Week 4 assignment

Write an application that employs at least one of the following: ListBox, GroupBox, ComboBox.
You will create a payment calculator for charges (including sales tax) related to a physician's
visit using the following criteria. Service type: office visit, consultation; Lab services: blood tests,
sample testing, other; treatment options: shots, medication, none; patient name, patient ID.
Attach values to each service as constants.
Include at least one function. Include submit, reset and exit buttons.
Challenge: Add keyboard shortcut hints to buttons.
Submit screenshots of your application in run and design modes as well as 3 examples of
output.

Try not to mirror example layout precisely and use your own variations on properties/names.
