2.1

To determine existing and to fix pavement level, cross fall and alignment
shall be carried out.

3.0 TEMPORARY TRAFFIC DIVERSION AND CONTROL.

3.1

Traffic Management Plan shall be submitted for approval to ensure


traffic flow is not much disrupted.

3.2

Diversion of road shall be constructed to the most suitable angle of


entry, adequate width, horizontal curvature and level that would
constitute a safe flow.

3.3

The speed limit for the traffic along the diversion shall be as requirement.

3.4

The diversion shall be well sealed with premix and to be maintained regularly.

3.5

All diversion shall be adequately furnished with the necessary signages,


barriers, reflective strings, barricades, night illumination blinker, beacons,
markers and traffic controller as and when necessary. The diversion and
approach road shall also be provided with proper road marking in
accordance to Arahan Teknik ( Jalan ) 2C/ 85 .

4.0 SURFACE DRAINAGE
4.1

Prior to any pavement work, all drainage system shall be present and
functional.

4.2

Permanent subsoil drains, ( if any ) must have already been installed to


ensure firm foundation and any localized soft spot shall be identified and
rectified in advance.

5.0 PREPARATION OF FORMATION


5.1

Prior to preparation of formation or subgrade, all foundation and


earthwork must have been completed up to the level of tolerance as
specified in the specification. Filling or cutting is normally done up to +/ 300 mm from the formation level, after which it is considered as
formation of subgrade level.

5.2

Excess material shall be removed to facilitate the works. The material


shall be imported from approved source of borrowed pit and shall satisfy
all suitability requirement as well as the CBR > 5%.

5.3

The compaction shall be checked by the Field Density Test 95% the
Relative Compaction.

5.4

No heavy vehicles shall be allowed over the prepared subgrade. Where


necessary, a protective layer shall be placed over the subgrade and to
be removed when no longer needed.

5.5

Subgrade shall be developed along the full width of the carriage way and
compacted in longitudinal direction from the outer edge to the inner edge
and shall be from the lower end to the higher end in the super-elevated
areas.

5.6

Any material that found unsuitable, to be removed and replaced with


imported material.

5.7

The line and level shall be control by dipping pegs, which indicated the
offsets and levels.

6.0 PLACING OF SUB-BASE AND ROAD BASE


6.1

SUB BASE
6.1.1
Prior to laying any sub base material, the under laying
subgrade shall have been shaped and compacted as per
requirements.

6.1.2

Material specification requirement for lower sub base ( CBR


Value > 30 and gradation Limit, table 4.1 ). Random sampl
shall be taken from site to comply with the specification.

6.1.3

Suitable spreading plant such as motor grader and


backpusher and compaction shall be carried out using suitable
compacting plant.

6.1.4

Cleanliness and uniformity of moisture and prevention of


physical segregation of the material laid shall always be
ensured through the used of a box spreader or prudent used
of the motor grader, water tanker, compactor combination.

6.1.5

The material shall be done in a single lift. If the required


thickness is more 200mm, the placing, spreading and
compacting of material shall be done in two layers.

6.1.6

Once the required 95% Relative Density is achieved, the level


shall be checked using dipping pegs at 12.5 m interval to
ensure the correct cross fall and adequacy thickness is
achieved with the tolerance + 10mm / - 20mm.

6.1.7

Compaction requirement
6.1.7.1
Compaction is not less 95% of the maximum
dry density as determined by BS 1377 test
(4.5 kg rammer method )

6.1.8

Field Control

6.2

Trial compaction shall be carried out to excess


the suitability of the compaction plant and to
establish the rolling pattern required to
achieved the specified compaction
requirement.

ROAD BASE
6.2.1

Prior to placing the road base material, the underlying


subgrade or sub base shall have been shaped and
compacted.

6.2.2

Upon approval of the preceding sub base layer, the road


base shall be in layers having compacted thickness not less
than 100mm and not exceeding 200mm.

6.2.3

Suitable spreading plant such as motorgrade or backpusher


shall be used and as well as for compaction activity.

Field Control
These shall be carried out to assess and determine the rate
of spread. A tray will be weighed, measured and will be put at
the middle of area to be sprayed. Hence, the rate of the
spread will be determine as follows :

7.0 ASPHALTIC CONCRETE BINDER COURSE


7.1

Works shall commence on site upon approval of design mix and


acceptance of previous prime layer.

