WELCOME TO MS.

COFFMANS 5TH
GRADE CLASS
Disclosure Statement 2016-2017
I feel very privileged to be your childs teacher this school year. This is
my first year teaching and I am thrilled to be working at Bluffdale Elementary
with the fifth grade team. I student taught for four weeks under April
Thompson during the fall of 2015 and for the rest of the semester with Jenni
Brown in second. Since then I have completed two long-term substitute jobs
in fifth and sixth grade in Nebo School District. I enjoy being outside,
traveling and reading any book I can find. I look forward to this year and cant
wait until school starts!
Classroom Rotations
This year the fifth grade will be doing rotations for science and for
math. During science, Ms. Fisher will teach magnetism and electricity, Ms.
Thompson will teach matter, and I will teach heredity and earth formations.
Science rotations will happen Monday through Thursday starting in
September. Each science topic will be taught for twelve days and will include
a test. Also, each day for thirty minutes classes break up and go into groups
based on subjects mastered and topics needing additional help. These math
groups will change every other week.
Homework / Late Work Policy
Homework will be sent home if the student is not able to complete the
work in class. This will hopefully make it so that students are not
overwhelmed with mountains of homework. Math, writing and grammar will
be sent home if not completed in class and will be due that Friday. Each
Friday a spelling and vocabulary test will be given and about every other
week a math quiz will be given.
In addition to these assignments students should read for at least
twenty minutes a day Monday through Thursday for a total of 80 minutes. You
may break up the reading differently and read over the weekend. Reading
will be tracked on a calendar sent home at the beginning of the month and
due the last school day of the month. Each month there will also be a book
report due the last school day of the month. The first one will be due in
September.
Late work will be docked four points, after it is a week late it will go
down to half credit. It will stay at half credit until the last week of the
trimester. Even if an assignment is late it still needs to be turned in so
students can attend Friday activities.
*On math work, students MUST show work. Each problem will be worth two
points, one for the correct answer and one for work shown. Work constitutes

as steps the student did or what they put in a calculator if a calculator was
allowed.*
Missing Assignments / Planners
This year we will be using planners daily to write in work and upcoming
events. These planners must be signed every night, Monday through
Thursday. If a parent or guardian is unavailable to sign it, another adult may
sign for credit. Planners will be checked daily and given points for each
signature.
If your child misses a day, there is a missing assignment bin at the
back of the classroom for them to pick up their assignments. If they have any
questions on the work, they are first to go to the student with the absent
student job. Then, if they have further questions they can come to me. These
assignments must be turned in within two days of the absence or it will be
counted as late work. Late work will be docked four points. At a week late it
will go down to half credit. It will stay at half credit until the last week of the
trimester. However, students will need to turn in all work and homework in for
CRASH and store activities.
Each week there will either be CRASH (an activity) or store (based on
Dojo points). To participate in these activities students must have all their
missing assignments completed. If the assignments are not done, they will go
to study hall and work on the assignments. Our goal is to have every student
to participate, so please make sure all of your childs homework is completed.

Classroom Rules and Expectations


This year there will be five rules for the classroom. These rules will
be posted in class along with what will happen if the rules are followed
or broken. Our classroom rules are

I will follow the E.A.S.Y way (eyes forward, arms folded, sit
straight, yap closed!).
5. I am respectful in my attitude and actions.
If these rules are followed the class will have the opportunity to receive
reward such as 15 minutes of music, five minutes extra of recess, miss
a homework assignment, free time or another class decided award.
However, if a rule is broken, the student will have a consequence that
goes in the following order (consequences are individual). When a
student gets think time, they will also lose the class reward. Think time
is a new management system that happens when a student breaks a
school rule or breaks a class rule four times. The student will go to a
pre-determined classroom to write down how they can improve their
behavior.

Class Website
The class website will be at bluffdale5th.weebly.com. This will include
the management system, math, spelling and vocabulary, social studies
and science, and school and class events.

Classroom Wish List


The following are some items that we could
use in our classroom.

Tissues
Highlighters
Crayons, Markers, or Colored Pencils
Zip-lock Bags
Cheap Items for Class Store (pens,
notepads, erasers).

Expo Markers
Hand Sanitizer
Clorox Wipes
Pencils
Treats (goldfish, pretzels, tootsie rolls,
etc.)

You can buy all of these items at Wal-Mart


or the Dollar Store. Every student will
receive all the items they need even if
you are unable to donate.
