AUTOPSY FINDINGS:
I PENETRATING GUNSHOT WOUND OF CHEST.
-Entrance wound: right side of chest, below and slightly lateral to the nipple.
-No soot, stippling or muzzle imprint.
~Track: through the 5" right intercostal space with fracture of the superior border of the
right 6" rib, through the middle lobe and upper lobe of the right lung, the right atrium,
the aorta and the pulmonary artery, the lower and upper lobes of the left lung, the 3" Ieft
intercostal space laterally, with fracture of the superior border of the 4" left rib, and lastly the
soft tissues of the anterior left upper arm, where the projectile came to rest.
-Trajectory: ‘The trajectory is right to left, upward and slightly front to back.
~Associated findings: Right and left hemothoraces of 1000 milliliters and 400 mil
respectively.
-Evidence recovered: Medium to large size, jacketed, mushroomed bullet recovered from the
soft tissues of the left upper arm.
I. PERFORATING GUNSHOT WOUND OF RIGHT UPPER ARM.
-Entrance wound: posterior/medial aspect of the right upper arm,
-Hemorrhagic track through the soft tissues of the posterior/medial and lateral aspects of the
right upper arm.
-Exit wound: lateral aspect of the right upper arm with surrounding contusion,
-Trajectory: Back to front, slightly downward.
Ill PERFORATING GUNSHOT WOUND OF LEFT FOREARM.
-Entrance wound: below and medial to the posterior left elbow.
Track: through the soft tissues and bones of the left forearm, with fractures and hemorrhage.
-Exit wound: lateral left forearm,
-Trajectory: downward and to the left.
IV. ABRASION OF THE MEDIAL ASPECT OF THE RIGHT ELBOW.

OFFICIAL PRESENT AT AUTOPSY: Detective Drake of the Palm Beach Sheriff's Office.
CLOTHING:
The body presented in a pair of green camouflage pants with a brown belt, two black T-shirts, gray boxer
shorts, two black socks, and two black and white sneakers. The hands are covered with white hand bags, sealed
at the wrists. There is blood, grass and yellow fly eggs on the clothing. The outer T-shirt has the logo “Future
Prez-idents” in the front. Both T-shirts have defects located on the right anterior sides and the anterior and
posterior aspects of the right sleeves. A white metallic ring is on the right index finger with a piece of green
grass wedged between the finger and the ring.
EXTERNAL EXAMINATION: The body presented in a sealed, white body bag with seal number
“1236165”. The bag was opened at 13:06 in the presence of Detective Drake.
This is the body of a well-developed, well-nourished, black male appearing the reported age of 31 years. The
body is 5 foot 10 inches in length and weighs 158 pounds (body mass index 22.7). The body is well preserved
and is warm to touch. Rigor mortis is fully developed in the major muscle groups. Livor mortis is faint, barely
apparent posteriorly. There is blood on the upper extremities and the chest. Colonies of yellow fly eggs are on
the left hand and the right arm.
The scalp hair is black and is arranged in multiple short twists. Facial hairis a small, short, trimmed mustache,
and a short, trimmed beard. The irides are brown. The pupils are equal. The comeae are clear. The sclerae
and conjunctivae have no petechiae, ‘The nasal bones are intact by palpation. ‘The nares are patent. The natural
teeth are in good repair. The oral mucosa and the tongue are free of injuries. ‘The extemal ears have no
injuries. There is piercing of the left earlobe.
‘The neck is symmetric and free of masses or injuries. The trachea is at the midline.
The shoulders are symmetric and free of scars or injuries. The chest is symmetric. The breasts have no
palpable masses. The abdomen is symmetric and free of scars or injuries. The back is symmetric and has no
scars. The external genitalia are those of a normally developed adult man, There are no injuries. The anus is
unremarkable, ‘The penis is circumcised. The testes are palpated in the scrotum. The upper and lower
extremities are symmetric.
EVIDENCE OF INJURY: There is a penetrating gunshot wound of the right chest, a perforating gunshot
‘wound of the right upper arm, and a perforating gunshot wound of the left forearm. A red abrasion ison the left
elbow.
Gunshot of Chest;
Entrance: The entry wound is on the right anterior chest measures 8 millimeters in diameter and has a
marginal abrasion, up to 2 millimeters in width, situated between the 5 and 11 o’clock position. No soot,
stippling or muzzle imprint surrounds the wound of entry. The wound of entry is situated 18-1/2 inches below
the top of the head, 5-1/2 inches right of the anterior midline.

