JOINT AFFIDAVIT OF TWO (2) DISINTERESTED

PERSON
We, MA. LINDA SUVERDEZ, of legal age, Filipino, and with
residence address at No. 3 Casanova Culiat Tandang Sora Avenue,
Quezon City, and LUISA GARING, of legal age, Filipino, and with
residence address at same as above, after having been duly
sworn to in accordance with law, do hereby deposes and states:
That we personally know ELMER P. DELOS SANTOS
and LUISA MARCHAN TANGCUANGCO DELOS SANTOS,
being their close friends and long-time neighbours, married on
March 1, 2001 at Basketball Court, Tandang Sora St., Old Balara,
Quezon City;
1.

That ELMER P. DELOS SANTOS demised on April 18,

2016 at Quirino Medical Center, Project 4, Quezon City and was a
bona-fide member of Pag-ibig and Social Security System with
Membership ID No. 3399862037;
2.

That LUISA MARCHAN TANGCUANGCO DELOS

SANTOS incorrectly and innocently used the first name of
MARICEL in all of her School Records;
3.

That the mother of LUISA MARCHAN TANGCUANGCO

DELOS SANTOS made her use the name of MARICEL;
4.

That
upon
securing
of
LUISA
MARCHAN
TANGCUANGCO DELOS SANTOS of her Certificate of Live
Birth on 2011 it appears thereon that her TRUE AND CORRECT
first name is LUISA;
5.

That however, their date of marriage was erroneously

entered in their sons, John Carl Raul T. Delos Santos Certificate of
Live Birth as June 23, 1997 instead of March 1, 2001 which is
the TRUE AND CORRECT ONE. Likewise, the first name of
LUISA MARCHAN TANGCUANGCO DELOS SANTOS was also
incorrectly entered therein as MARICEL instead of LUISA which
is the TRUE AND CORRECT ONE. Attached hereto is a copy of
their sons Certificate of Live Birth;

That we are executing this affidavit to attest to the fact

and confirm that the TRUE AND CORRECT wedding date of
ELMER
P.
DELOS
SANTOS
and
LUISA
MARCHAN
TANGCUANGCO DELOS SANTOS was on March 1, 2001 and

that MARICEL MARCHAN TANGCUANGCO and LUISA

MARCHAN TANGCUANGCO DELOS SANTOS refers to ONE
AND THE SAME PERSON;
That we have acquired information about the foregoing
facts because we are the neighbours and friends of the family of
ELMER
P.
DELOS
SANTOS
and
LUISA
MARCHAN
TANGCUANGCO DELOS SANTOS;

That we are further executing this affidavit as a

compliance with the requirements needed by LUISA MARCHAN
TANGCUANGCO DELOS SANTOS to claim Burial/Death
Benefits as the beneficiary of ELMER P. DELOS SANTOS in Pagibig and Social Security System;
9.

That we are executing this affidavit to attest to the

truthfulness and veracity of the foregoing facts for whatever legal
intent and legitimate purpose this may serve, especially for all
authorities concerned.
