Memoranda Nos. 190 and 324, Series of 2015, this Office hereby issues the guidelines in
the conduct of Performance Progress Monitoring, Evaluation, and Adjustment for June-August 2015.
2.
The Regional Office informs the field of the following schedules of activities related to the conduct of Monitoring,
Evaluation, and Adjustment (MEA) in all governance levelsschool, district, and schools division:

The aforementioned M&E Forms are designed for District consolidation and analysis as bases for DMEA. The
Public Schools District Supervisors and DMET Chair are hereby directed to customize the forms for school and division
purposes, respectively.

Enclosed in this Memorandum is the step-by-step Guidelines in Conducting Performance Progress Monitoring,
Evaluation, and Adjustment.
6.
The team leaders and members of the Regional Field Technical Assistance Teams will visit their assigned
Schools Divisions during the conduct of DMEA Conference on September 1 , 201 5.

Immediate and wide dissemination and prompt compliance with this Memorandum is hereby directed.