7.2

The surface to receive the asphaltic concrete binder course shall be free
of all dirt, loose material and water ponding.

7.3

The approved mix shall be delivered to site by tipper trucks covered with
tarpaulin to prevent heat lost.

7.4

The asphaltic concrete shall be laid using Vogele Super 1502 paver or
equivalent in single layer.

Compaction shall be carried out using specified or equivalent type of


compactors and the rolling pattern shall be as per established during trial
lay from the lower to the higher side of the carriage way. The minimum
rolling temperature is 110C.

7.8

The surface shall be finished to the grade and line as required. Dipping at
12.5m intervals shall be carried out and to check that the tolerance of
5mm is achieved.

7.9

COMPACTION REQUIREMENT
7.9.1

The compacted Asphaltic Concrete Binder Course shall


achieved 95% - 100% of Marshall Density.

7.9.2

Coring shall be carried out to determine Marshall Stability Test


and the compaction, 24 hours after laying.

7.10 FIELD CONTROL


7.10.1

These shall be carried out to access the


suitability of the mixing, laying and compacting
plant and to establish the sequence of the
laying operation.

8.0 ASPHALTIC CONCRETE WEARING COURSE


8.1

Works shall commence on site upon approval and acceptance of the


asphaltic concrete binder course.

8.2

Apply tack coat on the prepared surface. The full width of surface to the
treated shall be swept using a power broom followed by a compressed
air blower to remove all dirt, dust and loose material.

8.3

Tack coat of approval bitumen emulsion ( MS 161 ) shall be applied as


per procedure on the prepared surface prior to laying the wearing
course.

8.4

The asphaltic concrete wearing course shall be laid using paver in single
layer and the temperature of mix shall not less than 125C prior to
unloading from truck into the paver.

8.5

Backcasting shall be carried out if the laid surface is open-textured and


uneven size of aggregates removed before rolling.

8.6

Construction joints in a layer of bituminous mix shall be offset from those


in any immediately underlying bituminous layer at least 100mm for
longitudinal joints and at least 0.5m for transverse joints.

8.7

Compaction shall be carried out using specified compactor and rolling


pattern shall be as establish during trial lay from the lower to the higher
side of the carriage way with 120C rolling temperature.

8.8

The finished surface shall be to grade and line as required. Dipping at


12.5m intervals shall be carried out to check that the tolerance of
5mm is achieved.

4.0 METHOD STATEMENT


4.1 SETTING OUT
4.1.1 The land surveyor will identify the Right of Way boundary and location
of the piling point.
4.1.2 Offset for the piling point, finished road level, cut off level will be
provided at the peg allocated for the piling point.
4.2 SITE CLEARING
4.2.1 The site clearing will be performed at the allocated location.

4.2.2 The opening as a mean of access to the location will be constructed


and filling and cutting of the OGL are required until the difference in
ground level is appropriate for machineries and transportation
4.2.3 The location of machineries, the opening, the material will be arrang

4.3 CONSTRUCTION OF THE WORKING PLATFORM
4.3.1 The working platform is prepared as per cut off level reference.
The excavation will take place and the leveling of the area is performed
until it reaches a satisfactory perimeter for the bored piling machine
maneuver.
4.4 INSTALLATION OF SHEET PILE
4.4.1 The access and platform for vibro hammer will be prepared by means
of leveling and cut and fill method
4.4.2 The vibro hammer will install the sheet pile as per diagram

4.4.3 Previously the design calculation for sheet piling had been worked ou
as a reference for the installation of sheet pile work

4.5 COMMENCEMENT OF BORED PILING & SUB STRUCTURE WORK


4.5.1 Once the sheet pile has been installed, the leveling work continues
until it is appropriate for bored piling machine to operate on the platform

4.5.4 The sheet pile will be transferred to the another working platform and
installed there as per diagram
4.6 SHEET PILE & STEEL PLATE DESCRIPTION
4.6.1 The sheet pile to be used is Type SP 3A (refer to the attachment)
4.7 TIDE LEVEL
4.7.1 The highest tide level and lowest tide level is
recorded as per table below:

4.7.2 For detail references on the tide level, the full table can be obtained
from Jabatan Meteorologi Malaysia and Jabatan Pengairan dan
Saliran Malaysia. The record shown in table is based on actual level
measured on the site in respect to sea water level.