Track: The bullet went through the 5" right intercostal space fracturing the superior margin of the 6" right rib,
entering the chest cavity, with perforation of the middle and upper lobes of the right lung, lacerations of the
ascending aorta and the pulmonary artery trunk, perforations of upper and lower lobes of the left lung, and the
3" left intercostal space and fracture of the superior margin of the left 4" rib laterally. The bullet then entered
the soft tissues of the left arm, where the track terminates.
‘Trajectory: The trajectory of this gunshot wound is right to left, upward and slightly front to back,
Evidence Recovered: A deformed, jacketed, medium to large-sized projectile from the soft tissues of the left
upper arm.
Additional Findings: A 1400 milliliter right hemothorax; a 400 millimeter left hemothorax and a non-
displaced fracture ofthe left humerus.
Gunshot Wound of Right Arm, Penetrating:
Entrance: The entrance wound is a4 millimeter in diameter defect in the ventro medial right upper arm. Ithas
an almost circumferential marginal abrasion up to 2 millimeters in width. There is no soot, stippling or muzzle
imprint around the wound. The wound is situated 17 inches below the top of the head.
Track: The bullet went through the soft tissues of the ventro/medial aspect of the right upper arm, exiting the
iateral surface of the right upper arm.
EXIT: The exit wound is a I centimeter x 1.4 centimeter, gaping defect with numerous marginal tears, situated
17-3/4 inches below the top of the head. No soot, stippling or muzzle imprint is present.
There is a small cluster of deep purplish contusions, approximately 4.5 x 3.5 centimeters, surrounding the
wound between 1 and 6 o’clock.
Trajectory: The trajectory of this gunshot wound is back to front, slightly downward and medial to lateral.
Evidence: Two fragments of projectile were recovered from the wound track.
Gunshot Wound of Left Forearm, Perforating:
Entrance: The entry wound is postero/medial, measures 8 millimeters in diameter and has an up to 2
millimeter wide marginal abrasion. The wound is situated 24-1/2 inches below the top of the head. There isno
soot, stippling or muzzle imprint.
Track: The bullet went through the soft tissues of the left forearm, fracturing the left ulna and exiting the
lateral aspect of the left forearm. The fractured proximal left ulna protrudes from an 8 millimeter abraded
laceration of the dorsal left elbow, slightly above the entrance gunshot wound.

Exit wound: The exit wound is a gaping 3centimeter in length defect with skin tags pointing upward and
‘medially. The wound is situated 26 inches below the top of the head. There are two additional slit-like defects
next to the exit wound.
Evidence: Several fragments of projectile are recovered under the skin next to the exit wound.
‘Trajectory: The trajectory is right to left and downward.
Other Injuries: There is a 1.8 x 1.3 centimeter, red abrasion of the medial aspect of the right elbow,
surrounded by purple contusion.
INTERNAL EXAMINATION: The following excludes the previously described injuries.
BODY CAVITIES: The muscles of the chest and abdominal wall are normal in color and consistency. The
lungs are within their normal anatomic positions. The stemum and spine show no gunshot wound or fractures.
The right and left pleural cavities have no adhesions. The mediastinum is in the midline. The pericardial sac
contains bloody fluid. The diaphragm has no abnormalities. The subcutaneous abdominal fat is 1.5 centimeters
in thickness at the umbiticus. The abdominal cavity has glistening serosa and no collections of free fluid. The
organs are normally situated and pale. ‘The mesentery and omentum are unremarkable,
NECK: The soft tissues and strap muscles of the neck have no hemorthage. The hyoid bone and the cartilages
of the larynx and thyroid are intact and show no evidence of injury. The larynx and trachea are lined by
smooth, pink-tan mucosa, are patent and contain no foreign matter. The epiglottis and vocal cords are
unremarkable. The cervical vertebral column is intact. The carotid arteries and jugular veins are unremarkable,
CARDIOVASCULAR SYSTEM: The heart weighs 290 grams. The epicardial surface has a normal amount
of glistening, yellow, adipose tissue. The heart is ofthe usual configuration, The circumferences of the valves.
are within normal range. ‘The endocardium is tan. The valvular tissues are thin and pliable. The mural and
valvular endocardia have no vegetations or thrombi. The papillary muscles and projecting myocardial muscle
bundles are of normal prominence. The chordae tendineae have no abnormalities. The coronary ostia are in
their usual location and give rise to normally distributed arteries. ‘The coronary circulation is right dominant
with the posterior descending arising from the right coronary artery. ‘The major coronary arteries are free of
atherosclerosis. ‘The cut surfaces of the brown myocardium show no hemorrhage, necrosis or scars. The right
ventricle is 0.4 centimeters in thickness, and the left ventricle is 1.5 centimeters in thickness.
