Informed Consent

Purpose of the research study: You are invited to participate in a research


study that examines daily work behaviors. We ask that you read this form and
ask any questions you may have before agreeing to be in the study.
What you will be asked to do in the study: You will complete this study
over 10 consecutive work days. You will complete a 1 time background survey
and then 3 surveys per day for 10 working days. We will email you 3 brief
surveys a day, in the morning, afternoon, and evening. Completion of each
survey should not take you longer than 5-7 minutes each. We will need your
email address so that we can email the surveys to you.
Time required. Approximately 3 - 4 hours (total).
Risks and Benefits: There are no direct risks or benefits to you for
participating in the study.
Compensation: You will receive class credit for participating in this study.
Your participation in all parts of this research study is completely voluntary.
You are free to decline to answer any questions or to terminate your
participation at any time. If you should choose to not participate in the study,
you can earn your class credit by completing a research paper assignment as
directed by your class professor (see your course syllabus for more details).
Confidentiality: Your identity will be kept confidential to the extent provided
by law. Any unique personal identifiers, linking a particular individual to his or
her responses, will be recoded or destroyed prior to analysis. Results will only
be reported in the form of group data. You should take your surveys in
private, and not share responses with anyone.
There is minimal risk that security of any online data may be breached, but
our survey host (Qualtrics) uses strong encryption and other data security
methods to protect your information. Only the researchers will have access to
your information on their server.

As part of the study you will be assigned a unique user ID number that will
allow us to relate your responses on the survey to each other. We will collect
identifying information as a means of tracking participation in the study to
assign your credit. Your identifying information will not be linked with your
survey responses, and will be removed from the data once credit has been
assigned.
Your email will be used only to send you the daily reminders about the
surveys, and will not be linked to your survey responses.
Voluntary participation: When completing the surveys, please feel free to
skip any question(s) with which you are uncomfortable. Your participation in
this study is completely voluntary.
Right to withdraw from the study: You have the right to withdraw from the
study at any time. You will receive pro-rated credit for partial participation.