5.0 SAFETY REQUIREMENT


5.1 The existing traffic flow will be monitored by traffic supervision team
and mobilization and demobilization of machineries are not allowed
if the traffic supervision team is not available.
5.2 Only experienced personnel are allowed at working area.
5.3 Site personnel must be briefed and be well versed with all site safety
measures.
5.4 Site personnel must be in proper safety working attire e.g. Safety boots
and helmets must be worn at all times.
5.5 Site safety member should be available at all times and must be able to
respond correctly to emergency
5.6 Proper and adequate provisions of Temporary Signage and Barricades
at diversions and haul road as necessary.
6.0 AUTHORITY REQUIREMENT
6.1

The contractor will issue to the local authority involved such as JPS a
liability letter that will transfer the responbility of damage to the
surrounding area due to the work at site to the contractor.

4.0 METHOD STATEMENT


4.1 SETTING OUT

4.1 SETTING OUT
4.1.1 The land surveyor will identify the exact location of pile cap and mark

4.2.1 The surface of the base will be cleared from any debris and levelled
to ensure the flat and conceivable surface for lean concreting.

to ensure the base is ready for such work.

4.3 REBAR & FORMWORK INSTALLATION
4.3.1 Upon completion of bored piling the depth of excavation from ground
level to lean concrete i.e. : 50mm below pile cap soffit determine by the
surveyor.
4.3.2 Preparation of pile head by hacking to the required level for starter bar
extended into pile cap.
4.3.3 Prior to lean concrete, the pile cap dimension and soffit level shall be
marked by the surveyor.
4.3.4 The lean concrete preferably be 100-150mm larger then the size of p
cap and mark pile cap dimension prior to install rebars and formwork.
4.3.5 The bar bending schedule will be submitted in advance to the consul
for approval, where some of the rebars are custom cut. The size type
member, spacing and lap length shall be accordance to the drawing.
4.3.6 Concrete spacer shall be provided for adequate spacing and cover the
rebars are free from dirt or grease and in a proper storage.
4.3.7 All starter bar member, size, type and lap length are installed according
to the drawings.
4.3.8 The formwork are cut in size and dimension using waterproof plywoo
and shall be applied with mould oil to enhance the finish surface and to
prevent adhension of concrete.

4.3.9 The formwork shall be so erected to comply with the following


requirement ;
4.3.10 a) Conformity of dimension within allowable tolerance.
b) Strength and rigidity of formwork
c) Sufficient propping and bracing to the formwork support.
d) Tightness and sealing of joint from loss of grout.
4.3.11 Place the rebars in position according to size and spacing and properly
tied with wire.
4.3.12The process of formwork dismantling will be done soon after the curing
period has achieved without shock, disturbance and damage to the
structures.
4.4 CASTING
4.4.1 The qualities of cement, fine aggregates and coarse aggregates as
defined from the approved sources and the consultant shall be afforded
all reasonable opportunity to do the inspection and to carry out relevant
test comply to the specification.
4.4.2 Design mix and trial mix to the appropriated grade of concrete shall be
submitted for approval prior to commencement of concreting activity.
4.4.3 Prior to the commencement of work, the following item shall be carried
out:
a) All plant and equipment are adequate in place.
b) Proper access to point of discharge
c) Arrangement for protection against rain and water.
d) Compliance to the safety requirement.
e) Finished levels are clearly marked for reference.
4.4.4 Concrete shall be deposited in horizontal layer to a compacted depth not
exceeding 450mm by using vibrating poker, Until expulsion of air has
practically caused but would not cause over vibration that will lead to mix
segregation.
4.4.5 Concrete shall not be dropped from height exceeding 1.5m unless tremie
pipe or chute are used.
4.4.6 Curing shall be carried out after the compaction to the exposed surfaces
to keep the concrete continuously damped. Methods of curing may vary
subject to the practicality.

VIBRATION MONITORING

1.0 Introduction
Vibration monitoring shall be carried out at all such locations where it is
required to access the effect of induced vibrations due to any dynamic
loading. A standard vibration monitoring system shall carry out this
assessment of vibration. This equipment should be able to evaluate the
parameters of vibrations in all three planes i.e. Longitudinal, Transverse and
vertical. Construction process like pile driving, blasting, soil compaction
induces vibrations that are at times above permissible limits. Permissible
limits of vibrations shall be as defined by relevant compliance code.

