Admission Requirements
1. Admission of students to Global Academy is subject to existing policies and
requirements of the institution. All new students whether high school graduates or
transferees are processed through the Student Admissions Office or Marketing
Department.
2. The following requirements must be submitted at the Registrars Office before a
student is admitted to any Academic Program of GA.

a. Incoming Freshmen
1. Form 138 (Original copy of 4th year high school report card) or Form137
2. Copy of NSO Birth certificate
3. Two(2) 2x2 colored picture
4. Medical Certificate
5. Marriage Contract (Married Female Student Only)
6. Signed Payment Plan and Student Agreement Form (Signed by both student and
Parent/guardian). For self supporting students their signature will suffice.
b. Transferees

c. Returnees

1. No record of academic deficiency status or dismissal, exclusion or expulsion in

his/her last term at GA includes unofficial Leave of Absence.
2. For those with previous academic delinquency status, interview with the
Administrator is required.

d. Foreign Students with Credentials from a Foreign School

1. Any of the following visa:
1.1 Non-Immigrant Visa
a.
Student Visa
b.
Diplomatic Visa
c.
Investors Visa
d. Special Investors Residence Visa
e. Pre-arranged Employment or Missionary Visa
f. Medical Certificate
1.2 Immigrant Visa
1.3 Documents Required by the Philippine Government for Submission to the
Philippine Department of Foreign Affairs (DFA):
a. Original or certified true copy of scholastic record
b. Accomplished Personal History Statement (PHS) with 2x2) photos and
right thumb print duly signed by applicant.
c. Affidavit of adequate support to cover expenses intended to studies.

d. Authenticated Police Clearance Certificate
