AFFIDAVIT

I, Smt. Anju Devi Poddar, wife of Sri Radhe Shyam Poddar, aged about 57
years, by nationality - Indian, by faith - Hindu, by occupation - Housewife,
residing at 67, Dwarik Jungle Lane, Uttarpara, Bhadrakali, Hooghly, West
Bengal 712232, do hereby solemnly affirm and declare as follows:1.

That the actual and correct spelling of my name is Anju Devi Poddar.

That the actual and correct spelling of my name is Anju Devi Poddar
which has been rightly recorded in my PAN Card being Card No.
AFVPP5715P issued by Income Tax Department of India.

3.

That the spelling of my middle name has been wrongly recorded as Anju
Debi Poddar in my 5 Year Kisan Vikas Patra.

4.

That due to some inadvertent mistake my middle name has been spelled
as Debi instead of Devi.

5.

That, Anju Debi Poddar, wife of Radhe Shyam Poddar, and Anju Devi
Poddar, wife of Radhe Shyam Poddar, is self same, one and identical
person who refers to me only and none else.

That with a view to clear up and resolve the confusion/doubts over the
spelling of my middle name which has been ignorantly and/or
inadvertently and/or differently recorded in the 5 Year Kisan Vikas
Patra and for the purpose of necessary rectification/correction/insertion
of my relevant documents and papers, I am swearing this affidavit.

That I hereby declare and state that henceforth I shall use and write my
name as Anju Devi Poddar in all relevant documents and papers.