The method adopted shall measure peak pulse velocity, peak acceleration and
peak displacement in transverse, longitudinal and vertical direction for a
particular frequency. Every structure has its own frequency and unless the
induced vibration frequency does not match with this resonance frequency the
structure can sustain vibration safely. However vibrator forces above limits
specified in compliance code are detrimental to the health of the structure
and action contributory to such vibration should be immediately stopped.

2.0 Scope of Works


The vibration monitoring will be conducted for 8 selected locations at the
project site, Lebuhraya Pasir Gudang - Johor Bahru
The objective of this monitoring is to measure the induced vibrations for use
in vibration impact assessment and/or for compliancee verification during
construction stage of the proposed project based on the Department of
Environment guidelines.
The sampling points will be taken at the nearest constructed bridge and the
selections will be agreed by client. The vibration level readings were taken for
a consultant/S.O's decided duration at each selected location during piling and
these readings will provide data of the vibration level generated from the
surrounding of the selected points.

Appendix B

3.0 Test Equipment


The instrumentation used was the Instantel Model Minimate Plus. The
instrument captures the required data and stores in the memory for later
retrieval via computer software or direct print out. Monitoring data was taken
on the ground or structure of the locations. For ground vibrations, the
instrument measures the vibrations in three orthogonal axes, namely
transverse, vertical and longitudinal vibrations. transverse ground vibrations
agitate particles in a side to side motion. Vertical ground vibrations agitate
particles in an up and down motion. Longitudinal ground vibrations agitate
particles in a forward and back motion progressing outward from the source of
vibration

The parameter that is used in the assessment of vibration is based on the


vectorial sum of vibration in the three orthogonal axes, termed peak particle
velocity (mm/s ppv)

4.0 Methodology
a) Conduct sensor check using internal check facilities prior to measurement. Set
up the Vibration Meter by mounting the tri-axial sensors in the soil, pointing to
the sources of vibration.
b) The location of the sensor should be installed near the bridge structure. There
should be no obstacles along side the perimeter involved. The radius shall not
beyond 200 meter from the sensor's location and point of observation.

c) Three vibration readings will be taken simultaneous which is vertical,


transverse and longitudinal.
d) After certain predetermined duration the vibration readings will be stored in
the memory of the vibration meter which can be retrieved for data processing
using vibration software.
5.0 Data Processing
The test report shall consist of
i) Site name, client and test agency
ii) Distance from activity.
iii) Location of monitoring.
iv) Peak particle velocity, peak acceleration, and peak displacement in all
three planes.
Specific job objectives need to be addressed during the interpretation and
result evaluation. Any peculiar findings shall be highlighted and reported
during the study. Clear statement of limitation and difficulties faced during
various stages of testing shall also be reported.
6.0 Reporting
The test report shall consists on:

7.0 Limitations
The test cannot provide data about the maximum vibration that can occur as
the test provides result for the loading imposed on the location during testing
procedure. This test enables evaluation of more specific strength parameters
only when used in conjunction with other tests. Location and time specific
damping can affect the results of the test. Displacement evaluated and
maximum frequencies are evaluated during separate tests and if the loading
is not same the result could vary.

8.0 Acceptance Criteria


MS codal provision provides for acceptance criteria for bridge and its
dependant on the use of the bridge. This should be kept in mind when
evaluating the results of the test.

1.0 GENERAL
Structures such as bridges and tunnels may be required to be
monitored for horizontal and vertical displacement. It is important
to be able to accurately measure the amount of displacement at
predetermined locations over a period of time.
All survey work associated and undertaken for the monitoring of
structures must be sufficiently accurate to detect horizontal and
vertical movement of + or - 2mm
2.0 SURVEY DATUM
All co-ordinates supplied shall be as per setting out drawings
provided at the latest unless otherwise specified.
3.0 SURVEY CONTROL
Survey control for the monitoring of structures should be
established so that the integrity of the monitoring data can be
guaranteed. 4 points on each structure will be taken into
consideration. The actual co-ordinates and level will be recorded.

