Significant issue:
High potential incident
SCL 1111
On 22 November 2013, at Winslow Street work area, a backhoe was deployed to carry out backfilling
work of a trench. The backhoe was sitting on existing concrete standing but since part of the backfilling
area is far from the backfill stock pile location the backhoe needed to move forwards to carry out the
work. A soil incline was formed to allow the machine to traverse closer to where the backfill was
required, when the machine moved down the incline the soil underneath the machine sank and caused
the machine to sit vertical (on its counter weight) in the excavation. A 45T crane lorry was mobilized
from other works area to lift the machine back onto the ground level. A director review has been
completed and a full investigation is currently being completed.
Reportable Accident
HATS 2A
On 20 November 2013, a locomotive pushed 2 shuttle cars with man rider ( the train) toward the tunnel
face at Stonecutters Island Site, 2 workers were installing lifting eye from a aluminum scaffold for
transformer relocation at single track area and a banks man acted as a lookout, the three man team set
red barriers at around 35m both directions on the track to act as early warning. As the train approached
the 3 workers the banksman identified that the train was coming and raised the alarm and signaled the
locomotive driver using hand signals. The two workers climbed out from the scaffold and jumped down
or fell down onto the floor.
The operator of the locomotive applied his brake but had already run over the red barrier and eventually
the train came to a stop after it struck the aluminum scaffold as a result of the incident one of the
workers suffered a dislocated left elbow and the second IP suffered only minor abrasions. The IP was
sent to Princess Margaret Hospital, for treatment the IP was requested to stay at hospital for
observation and released on the 23/11/2013. A director review has been completed and a full
investigation is being completed .
Minor accident
Midfield Concourse Works
On 19 November 2013, when a worker was working at area 20, his coworker cut the steel tube by
portable electrical grinder and accidentally some particle flew into his right eye. He was sent to panel
doctor for treatment after first aid treatment. Designated hot work area is required for all cutting
operation and remind worker to wear goggles during steel cutting working area.
On 22 November 2013, when a worker was dismantling a working platform, some particle went to his
right eye. He was sent to panel doctor for further treatment after first aid treatment on site.
No. 8 Mount Nicholson Road
On 19 November 2013, when a worker was carrying his work at House 16, 1/F staircase, his nose was
scratched by the staircase wall formwork when he was turning around his body. He received first aid
treatment on site and sent to panel doctor for further wound treatment. To maintain the housekeeping
and lighting for work area.
St. Paul Hospital
On 15 November 2013, a worker and his coworker were assigned to carry out some remedial work at
LG3, in the course of work a piece of scaffold tube leaning against the wall toppled and hit onto the
workers back. No abnormity was observed after a short break and he was continued his work till
closing of the day. On the next day, he felt a bit painful on his back and was escorted to consult panel
doctor and light duty was recommended until 20 November 2013. To improve the housekeeping on site
in particular stacking method for loose material.
