AFFIDAVIT OF LOW INCOME

I, WILMA V. LADOR, of legal age, Filipino citizen and a resident of Brgy.

___________, Tukuran, Zamboanga del Sur, Philippines, after having been duly
sworn to in accordance with law hereby depose and state THAT:
1. I am a solo parent and the mother of XAVERA GRAXIA L. SINGSON, a high
school graduate who is applying for a CHED Scholarship Program;
2. I and my daughter live with my parents and rely greatly for support from them
who are likewise low income earners;
3. My annual income of FIFTY THOUSAND PESOS (P50,000.00), derived from
intermittent jobs, is so meager and only enough to cover our daily needs;
4. I executed this affidavit to attest to the veracity of the above and for whatever
legal purpose this may serve.
IN WITNESS WHEREOF, I have hereunto affixed my signature this 13th day of