APPENDIX A

4.0 SURVEY EQUIPMENT


All instrumentation shall be calibrated on any certified baselines
prior to the commencement of work and be checked and adjusted
for horizontal and vertical collimation.
The instrument will have been serviced by an authorised agent for
that make of instrument within the last 12 months.
5.0 METHOD OF SURVEY
Times of extreme weather (heat and wind) should be avoided. Care
should be taken to avoid monitoring during periods of high traffic
volumes as this may affect the survey results
All targets are interchanged and tribrachs must remain fixed during
observations (forced centering), thus minimizing plumbing erros.
All results should be recorded electronically in the correct format to
allow the reduction of angle sets and distances.
Horizontal Survey
The coordinates for each eight points will be compared weekly and
the differences will be recorded as the horizontal displacement.

4.0 METHOD STATEMENT


4.1 SITE PREPARATION
The platform shall be prepared to the required platform level and it
shall be reasonably firm, flat and accessible. Proper drainage
systems shall be provided and a pump sump shall be constructed, if
necessary. A proper access road shall be established prior to the
construction works to allow un-obstructed flow of traffic for
concrete trucks during bore pile casting and for exporting of bore
pile spoil (excavated earth). During the commencement of work, a
steel plate will be placed over the platform to ensure the stability of
the boring plant of top of the platform.
4.2 ADDITIONAL S.I
Before commencement of any boring works, the S.I data must be
obtained for each point involved to determine the hard layer
(N>50). This data will be required to ensure the drilling progress
works well without any obstruction. Modification to the drilling
procedure will be decided once the hard layer is determined.
4.3 SETTING OUT
The contractor shall provide a surveyor for setting out of the pile
points. 2 offset peg will be prepared for each pile points. The pile
points should be set-out as shown in the Construction Drawings.
Attention shall be paid for raking piles and shall be set-out in
accordance to the inclination required in the Construction
Drawings.
4.4 BORING PLANT
The boring plant intended to be used for the site is a Mechanical
Boring Rig Model CMV TRM 35/21 mounted on a 38RB base crane or
equivalent. A telescopic kelly bar of external dimension 400mm
diameter and 4 x 12 metre length is mounted on the boring plant.
The lower end of this Kelly bar is adaptable to other drilling tools,
e.g augers, coring buckets, cleaning buckets, etc.
Before any commencement of work, documentation such as boring
plant catalogue, PMA and operator certificate will be submitted to
client for approval.
4.5 POSITIONING OF BORING PLANT
The boring plant will be moved to the pile point intended for
excavation. The kelly bar mounted with an auger is placed just
above the pile point. The verticality of the kelly bar shall be
checked by means of a spirit level. Adjustment should be made so
as to enable the kelly bar to be as vertical as possible. Then the
position of the auger is checked against the pile point. Adjustment
shall be made so that the centre of the auger is nearest to the pile
point. The auger is then lowered and boring operation commences.
Reasonable care shall be taken so that the pile position and
verticality are constructed within the specified tolerance. The
maximum permitted tolerance shall be 75 mm from the pile
position and the verticality shall not exceed 1 in 75.

CONSTRUCTION OF BORED PILE

4.6 BORING
Boring commence after the position of the boring plant is checked.
Usually, an auger of the required size is used to excavate the hole.
During excavation, the hole shall be observed for its stability. If the
excavated hole is collapsible, a temporary casing of the same size
shall be installed using a vibro-hammer, to prevent collapse.
Excavation of the hole then continues until the required depth.
When rock is encountered during the excavation (usually observed
by the difficulty in advancing the excavation) other drilling tools
may be utilised. A rock auger, coring bucket, chisel or a
combination of these tools may be used to break up the rock.
After reaching the required depth, a cleaning bucket shall be used
to clean up the bottom of the borehole. A joint measurement of the
depth shall be made before lowering of the reinforcement and
concreting.
4.7 REINFORCEMENT
Reinforcement for the bored piles shall be pre-fabricated in the
reinforcement yard (near the piling platform) and lifted to the
borehole for placing, using a service crane. After fabrication by the
bar benders, the helical links will be tack welded to the main
reinforcement. Laps are also to be welded sufficiently so that they
remain intact during transportation and handling. 100mm diameter
round spacers will be welded to the reinforcement to provide the
cover to ther reinforcement against the soil during concreting.
4 numbers of sonic logging pipes with 50mm diameter and 6 meter
length will be tack welded to the main reinforcement. Each length
will be jointed by another 6 meter length sonic logging pipes until it
reaches the bottom of the hole. The opening of sonic logging pipe
must be located exactly above the cut-off level (on the same level
as platform level) and be cleared from any debris or blockage by
any means necessary.
The reinforcement will be lowered into the completed borehole,
prior to concreting, by means of a service crane and to be adjusted
to the correct level.
4.8 CONCRETING
Concreting shall commence immediately after the reinforcement is
placed.
Concrete shall be supplied by ready-mixed truck to the pile
positions. The concrete slump to be used shall depend on the
method of concreting, as below.
The tremie pipe shall be extended to the bottom of the excavation.
The tremie pipe shall at all times be embedded in the concrete and
shall be withdrawn after completion of the concreting process. No