The pulmonary trunk and arteries have no thromboemboli. The intimal surface of the aorta is free of
atherosclerosis. The ostia of the major branches are of normal distribution and dimension. The inferior vena
cava and tributaries have no antemortem clots.
RESPIRATORY SYSTEM: The lungs weigh 225 grams and 205 grams, right and left, respectively. Thereis
a small amount of subpleural anthracotic pigment within all lobes. The pleural surfaces are thin and free of
exudates. The trachea and bronchi are lined by smooth, tan epithelium and havea small amount of mucus. The

RESPIRATORY SYSTEM: The lungs weigh 225 grams and 205 grams, right and left, respectively. There is
small amount of subpleural anthracotic pigment within all lobes. The pleural surfaces are thin and free of
exudates. The trachea and bronchi are lined by smooth, tan epithelium and have a small amount of mucus. The
cut surfaces of the lungs are pink-ted and the lung parenchyma is of the usual consistency. ‘There is no
neoplasm, consolidation, fibrosis or calcification.
HEPATOBILIARY SYSTEM: The liver weighs 1255 grams. The capsule is intact. The cut surfaces are pale
brown and ofnormal consistency. ‘There are no focal lesions. The gallbladder has approximately 0.4 milliliters
of yellow-green bile. There are no stones. The mucosa is unremarkable. The large bile ducts are patent and
non-dilated.
HEMOLYMPHATIC SYSTEM: The thymus is not identified. The spleen weighs 75 grams. The capsule is
slate gray and intact. The cut surfaces are pale. The lymphoid tissue in the spleen is within a normal range.
‘The lymph nodes throughout the body are not enlarged.
GASTROINTESTINAL SYSTEM: The esophagus is empty and the mucosa is unremarkable. The stomach
has 450 milliliters of beige-colored content. The gastric mucosa has no inflammation or ulceration. ‘The rugal
pattem is regular. The duodenum contains bile-stained fluid. The remaining gastrointestinal tract has no major
alterations to external inspection and palpation. ‘The pancreas has no neoplasia, calcification or hemorrhage.
UROGENITAL SYSTEM: The kidneys weigh 105 grams and 110 grams, right and left, respectively. The
capsules strip with ease. The cortical surfaces are smooth and diffusely pale. The cut surfaces show well-
defined corticomedullary junctions. There are no structural abnormalities of the medullae, calyces or pelves.
The ureters are patent, The urinary bladder has approximately 50 milliliters of yellow, clear urine. The mucosa
is unremarkable.
‘The prostate gland is not enlarged. ‘The cut surfaces are unremarkable. ‘The testes have no masses or
hemorrhages.
ENDOCRINE SYSTEM: The adrenal glands have a normal configuration with the golden yellow cortices
well demarcated from the underlying medullae. The thyroid gland has no gross alterations. The pituitary gland
is unremarkable.
MUSCULOSKELETAL SYSTEM: Aside from injuries, the musculoskeletal system is unremarkable.
CENTRAL NERVOUS SYSTEM: The scalp has no hemorrhage or contusions. The calvarium is intact.
There is no epidural, subdural or subarachnoid hemorrhage. The brain is of a normal convolutional pattern
and weighs 1510 grams. ‘The meninges and cortical surfaces of the brain show diffuse pallor. There is no
uneal or tonsillar herniation. The cerebral arteries are free of atherosclerosis and patent. The cut surfaces
of the brain have normal relations of gray and white matter. There are no intraparenchymal hemorthages or
evidence of neoplasm. There are no fractures of the base of the skull. The dura mater is free of stains and

On October 18, 2015, at 08:18 hours, I received a call from the Palm Beach County Sheriff's Office (P.B.S.0.) |

Dispatch Center instructing me to contact P.B.S.O. Det. A. Pfeifle (ID# 9585) regarding a death notification. 1
contacted the number provided and reached P.B.S.0. Det. S. Tobey (ID# 7250), who advised an individual tentatively
identified as Corey Tones, B/M, D.O.B. 02/03/1984, had been shot by an officer and pronounced dead on scene tthe
southbound Interstate 95 (S.R. 9) exit ramp to PGA Boulevard in Palm Beach Gardens, Florida (P.B.S.O. Case# 15-
133920). ‘The incident was reported at 03:15 hours on October 18, 2015. Initiated my response to the scene at 08:32
hours ater informing Det. Tobey would respond.
