5. The physician orders Wellcovorin 1mg IM. The drug comes in powdered form
in a vial containing 50mg. The directions for reconstitution are: Add 5mL of
diluent for a final concentration of 10mg/1mL. After reconstituting, how many
mL should you draw up? 0.1 mL Once you reconstitute the medication, you
go only by concentration!
6. Pediatric conversions
a. Your patient is 2 years old and weighs 35 pounds. How much is that in kg?
15.9 kg (remember when going from pounds to kg you divide by 2.2 and if
converting from kg to pounds, you multiply by 2.2.
b. Your patients mother asks how much is 15 kg in pounds. How much is it? 33
pounds
c. Your patient has a diaper weighing 10 g (new or empty) and has a soiled
(Urine) diaper weighing 50 g. How much would you enter for output for urine
that hour? 40 mL would be recorded. Subtract weight of empty diaper
(weight will range by size of diaper) from total and that is your output

a. IV 0.9% NS has been ordered as a bolus 250 mL over 2 hrs. What
is your hourly rate? 125 mL/hr (make sure you ALWAYS put the
measure (such as mL, mg, mL/min, mL/hr)
b. If 500 mL of D5 1/2 NS was ordered to infuse over 8 hrs and it was
started at 0915, what time will the infusion be completed? Give

c. The physician orders D5LR 1000mL/10hr. The IV set drop factor is
20gtt/mL. How many gtt/min should the patient receive?
77gtts/min sorry, I never do gtts (always on a pump), but there are NCLEX
questions and ED & OR often do without pump.
Maternity Question
4. Your patient is to receive an infusion of Pitocin. The drug comes 20U/1000mL
in D5W. The doctor orders 0.01U/min. How many mL/hr should the patient
receive? 30 mL/hr