Permanent casings are to be installed using a vibro-hammer.

4.4 REINFORCEMENT - PLACEMENT
a) The engineer shall direct and supervise the personnel responsible
for the placement of rebar in accordance with all related designs,
specifications, conditions of contract and associated procedures.
b) All reinforcement shall be securely placed to ensure against
displacement during the placement of concrete. Provide the

j) On no account shall reinforcement be heated to facilitate bending.


Reinforcement shall not be re-bent unless approved by the
Engineer.
k) Reinforcement in structures shall not be welded without the
approval of the Engineer.
l) The supervisor shall also ensure that all required builder's work in
connection with M&E requirements including openings, conduits,
block outs, puddle flanges etc. are installed before placement of
the top bars.
m) Carry out inspection on reinforcement placed as per Pre-Concreting
Checklist. Bar bending schedule shall be submitted before
concreting.
4.5 ON SITE FABRICATION & FIXING OF FORMWORK
a) The surveyor will first set up a reference line offset from the
nearest TBM or from a previously constructed structure with
established survey data.
b) The subcontractors shall determine the side line of the structure of

offsetting from the reference points, established by the surveyor


using string lines and measuring tapes.
c) Before installation of formwork, the surveyor shall survey the level

of the lean concrete and the Engineer shall ensure that the base is
clean and level.
d) For horizontal structures, one side of the form shall be erected
initially along the stringed side line. Temporary strutts and shores
are placed to hold the form in position.
After the placement of rebar, the other side of the form shall be
e) erected. Straightness and verticality check executed and strutts
secured in placed together with horizontal braces to maintain the
required width throughout.

f) Where holes are needed in formwork to accommodate projecting


reinforcement starters, services of fixing devices, care shall be
taken to ensure that all holes or openings are plugged or blocked to
prevent the loss of grout when concreting or damage to the

g) Before concreting, the Site Supervisor shall ensure that all forms
are thoroughly cleaned out, free from sawdust shavings, dust, mud
or other debris and deleterious matter by using appropriate tools
and equipments as follows:
i) Mud deposits on or in formwork shall be removed by brushing or

water jet as required.


ii) Sawdust shavings, dust or loose particles and matters can be moved
to a corner using a compressor. These can be removed by sweeping
into a bin if a hatch opening is provided for.
h) The formwork face in contact with the concrete shall be coated with

a suitable mould oil or approved release agent. Such oils or release


agents shall not come into contact with the reinforcement.

i) All forms shall be formally inspected by the Engineer after


preparation and prior to placing of concrete as per Pre-Concreting
Checklist. No placing of concrete shall be commenced until the
formwork has been approved.
4.6 STRIKING OF FORMWORK
a) The Sub-contactors shall liase with the Engineer to obtain approval
before striking any formwork and to conduct inspection after
striking of forms as per Post Concreting Checklist.
b) The removal shall be done in such a manner as not to damage the
concrete and shall take place at times to suit the requirements for
its curing.
c) Where it is intended that forms are to be reused, it shall be cleaned
and made good to the approval of the S.O's representative.
4.7 PREPARATION FOR CONCRETING
Upon completion of the formwork, reinforcement and all associated
a) builder's works in connection with the Mechanical, Electrical and
other related services the Supervisor shall undertake a preliminary
inspection to ensure that such items are complete and in
accordance with all controlling relevant construction drawings and
pre concreting checklists.
b) Following this preliminary inspection the Site Supervisor shall
advise the Project Manager who shall ensure that the following
arrangements have been or being made prior to placement.
c) Place an order for the supply of concrete on a scheduled date.
d) Standby all workmen necessary to carry out the works till
completion. For long hours (more than 16 hours), shift may be
required and night lighting a must.
e) Arrange the rotation of supervisory staff.
4.8 PLACING OF CONCRETE
a) For lean concreting, a minimum of 50mm thickness blinding layer
shall be laid on well prepared firm ground after or before formwork
is placed in position.
b) Concrete shall be placed into the forms or excavations using the
appropriate plant and equipment for the purpose including direct
chute discharge from concrete mixer trucks, concrete pumps or
crane assisted concrete bucket.

