OBJECTIVE
To obtain an OTA/C position or similar position in a facility or out in the community where I can use my skill and knowledge and have room
to continue to learn and grow in the profession. I am looking for a position where I can be one on one or in a group with patients of all ages
and diagnosis/disabilities physical and mental to challenge myself.

Responds promptly to acute changes in resident physiological state and reports modifications or adjustments
to the treatment plan within limits of the Plan of Care.
Communicates resident progress and response to treatment to Wellness Director, therapy, and Restorative
Nurse Manager.
Resident/Family education: Plans for and provides resident, family, and caregiver education, identifies learning
needs, clearly communicates information, and documents response to education provided.
Participates in plans for resident admission/discharge, anticipates long term and short term resident needs,
and collaborates with the interdisciplinary team and make recommendations and attends Care Conferences as
needed.
Based on observations, makes suggestions to supervision nurse for new exercise/restorative programs which
might benefit the resident