L arrived on scene at 09:10 hours on October 18, 2015 and was met by individuals from the Palm Beach County |
Sheriff's Office, including Det. Tobey, Det. Pfeifle, Det. K. Smith (DE 6655), who was the lead detective, Sgt. C. |
Karpinski (ID# 6682), Crime Scene Investigators K. Thomas (ID# 7144), J. Chehab (ID# 7852), and E. Patrick (ID#
8120), as well as Chief Investigator William Fraser and Assistant State Attomey Aleathea McRoberts from the Palm =|
Beach State Attomey’s Office, and several individuals from the Palm Beach Gardens Police ae |
(P.B.GP.D.). I was informed a P.B.G.P.D. officer, Officer N. ‘Raja, wearing “plain clothes” driving an unmarked |
‘van was patrolling the area following recent vehicle burglaries at a nearby hotel when he observed a S.U.V. stopped
fon the exit ramp’s shoulder. Officer Raja pulled in front of the S.U.V. and got out of his van after not seeing anyone
inside the S.U.V. According to Officer Raja, a male then exited from the S.U..’s front driver door and stated, “I’m
good, I'm good.” The individual pulled outa firearm and Officer Raja identified himself as police and told the |
individual to “drop” the gun. The male ran after discharging the gun and Officer Raja fired his Glock mode! 27 .40 |
caliber pistol at the male, who ran northbound along a grassy area flanking the exit ramp. The officer stated he fired
again while on the ramp after the male turned as he was running with the gun in his hand and pointed it atthe officer.
1¢ officer then used his cell phone to call 911 and P.B.G.P.D. officers and fire rescue responded. ‘The male was
discovered prone on a grassy slope, rolled to a supine position, checked for weapons and vitals, and pronounced dead
on scene, ‘cones were placed during a reenactment performed by Officer Raja prior to my arrival to mark his
approximate location when he discharged his weapon. Orange flags marked the location reported by Officer Raja of
the decedent as he ran through the grass. It was nighttime at the time of the incident and I was informed three of the
four bulbs in a streetlight above the two vehicles had been illuminated. Six spent casings were located and Officer
Raja’s firearm, which had been collected, had one “live” round in the chamber and three “live” rounds in the
ine, A firearm was discovered in the grass near the ramp. I notified Associate Medical Examiner Dr, G, Juste
of the Palm Beach County Medical Examiner's Office while on scene.
Scene Observations:
The scene is located at the southbound $.R, 9 exit ramp to PGA Boulevard in the city limits of Palm Beach Gardens,
Florida, The aea surrounding the scene was secured by law enforcement prior my arrival. Tis daylight hazy, and
the ambient temperature is 77°F when I arrive, S.R. 9 is a paved roadway running north and south. PGA Boulevard
runs east and west. The one way paved exit ramp to PGA Boulevard bifurcates from the west side of southbound S.R.
5 and grades downward, praliel io SR. 9, approaching PGA Boulevard to a traffic signal atthe south end of the
ramp. ‘There are four paved lanes with two lanes for traffic traveling east at the end of the ramp and two lanes for
traffic traveling west at the end of the ramp. A gray 2008 Hyundai Santa Fe $,U. V. and a white 2004 Ford E-250 van
are parked beneath a streetlight on the westbound lanes near the end of the ramp. ‘The Hyundai is on the outside
shoulder with the front pointing south, the rear pointing north, and the front driver door open. ‘Two bottles of motor
oil are on the ground nea the font passenger side and an unknown iquid is onthe ground beneath the font bumper. |
The key in the ignition switch is turned to “ACC” and clothing, a bottle of motor oil, personal items, and drums are |
inside the vehicle. The glove boxis open, The Ford is ‘on the westbound lanes a short distance south of the |
Hyundai and is perpendicular to the S.U. V. with the front pointing west and the rear pointing east, Two orange cones
Page 1 of 2with “1” and “2” are on the ground between the two vehicles. The van’s front driver door is open and the vehicle is
running with the headlights on. A police vest is in the center console, I did not disturb or remove any items from
either vehicle. No obvious bloodstains are observed on the pavement. On the outside westbound lane, northeast of
the Hyundai, spanning 8 distance of sis to seventen fee from the SUV. accarding tothe Leica Scan Station ase
scanner used by PBS. are tres yellow crime scene placards numbered “I,” “2,” and “3” marking three “spent”
casings headstamped “WIN 40 SW", On the outside westbound lane spanhing a distance reported by the Leica
ScanStation of forty six to fifty three feet north of the S.U.V., three yellow placards with “4,” “5,” and “6” mark three
“spent” casings headstamped “WIN 40 S&W”. An orange cone with “3” is south of the aforementioned casings and
an orange cone with “4” is north ofthe casings. There isa metal guardrail ining the west side of the ramp, Seventy
feet north of the S.U.V., in the grass at the base of a sign west of the guardrail, is a Jimenez. Arms J.A. .380 cal pistol

rounds in the chamber. There are thre orange lags several fet northwest ofthe sign inthe grasy ditch ofa swale
flanking the west side of the ramp. West of the guardrail and one hundred twenty six feet north of the sign is a tree
with a defect on the south/southeast side near the base.
