AFFIDAVIT OF CONSENT

We, Albert Cua Salonga and Virginia Lin, parents of minors Alyssa Kaye L. Salonga
and
Andrei
Kenzo
L.
Salonga,
and
residents
of
__________________________________________, after having been sworn in
accordance with law, hereby depose and state that:
1.

We are giving full consent for our children to travel from the Philippines to
France, Switzerland, and Germany with traveling companion Daisy Cua
Salonga;

2.

Daisy Cua Salonga is the sister of Albert Cua Salonga and the aunt of minors
Alyssa Kaye L. Salonga and Andrei Kenzo L. Salonga;

3.

Daisy Cua Salonga shall act as the guardian of the aforementioned minors for
the duration of their visit to France, Switzerland, and Germany;

4.

We are giving consent to the guardian to process and procure tickets, visa,
and other requirements in relation to the said trip;

5.

The guardian undertakes to shoulder all the expenses that are needed for the
upkeep and personal care of the aforementioned minors;

6.

The guardian is willing and able to maintain and support our children
financially and guarantees that they shall not become public charge or burden
during their stay in France, Switzerland, and Germany.
