laws of the Republic of the Philippines. Located at 160 Fernando
Compound Sauyo Novaliches, Quezon City. By its Owner/Manager Mrs.
Liza H. Rual.

Ms. CHERILYN M. PUMARAS a Registered Midwife, residing at Sauyo Road

Novaliches, QC.
Terms and Condition of the owner

Free food and lodging

Philhealth and SSS contribution monthly.
Monthly salary P5000.00
Incentives of P500 every delivery
Stay in and have 2days, day off in 1 month between 8am to 5
am.
FreeHepatitis B vaccine in 3 months.
Free laboratory, medical check and medicine if needed.
1 year contract renewable or as long as both parties and the
management of the clinic are willing to continue this Agreements.
Midwife failed to do her jobs, obligations and slow learner
the managements have the right to decide for the termination
or destroy of this contract for the safety and security of the clinic.

Staff Midwife Duties and Responsibilities

1. Midwife must be wear scrub suit all the time
2. Take down and notes the history of patients vital signs such as
( Leopolds Manuevers, Fetal heart beat, Fetal height, I.E) if
necessary and use Parthograph.
3. Handle patient during labor and delivery up to post partum period.
4. Assessment of the new born ( apgar scoring, vital signs taking)
5. Can inject vita k. And heppa B. Vaccine for the baby
6. Monitoring the vital sign both mother and baby.
7. Clean the clinic if no patient.
8. Must do the recording

9. Midwife must do all the rules and regulations inside the clinic.
10. Midwife must be taking care all things inside the clinic.

Novaliches, QC.

Stay out and have 4days, day off in 1 month
Free Hepatitis B vaccine in 3 months.
Free laboratory, medical check and medicine if needed.

7. Employee failed to do her jobs, obligations and slow learner
the managements have the right to decide for the termination
or destroy of this contract for the safety and security of the clinic.

Utility Duties and Responsibilities

Responsible for the Cleanliness of the Clinic including the Proper Waste
Management & Disposal
