revision dates included. Both IMS & QMS procedures found indeed
For Omega Laborataries, third party certification done by NABL has been documented and validity of
certificate was found upto 30/09/2015
Latest ISO procedures for Monthly Review meeting - PR 9940 05-R0 received by site from ISO cell
through hard copy, being documented and distributed to all concerend departements evidence found
Third party documents were all reveiewed by QA/QC department by marking a rubber seal over the
doucment and alos mentioning status of the doucment

Customer Feedback form received for every three months from the General conusultant duly signed b
the Resident Engineer. Till Sep-2014 feedback form available
Analysis of customer feedback are done in every MRM meeting

Standard deviation for the period 26.09.14 to 25.10.14 found to be 4.2 for the M40 grade of concrete
If results are deviating, analysis of properties of Cement, sand and aggregates has to be done. But til
now no such cast found
Cube Report register for Washermen pet Station - Base slab, sample no. 8334, Date of casting
11.09.2014, for pour 8/grid 16-18, 28 day strength found to be 52-59 Mpa for M-40 grade of concrete
found satisfactory

NC raised by counsultant from log book UAA-01/NCR/RE/Q4/396 is raised for water stoppage. But NC i
kept open and suitable action is taken when excavation is carried out

QAP plan - ITP for PVC water stopper (water proof membrane) from Omega Laboratory supplier for M/S
Duron Polyvinyls Ltd. Date of testing : 20/09/2014, certificate no. OIAL/2014/09-7464 with reference
taken from IS 15058/2002 found satisfactory
Monitoring for 32 dia Fe 500 bard , Quantity 23.85T, source is from SAIL, MTC and Test report
ML/17265/7/14-15, dated 21.10.2014, cross reference found Ok
Vendor for steel supplier for "SAIL" has been apporved by the client through letter no. GCC-282-CIV1107436 has been evident
Quality manual plan UAA-QP-TTA-220-COU-1100001-B found appoved via letter no. TTA-GC-COU1101010 dated 29/04/2011, evidence found ok

It has been observed that NCR has been raised on same issues say water stopper damage, joints on D
wall etc., should be monitored and suitable corrective and preventive action has to be taken

Quality Policy and Qualtiy manual were displayed

test certificates, load test report, Insurance, Driver's license with Medical
certificates found evident
150T Crane (hired from EASTMAN) utilization found to be 70% against 95%
mentioned in manual PR 9952 CP 03
List of total equipments was available for the month of November 2014
For Front end Loader, FIP Nozzle(S.No. 19923 & 19920) got broke down after
using 1500 hours on an average. Analysis has been carried out, root cause
found out and communicated to the supplier found satisfactory
Preventive Maintainence done for 500KV - DG S.No. 1035593 and also got
updated in SAP report
Trench cutter, PMR code 20316 test certificates with "Safe for Use" (Client
recommendation) available. Insurance, Driving License and Medical certificate
found ok
Major Equipment like Trench cutter, D-wall rig, Pilling rig etc., breakdown should
not exceed 10% as insisted by client, found 7.54% for the period October 2013 October 2014, found satisfactory
File codification may be improved. File number and Name of file to be properly
labelled
Request for purchase of Exhaust Silencer for Water Tanker has been raised
through purchase no. 17027
For Trench cutter BC40 (20315/20316), utilization available was only 11% for
November 2014 against 95% as per mentioned in manual PR 9952 CP03

Departmental Organisation chart R21 dated 20/10/2014 displayed with roles and
responsibilities
Objectives of the department were not defined
Wastage statement of Steel reinforcement for Sep-2104 found to be 4.78%
against 3%. Analysis of data carried out. In D-wall, length of 40mm bar
required is only 10m so remaining 2m going on waste. So now order has been
placed for 40 dia bar of length 10m to vendor and found ok
Wastage also available for bulk material like cement, flyash, sand, fine
aggregate 10mm & 20mm and concrete wastage has been analysed. Wastage
analysis done for once in 2 months period
All the BBS has been signed and controlled by design co-ordinator, sample
checked found ok.
Method statement for TBM retrieval & lifting of TBM components at Central
station UAA 01 - TTA-446-SCC-14000001 thorugh transmittal TTA-GC-COU1403163 dated 22/08/2014 got approved by GC
Method statement register available for retreiving superseeded copy of
drawings & method statement found updated
In order to achieve the target dates, and also for the speedy work required
drawings on priority were mentioned and sent to the permanent designer (PB)
Staff productivity ratio upto the month of September 2014 - 0.13Cr
Planned Vs Achieved carried out for every week (for all stations), base slab
construction VS03 pour 1 concrete target date 24/11/2014, but not achieved,
suitable analysis carried out and found due to D-wall leakage
Daily progress chart for station building & tunneling was available
Customer feedback form done once in 3 months. Feed back form verified for
July-September 2014.

Master List of documents available in IMS format & RA bills were included in
No.6 and found ok

Sub-contract evaluation done by station manager/concerend head once in 6
months. Sub contractor "J.K.Bhola" for labour supply and found satisfactory

If sub-contractors is doing some extra works, suitable amendments were made
with revised rates found ok
Sub contract selection made by making comparative statement and by
analysing the rates found for 150T crane. Three suppliers (M/s. Sanghari/ M/s.
Eastern / M/s.express equipment) and m/s. Sanghari were chosen based on
capability and financial, evidence found ok
Conditions for Peace Rate Work (PRW) and Labour Supply has been listed and
approved by Project mananger, if contract worth more than 3 crores, then
project co-ordinator approval is required
Till now there are no sub-contractor appointed by Head office
Diaphram wall construction sub-contract is given to Ipex Infrastructure worth Rs.
4 crore give to sub contract because due to parallel work based on base line
program. Evidence were justified
As per Procedure Manual, Sub-contract planning has to be done

Objectives of the deparment has been refined with time boundary

Weigh bridge calibration found for make CAS which is located at main store @
PWD, capacity - 300kg, minimum-1 kg with accuracy 50g and validity was upto
18/09/2015
Material which is non-circulated will be mailed to corresponding section heads
Separate stock list updated daily for TBM found ok
Material - welding rod issued S.No. 38100132, 1 nos and the same SAP entry
made through no. 49002041355
