SUBJECT : PE DESIGN FOR SLAB/FORMWORK/SCAFFOLD ERECTION

We take note with serious concern of the recent issue on above subject matter.
In light with the recent accidents involving collapse of formwork/scaffold, the management of CSCEM is
sending out this advisory to remind all the Professional Engineers (PEs) of their obligations in the design,
construction and inspection of formwork structures.
2) Before commencement of work, the responsible person shall conduct a risk assessment in relation to the
safety and health risks posed to any person who may be affected by his undertaking in the workplace.
3) The Professional Engineer who designs the formwork structure shall take, so far as is reasonably
practicable, such measures to ensure that his design can be executed safely by any person who constructs or
uses the formwork structure according to his design. Referring to the Factory and Machinery Act 1994
under regulation (20) part (1)general works;

Formwork and reshores shall be certified structurally safe by a Professional Engineer and shall be
properly braced or tied together so as to maintain position and shape.

Regulation (30) part 4 and (5) beams floors and roof;

Where the floor to ceiling heights exceeds 9.14 metres or where the formwork deck is supported by
shores constructed in two or more tiers, or where the dead, live and impact loads on the formwork
exceed 732,2 kgf per square metre the formwork structure shall be designated by a Professional
Engineer and the specification and drawings shall be kept on the job for use by an Inspector and a
copy of the said design and drawing shall be submitted to the Chief Inspector before work
commences.

Where the formwork structure is designed by a Professional Engineer, he shall be responsible for
the supervision of the construction and the stability of such structure.

Regulation (75) part 1 scaffolding;

Every metal tube scaffold exceeding 40 metres in height and every other scaffold exceeding 15
metres in height shall be constructed in accordance with the design and drawings of a Professional
Engineer. All other metal tube scaffolds shall have their designs and drawings approved by the Chief
Inspector.

4) The Professional Engineers design should include the following considerations, but not limited to:
- Vertical and horizontal formwork components;
- Bracing members;
- Connections; and
- Support on which the formwork rest.
5) The formwork design should encompass all sides, corners and bends supporting the concrete to be cast.
Bracing and coupling details should be provided in the design to ensure clarity.
6) No alteration shall be made or allowed to be made to a formwork structure designed by the Professional
Engineer unless its design and drawings have been reviewed and endorsed by a Professional Engineer.

7) Due to this, JKKP is conducting inspections at all the construction site along with CIDB.

8) The images of formwork collapse and inspection by authorities is as per attached in Appendix 1
