RESOLUTION REQUESTING THE PHILIPPINE CHARITY SWEEPSTAKES OFFICE ONE (1)
UNIT OF AMBULANCE ON A COST-SHARING SCHEME TO BE USED BY THE CITY
DISASTER REDUCTION AND MANAGEMENT OFFICE, THIS CITY IN RESPONDING TO
EMERGENCIES CAUSED BY NATURAL AND MAN-MADE CALAMITIES AND THE CITY
HEALTH OFFICE, ALSO OF THIS CITY AS SERVICE VEHICLE TO ENSURE AVAILABILITY
OF TRANSPORTATION DURING EMERGENCIES AND OTHER MEDICAL SERVICES

WHEREAS, the City of Ormoc , has an estimated population of _______________;

WHEREAS, City Health Office of Ormoc has three (3) ambulances , two (2) are
undergoing repairs , the other one (1) which was used for nine years is out of
operation already ;

WHEREAS, to this day, rescue has only two ambulances both undergoing repairs ,their
service vehicle now being used as ambulance ;

WHEREAS, the town has one serviceable ambulance operating within the area and
catering to the sick people of one hundred ten (110) Barangays;

WHEREAS, there is a need to request for another ambulance to service the rising
population of the City and to be able to address the needs of the sick with haste and
with more efficiency;
WHEREAS, after a thorough deliberation, the members of this Honorable Body is
of the consensus to approve the said matter;
NOW, THEREFORE, upon motion of SP _______________, duly seconded by SP
____________, the Sangguniang Panlungsod
RESOLVED, as it is hereby resolved, to request the Philippine Charity Sweepstakes
Office One (1) Unit of Ambulance on a Cost-Sharing Scheme to be used by the City
Disaster Reduction and Management Office, this City in responding to emergencies
caused by natural and man-made calamities and the City Health Office, also of this City
as service vehicle to ensure availability of transportation during emergencies and other
medical services.
RESOLVED FURTHER, that copies of this Resolution be furnished all parties
concerned;