e) Placement of concrete shall be in horizontal layers and to a


compact depth not exceeding 450mm. The rate of discharge shall
be such that the concrete placement is uniform across the section
and increasing at a rate to prevent segregation of the bottom
layers.
f) Compaction shall be carried out by both hand tamping and
mechanical vibrator and shall be thoroughly worked into corners.
g) To avoid a vertical drop of concrete greater than 1.5 metres,
trunking and chutes shall be used.
h) Fresh concrete shall not be placed against in-situ concrete during
the pouring sequence within certain time limits based upon the
design mix of the concrete. No pouring of water into semi-hardness
concrete in the truck is allowed.
i) Concreting operation shall cease immediately if leaks occurs in the

formwork and shall be rectified on the spot.


j) All concrete, after it has set shall be kept continuously damp by
placing wet jute sack on top of the surface.
k) The use of any curing compound shall be subjected to the approval
of the Engineer.
4.9 CONSTRUCTION JOINTS
a) Construction joints shall be located in the positions as shown on
drawings or as required.
b) The joints shall be at right angles to the general direction of the
member and shall take due account of shear and other stresses.
c) Concrete shall not be allowed to run to a feather edge and vertical
joints shall be formed against a stop board.
4.10 TESTING OF CONCRETE WORKS
a) The indicative sampling rate shall be as follows:
- One sample per 10.0 m3 or every group of 10 batches for critical
structures such as columns, pile caps and abutment wall.
- One sample per 20.0 m3 or every group of 20 batches for slabs.

However, for designed mix, at least four samples (12 cubes) shall
be taken of concrete on the first day of concreting for each grade of
concrete to be used in the Work, irrespective of the volume of

sampling shall be at least one sample from each source of
production on each day that concrete of any particular grade is
used.

d) The actual rate of sampling may be increased in appropriate


circumstances e.g. for critical elements, when required by the S.O.
For each sampling, three cubes shall be made from a single sample

ready mixed concrete, at the point of discharge from the delivery

j) On no account shall reinforcement be heated to facilitate bending.


Reinforcement shall not be re-bent unless approved by the
Engineer.
k) Reinforcement in structures shall not be welded without the
approval of the Engineer.
l) The supervisor shall also ensure that all required builder's work in
connection with M&E requirements including openings, conduits,
block outs, puddle flanges etc. are installed before placement of
the top bars.
m) Carry out inspection on reinforcement placed as per Pre-Concreting
Checklist. Bar bending schedule shall be submitted before
concreting.
4.5 ON SITE FABRICATION & FIXING OF FORMWORK
a) The surveyor will first set up a reference line offset from the
nearest TBM or from a previously constructed structure with
established survey data.
b) The subcontractors shall determine the side line of the structure of

offsetting from the reference points, established by the surveyor


using string lines and measuring tapes.
c) Before installation of formwork, the surveyor shall survey the level

of the lean concrete and the Engineer shall ensure that the base is
clean and level.
d) For horizontal structures, one side of the form shall be erected
initially along the stringed side line. Temporary strutts and shores
are placed to hold the form in position.
After the placement of rebar, the other side of the form shall be
e) erected. Straightness and verticality check executed and strutts
secured in placed together with horizontal braces to maintain the
required width throughout.

f) Where holes are needed in formwork to accommodate projecting


reinforcement starters, services of fixing devices, care shall be
taken to ensure that all holes or openings are plugged or blocked to
prevent the loss of grout when concreting or damage to the
concrete surface when removing forms.
g) Before concreting, the Site Supervisor shall ensure that all forms
are thoroughly cleaned out, free from sawdust shavings, dust, mud
or other debris and deleterious matter by using appropriate tools
and equipments as follows:
i) Mud deposits on or in formwork shall be removed by brushing or

water jet as required.


