I HP/SW/14-15996 Under Officer Monika Thakur of PGC Solan under 1 HP(G) Bn, Solan
state the following.

I am part of the TSC Team of NCC Gp Shimla. I alongwith 41 other cadets are
participating in CATC 190 at GSSS Rewalsar. The camp is orgnised by 2 HP Bn NCC Mandi, but
our training is being conducted by Col Harsh Sharma, OC, 6 HP (I) Coy Una.

On 16 Jul 2016 firing of .22 rifles was orgnised for the TSC cadets. The firing range is
about 200 meter from the school premises. Cadet GSSS Saigaloo was also part of the firing
practice . After firing at about 0930 AM , she complained of stomach pain to Col Harsh Sharma. I
was ordered by him to escort Cadet Kamna to the lines. I took her to the lines and asked her as
to how she felt. She replied that she had some pain but this sometimes happen with her in the
camps. She wanted to take some rest. I left her in the room and told her that she could join for
firing in case she felt better. I returned to firing location thereafter.

When I returned after firing at 01.15 pm I tried to look her up, but she was not present. I
enquired as to her whereabouts and I was told that she had been taken to the hospital. At about
330pm, Cdt Kamna returned to the room. She then had her lunch alongwith Cdt Anchal. She then
returned to the room. I asked her where she had gone and she replied that she was at Rewalsar
Hospital. She had been administered glucose and two injections. She also told me that she
wanted to rest and was feeling better. Thereafter she rested and at 430pm I went for my written
test.

At about 7.15 pm I returned to the room. Cdt Kamna told me that she was feeling very
warm. At about 7.45 pm she wanted to go to the washroom. I took her to the washroom where
she went for latrine. It took us about 30-40 min for the activity . At about 830 pm, I reported her
condition to the F/O Leeladhar Sharma, ANO, GSSS Rewalsar and Camp adjutant. The
ambulance was immediately called which arrived about 10-15 min. Cdt Kamna was put in to the
ambulance. Her sister Cdt Archna and one JCO also accompanied her. We had meantime
packed her bags and gave to her. She left for hospital at Mandi shortly afterwards.

I am attending in CATC 190 which is being held at GSSS Rewalsar from 11 Jul 16 to 20
Jul 16. There were a total of 11 cadets for my school in the camp out of which two cdts (Kamna
and Anchal) had come as part of TSC Team.

Cdt Kamna was a very good cadet . She was very fit and enthusiastic but used to have
pain in her stomach during her mensural cycle. On 16 Jul 2016 at about 1230 pm I met cdt
Kamna in the room and she complained of vomiting. I immediately came down and showed her
the Lady Primary health worker in the school premises.Cdt Kamna reported that she having her
mensural periods. The healthworker adviced that Cdt Kamna be take to the Hospital for
injections. I informed Sub Anil Kumar of 2 HP Bn NCC Mandi . He arranged for the Maruti Van of
CO, 2 HP Bn NCC Mandi . I and lady Primary Health Worker accompanied her to the Primary
Health Centre at Rewalsar which is about 200m from the Camp.

At the Primary Health Centre her registration was done and a slip made. Cdt Kamna went
alone to the doctor for checkup and stayed outside. Withion 3-4 mins she came out and we went
to the registration Counter for medicine. Then she was asked by the nurse if she had her
mensural periods and Cdt Kamna reply was affirmative. Cadet Kamna was administered two
injections and glucose. I was thereafter returned back by the health worker accompanying us
and she said that she will look after Cdt Kamna.

Cdt Kamna returned back at about 330pm with the Primary Health Worker. She told me
that she was feeling hungry and we took her for lunch. She then said the she wanted to rest. At
about 7pm I went to look her up. She was feeling very warm. After sometime she said that she
wanted to go to the washroom. We then brought her to the CO Office and reported the matter to
the F/O Leeladhar Sharma, ANO, GSSS Rewalsar.An ambulance arrived and she was transferred
to the hospital.

I Col Piyush Agarwal state the following.

I am posted as Commanding Officer of 2 HP Bn NCC Mandi. There are 3050 Junior
Division/Wing Cadets and 640 Senior Division/wing cadets under this Battalion. The cadets have
to undergo minimum one Annual Training Camp as part of the training curriculum.

The Combined Annual Training Camp (CATC-190) for the cadets of the battalion was
orgnised at GSSS Rewalsar from 11 Jul to 20 Jul 16. 443 Cadets were part of the camp. The
details of which are submitted.(Parade State with break down of cadets is received and attached
as Annexure-1)

Along with the ATC, the NCC Shimla Group Team for Thal Sena Camp(TSC) was also
being trained which comprised of 42 Cadets(included in the strength of 443). Col Harsh
Sharma, OC, 6 HP(I) Coy NCC Una was incharge of the training.

