PETITION FOR CHANGE OF FIRST NAME

I, MARIA ESTRELLA VERONICA PRIMITIVA DUTERTE A.K.A. ESTRELLA S.
ALFON, of , of legal age, Filipino, and a resident of #12 Kapayapaan Street, Bangkal, Alaminos
City, Pangasinan, after having been duly sworn to in accordance with law, hereby declare that:
1. I am a petitioner seeking a change in my full name as reflected in my Certificate of Live Birth;
2. I was born on February 2, 1991 at Lourdes Hospital, Alaminos City, Philippines;
3. The birth was recorded under registry number 567890;
4. The name to be changed is from Maria Estrella Veronica Primitiva Duterte to Estrella S. Alfon
5. The grounds for filing this petition are that:
a. I have been using the name Estrella Alfon since her childhood;
b. I have been enrolled in the grade school and in college using the same name;
c. I have continuously used the name Estrella S. Alfon since infancy and all my friends
and acquaintances know me by this name;
d. I have exercised my right of suffrage under the same name.
6. I submit the following documents to support this petition:
a.) Academic records from elementary up to college showing that my name is Estrella S. Alfon;
b.) Birth certificate of my child where Estrella S. Alfon appears as my name.

7. I have not filed and similar petition and that, to the best of my knowledge, no longer similar
petition is pending with any LCRO, Court or Philippine Consulate;

PRAYER
WHEREFORE, premises considered, it is most respectfully prayed of this Honorable Court
that judgment be rendered in favor of the petitioner after judgment;
The petitioner be allowed to change her name from Maria Veronica Primitiva Duterte to Estrella
S. Alfon;
Such other reliefs and remedies under the premises are likewise prayed for.