ii) Sawdust shavings, dust or loose particles and matters can be moved
to a corner using a compressor. These can be removed by sweeping
into a bin if a hatch opening is provided for.
h) The formwork face in contact with the concrete shall be coated with

a suitable mould oil or approved release agent. Such oils or release


agents shall not come into contact with the reinforcement.

i) All forms shall be formally inspected by the Engineer after


preparation and prior to placing of concrete as per Pre-Concreting
Checklist. No placing of concrete shall be commenced until the
formwork has been approved.
4.6 STRIKING OF FORMWORK
a) The Sub-contactors shall liase with the Engineer to obtain approval
before striking any formwork and to conduct inspection after
striking of forms as per Post Concreting Checklist.
b) The removal shall be done in such a manner as not to damage the
concrete and shall take place at times to suit the requirements for
its curing.
c) Where it is intended that forms are to be reused, it shall be cleaned
and made good to the approval of the S.O's representative.
4.7 PREPARATION FOR CONCRETING
Upon completion of the formwork, reinforcement and all associated
a) builder's works in connection with the Mechanical, Electrical and
other related services the Supervisor shall undertake a preliminary
inspection to ensure that such items are complete and in
accordance with all controlling relevant construction drawings and
pre concreting checklists.
b) Following this preliminary inspection the Site Supervisor shall
advise the Project Manager who shall ensure that the following
arrangements have been or being made prior to placement.
c) Place an order for the supply of concrete on a scheduled date.
d) Standby all workmen necessary to carry out the works till
completion. For long hours (more than 16 hours), shift may be
required and night lighting a must.
e) Arrange the rotation of supervisory staff.
4.8 PLACING OF CONCRETE
a) For lean concreting, a minimum of 50mm thickness blinding layer
shall be laid on well prepared firm ground after or before formwork
is placed in position.
b) Concrete shall be placed into the forms or excavations using the
appropriate plant and equipment for the purpose including direct
chute discharge from concrete mixer trucks, concrete pumps or
crane assisted concrete bucket.

e) Placement of concrete shall be in horizontal layers and to a


compact depth not exceeding 450mm. The rate of discharge shall
be such that the concrete placement is uniform across the section
and increasing at a rate to prevent segregation of the bottom
layers.
f) Compaction shall be carried out by both hand tamping and
mechanical vibrator and shall be thoroughly worked into corners.
g) To avoid a vertical drop of concrete greater than 1.5 metres,
trunking and chutes shall be used.
h) Fresh concrete shall not be placed against in-situ concrete during
the pouring sequence within certain time limits based upon the
design mix of the concrete. No pouring of water into semi-hardness
concrete in the truck is allowed.
i) Concreting operation shall cease immediately if leaks occurs in the

formwork and shall be rectified on the spot.


j) All concrete, after it has set shall be kept continuously damp by
placing wet jute sack on top of the surface.
k) The use of any curing compound shall be subjected to the approval
of the Engineer.
4.9 CONSTRUCTION JOINTS
a) Construction joints shall be located in the positions as shown on
drawings or as required.
b) The joints shall be at right angles to the general direction of the
member and shall take due account of shear and other stresses.
c) Concrete shall not be allowed to run to a feather edge and vertical
joints shall be formed against a stop board.
4.10 TESTING OF CONCRETE WORKS
a) The indicative sampling rate shall be as follows:
- One sample per 10.0 m3 or every group of 10 batches for critical
structures such as columns, pile caps and abutment wall.
- One sample per 20.0 m3 or every group of 20 batches for slabs.

c)

However, for designed mix, at least four samples (12 cubes) shall
be taken of concrete on the first day of concreting for each grade of
concrete to be used in the Work, irrespective of the volume of
concrete produced or the sampling rate. Subsequent rate of
sampling shall be at least one sample from each source of
production on each day that concrete of any particular grade is
used.

d) The actual rate of sampling may be increased in appropriate


circumstances e.g. for critical elements, when required by the S.O.
For each sampling, three cubes shall be made from a single sample

ready mixed concrete, at the point of discharge from the delivery

4.1 SETTING OUT
4.1.1 The land surveyor will identify the Right of Way boundary and location
of the piling point.
4.1.2 Offset for the piling point, finished road level, cut off level will be
provided at the peg allocated for the piling point.

4.2.1 The site clearing will be performed at the allocated location.
