IMPLEMENTATION OF NEW BIRD / ANIMAL HIT REPORTING PROFORMA NO.CAAF-002-SBXX-1.1

1.

Refer 5th National Aviation Safety Team (NAST) Meeting 2011 held on 02 December 2011.

To ascertain the authenticity and determination of extent of damage caused to aircraft in the event

of Bird / Animal Hit, need for new reporting procedure was discussed during above referred meeting. Later,
the new Bird / Animal Hit Reporting Procedure was amply discussed and approved for implementation in
NBCC meeting held on 23 December 2011. The new proforma has now been designed, which shall be
effective from 01 January 2012, which has following additional columns:
a) Bird / Animal Hit Reported by Operator (Name, Signature and Date / Time).
b) Bird / Animal Hit Verified by Duty Airworthiness Surveyor PCAA (Name, Signature and Date).
c) Bird / Animal Hit Verified by Duty Manager Airside PCAA (Name, Signature and Date).
d) After completion of all formalities Proforma be sent to President SIB by the Operator through
Fax & Email, or any other quickest means.

Specimen of new Bird / Animal Hit Proforma is attached, and is also available on PCAA website

(www.caapakistan.com.pk) at SIB portal Publications. Bird / Animal Hit report will now be considered for
necessary action when the report is raised using new Bird / Animal Hit Proforma No.CAAF-002-SBXX-1.1.

All Airlines / General Aviation Companies / Operators are, therefore, to disseminate this information

at all the operating locations, to ensure adherence to new procedure, and to report the Bird / Animal Hit
cases using new proforma no.CAAF-002-SBXX-1.1 with effect from 01 January 2012.
