RCC Paving close to TK174 is damaged - exposing the
steel reinforcements. This presents a tripping hazard and
will lead to accelerated further deterioration.

For a long time - scrap and rubbish has been allowed to

accumulate in the fabrication yard. Despite various
notifications - nothing has been done. This present a
safety risk at multiple levels: fire hazard, tripping hazard
and emergence access/exit hazard. Scrap should be sold rubbish should be cleared. This should be done at
regular intervals so that build up does not occur.

simply not been cleaned up and was thus presenting a
fire and environmental hazard. I asked it to be removed

On 31st July 2014, while making rounds with Jani

Keshaba, the following unsafe acts have been observed
and the drivers and cleaners have been stopped to
proceed further and advised them to correct the
situation before allowed them to continue. 1. GJ-1-UU
4678 Tanker top sampling job was going on without
wearing the goggles. Informed the guy to wear the
goggles and corrected immediately. 2. GJ-6-VV-4339:
Driver Alaudeen was taking sample from the product
(NBA) without wearing gloves. He was carrying 2 bottles
of the sample on one hand and was coming down with
only one hand holding the hand rails and sometimes
even without holding the handrails. Called and explained
the dangers. He complained that he do not have cleaner
to help him. 3. MH-04-GC 5497: Cleaner Kailash was
taking sample from the product (PG) without wearing
gloves by dipping his hand inside the product. Called and
explained the dangers.

At 12.25 pm (Dt 06.08.2014) while placing the tanker

below gantry for loading cleaner who was guiding the
tanker slipped & fell down near tanker wheel & got
pinched between floor & tanker wheel injuring his right
hand & leg (laceration). Casualty was sent to tie up
hospital for medical attention where he was relieved at
17.00 hrs (Dt 06.08.2014). Detailed investigation is
under way.

At Gantry # 8 (Near Tk-148/149), the following 2 tankers

were found loading and the driver/cleaner who was on
top and loading the product was wearing the normal
safety glass. Both were having the safety goggles and
kept on the top of the head. After reminded, they were
corrected. 1) GJ-12-AY 8253 loading BAM 2) MH-06-AC
1879 product not known. The valve operator did not
intervene. Valve operator has to be given with clear
instruction that loading should not be started if they do
not wear the safety goggles.

Grass growth in terminal creating unsafe condition
75 Kg extinguishers are placed in many parts of the
terminal in open spaces. As a result, the colour faded
away and the instructions are not visible and not
readable. The hose looks very brittle and may not
withstand the pressure while operating the extinguisher.
This may create an unsafe situation and may hurt the
operator who is using it. Urgent attention is required to
check these hoses and to provide a shade or cover to
protect from further damages. Photos are attached.

UC : Cross-over ramp opposite to tank 145 to be

modified for easy access & cross over the dyke wall still
one who is using cross-over ramp, has to walk on dyke
wall.
UC : Manifold for Haxane tanker unloading found
without proper supports & not fixed.
UC: Silver line 6" opposite to tank 146, temporary
support given with 3" pipe line and resting on pipe rack.
UC: Scissor lift not placed properly at cemented floor
and the time of emergency, same can not be shifted
easily & timely to the site.

Scrubbers lying at various junctions are found without

proper grounding arrangement. Its is being used for
depressurization of dock lines, there are different phases
involved in the process namely air/nitrogen, water &
chemicals. Chances of static generation is very high so it
requires proper grounding.
OJ-5 air line valve is very difficult to operate as the
position is difficult to reach. It is an unsafe condition
specially during night hours. Suggest to extend the air
line to an accessible point.
The lighting is improper at 199 junction. During night
hours the visibility is very poor and is unsafe condition
for the operation team
Plastic Hoses are being used to unload diesel in terminal.
Not too sure if can be used as the hoses are nonconductive and should not be used for product transfer
for product such as diesel.
Near T-203, process line of T-201 leak. Approx. 5 ltr
product spilled
During routine round it was observed that Three labors
of M/s. Patel Construction Hand gloves were worn out.
Immediately stopped the work & instructed supervisor
to change the gloves. Also warned supervisor if repeated
will impose penalty.
Water clogging was observed near T 208 - informed
labor supervisor to clean it
During routine round it was observed that T 218 ladder
step is in bad condition. Instructed operation staff not to
use this ladder unless Maint. repair it
Cargo CDSBO spilled approxmately 05 liters by the
tanker No. GJ18X 8191 during unloading of the cargo.
Transporter is : Nakoda Farwarders, Driver Name: Manoj
B Singh.Time : 08:30 hrs.
During routine round it was observed that T 303 ladder
step is in bad condition - need to replace it

Platform at corner is resting on product lines. It is unsafe

as the platform is brushing itself with cargo lines.
Tanker no-MH43Y-2765 was loaded yesterday from our
terminal & today tanker to be load more 3 Mts. while
loading in 3rd compartment, driver has not informed to
loading master or loading operator to they have to load
only 3mts and driver has not proper gauging
compartment & overflow approx.- 3 to 4 litre. Tk-001

During routine testing and maintenance found scissor lift
not working
The discharge line of tank no 006 found leaking near the
dyke crossing. The Line was emptied out in the tank and
a blind fitted near tank side as tank body valve is
assumed to be passing.
