right gantry but due to over lapping of loading hoses on
the gantry contract labour inserted loading hose of tank

master noticed the abnormality but till the time he
stopped the loading, 300 kgs of cargo was loaded.Tank

Main junction worker got minor cut on ankle in main

junction, he slipped from platform, He is not performing

During line walk of MEK receipt in tank no 117 by
fireman Jitendra Singh, pin hole leakage was observed in
delivery line crossing dyke wall of the tank.After
completion of vessel (MT EVERRICH 3 @ OJ 3)line was
emptied out.Another case of leakage at pipeline dyke
crossing. Earlier reported same case for tank 108 & tank
no 6.
During the morning round leakage was observed in
delivery line crossing dyke wall of the tank-122. same
was emptied out by nitrogen blowing and blinded out at
tank side.

morning.

wearing Safety Goggles and without provided wheel
chock. Immediately stop them and instruct him to
provide wheel chock & wear safety goggles

the same. Intervene the act and made understood him

Tanker No.: RJ 01GA 4974 (HG Transport), is parked near
Surveyor room and waiting for loading. Observed tanker
driver is sitting near the tanker instead of driver sitting
shed. Intervene and instruct him to use driver shed till is
turn not coming..
Tanker no.: GJ 12AU 7100 (Vimla Transport), place for
loading under gantry. Observed key was not removed
and wheel chock not provided. Intervene the act and
made necessary correction.
Tanker No GJ 12 AY 8802, OM Transport : Driver has not
wear the glasses in proper way, intervene the act and
corrected the same by guide the driver to wear the
glasses in proper way.
M/s Patel construction labor is shifting the sand in hand
cart from outside to near T-410 . Observed that hand
cart is full loaded and also labor has not wear the hand
gloves even though he has having. Intervene the act and
giving instruction him as well as his supervisor not to
load the hand cart full and to wear the gloves.

At Adm building First floor Project manager office Switch
board minor spark observed and MCB Tripped during
Chamuda trading electrician doing wiring work for
changing seating arrangements.
Road tanker no-MH04CG 7769 during sampling & sealing
leakage was observed through flange joint of 3rd
compartment & it's tanker body. same was temporarily
arrested & gated out. product-NBA.