‘A deceased Black male is lying supine on the grassy west slope of the swale west of the aforementioned tree and a
distance of one hundred twenty four feet northwest of the sign as reported by the Leica ScanStation. The decedent’s
identity is confirmed as Corey Jones, B/M, D.O.B. 02/03/1984 by fingerprints submitted at the scene by P-B.S.0.
through Rapid ID, His head and upper body point northeasVeast Both arms are behind hs back and bent atthe
elbows. The decedent's left leg is bent at the knee and his right leg is stretched straight out towards the
west/southwest. The decedent's head is tilted back and tured to his right (north). He is clothed in two bloody black
Tshirts, which ae pulled up exposing his chest, camouflaged cargo pans lac belt, gray drawstring shorts black.
socks, and black and white sneakers. There is @ hole in the right sleeve and on the right side of both T-shirts. An
electrocardiographic pad is on each side of his abdomen, his right chest, and his left arm. Partially dried red liquid
that appears to be blood is on his chest, abdomen, and on the grass next to his right side. ‘The decedent has a defect in
his right chest, defects and a fracture of the lower left arm, two defects in the upper right arm, and an abrasion on the
right elbow. His arms are bloody and dirt adheres to his hands. A white metal ring with a blue stone is on the index
finger of his right hand, Ants, flies, and insect eggs are on the decedent’s body and clothing, The Palm Beach
County Medical Examiner's Office Homicide Protocol is followed. ‘The decedent is placed by Elite Removal
Services personnel on top of a new body bag lined with a new white sheet that has been spread out on the ground. I
placed each of the decedent’s hands in a new white hand preservation bag, taped both bags around the decedent's
wrists, and wrote the date and my initials on the outside. Upon further cursory examination, I note the decedent is
ool to the touch, rigor is full, and fixed lividity is on his back. Red liquid and possible abrasions from insect activity
are beneath his right eye, What appears to be emesis isin his mouth. A small red droplet is in his left ear. Brown
discoloration is on the anterior knees of his pants. I search the pockets of his pants and remove five one dollar bills
and four quarters, all in United States currency, and a debit card issued to Corey Jones from the front left pocket, one
quarter from the right cargo pocket, and a wallet containing one ten and one twenty dollar bills in United States
Gurrency and two Florida divers licenses issued to Corey Jones and one quarter fom the right font pocket. All of
the items from the decedent's pockets are tured over to Investigator Chehab at the scene, Once my cursory
examination is complete, the body bag is zipped closed and sealed with a security tag (tag# 1236165) I inscribed with
the case number, my initials, and the date. ‘The decedent was removed from the scene by Elite Removal Services
personnel and transported to the Palm Beach County Medical Examiner's Office.

The Palm Beach Gardens Fire Rescue report, Incident# 15-008047, was obtained and indicates the decedent had no
Vitals when assessed at 03:34 hours on October 18, 2015.
The decedent’s father, Clinton Jones, was notified by law enforcement on October 18, 2015.
At 11:15 hours on October 19, 2015, I spoke by phone with the decedent’s sister in law, Denise Jones, who advised
the decedent resided with her and her husband (the decedent’s brother), Clinton “CJ” Jones Jr., in Lake Worth,
Florida. The decedent was never married, had no children, and his mother was deceased. He played drums in a band
and informed his brother by phone at 02:50 hours on October 18, 2015 he was having vehicle trouble while on his
way home from a show.