HP/JW/15-17401 Cdt Kamna of GSSS Saigaloo D/o shri Balbir Singh was also part of
the TSC team. On 16 Jul 2016 Cdt Kamna did Rif .22 firing under the supervision of Col Harsh
Sharma and his team. At about 0930AM she complained of uneasiness and was brought to the
lines by Under Officer Monika of 1 HP(G) Bn Solan. She rested for sometime and was shown to
the Primary Health Worker who was deputed by medical tauthorities for the camp. At about
1245PM she was taken to the Primary Health Centre, Rewalsar in the Maruti Van by the Primary
Health Worker and Cdt Yamini Sharma. She was treated and discharged by the PHC at around
3.15 pm. PHC is about 100m from the camp.

In the evening I went to hospital at Rati and Mandi to visit the cadets who were in the
hospital. I returned back at about 0840PM. The ambulance was parked in the school for Cdt
Kamna. I took charge of the situation. Her parents had been contacted. Sub Balbir Singh and Cdt
Archana were accompanying her. At about 10 PM, I was informed that the cadet had reached
Zonal Hospital, Mandi and was admitted to ICU. The parents were also present there. At about
1115 pm I was informed about the unfortunate death of cdt Kamna.

After ascertaining the details, I reported the matter to Brig Lalit Joshi, Gp Cdr Shimla Gp in
the night.

Yes it was a planned camp. It was approved by the NCC Dte PHHP & Chd. I produce the
requiste documents. The AJI is also produced.
Q. 2 Was Kamna Devi a bonafide NCC Cadet?
A. 2 Yes, Cadet Kamna Devi was a bonafide NCC Cadet. She was enrolled in 2015 and was in
her second year JW. Her enrolment documents are produced for the verifications by the court.

produce her medical certificate and the other camp documents.

Before the commencement of the camp, a letter had been written to the CMO, Zonal
hospital Mandi to provide medical support. Copy of letter is produced herewith. The CMO had
detailed staff from PHC Rewalsar. A health worker used to be present in the camp from 1000h to
1600h daily. Besides, PHC Rewalsar was 100m from the camp where a doctor was also available
during the same time. After 1600h there was no medical support at Rewalsar. Any personnel
falling sick had to be taken to hospital at Ratti or Mandi in either ambulance (108) or battalion
transport. The ambulance were generally available at Rewalsar and were quick to arrive. The
cadets were taken to Hospital at Ratti and either admitted or given medicines and sent back.

What was the treatment/diagnosis by the medical authorities?

We had approached the medical authorities at all three locations. The doctor at Rewalsar
refused to give a statement. The Doctor at Ratti have given a list which I produce to the court. The
doctors at Mandi have also not given us the list till now.

What were the arrangements for food preparation and distribution ?

The food was being prepared centrally for all personnel of the Camp i.e cadets, staff and
officer. It was being prepared at the cook house of the school where Mid Day Meals are prepared.
Seven cooks had been hired for cooking. Mr Leeladhar Sharma, ANO GSSS Rewalsar and JC594230M Sub Ashok Singh were the incharge for food preparation and distribution (Copy of
Camp Routine Order is produced to the court). Food was made fresh for every meal. The dry
ration were stocked in the store attached. The milk was purchased daily and the vegetables were
purchased once in two or three days.

What were the arrangements for water ?

A.10 The water was being taken from the school source which had been previously tested and
found to be of one of the best qualities in Rewalsar. Even local people took water from that
source.

A. 11 Yes, the FIR was filed . I hereby handed over a copy to you.

Ans
Any cadet feeling unwell would fallout during the morning PT Parade and report to the
Administrative office / SM. Their names were entered in the sick report both. In case the cadet
reported sick during 1000h to 1600h, they were shown to the health worker at the camp and
further shown at PHC Rewalsar. After 1600h till next day morning, any cadet falling sick was sent
to Ratti Hospital through camp vehicle or ambulance 108.

No, there were no police personal at the camp. The SP Police had been informed
through a letter before the commencement of the camp, but presence of Police personnel at the
camp location has never been the practice.
Q15 Who was given the contract for cooking and why?

The contract for cooking was given to M/S Daulat Ram, who are professional and have
done cooking in various NCC Camp including of 2HP Bn, earlier .There were therefore tired and
tested and there was never a complaint about their work.

No there was no Doctor on call. The Medical arrangement have already been explained in
my statement.

Where were the officers staying?

I was staying at 100m from the school location.

Q18 Why was a lady ANO /WTLO/GCI, not part of the camp?
Ans
The unit has two lady ANOS and no WTLO/ GCI on its strength. One lady ANO has
already take part in the camp from Jun 12 to Jun 21 at MLSM collage . The Second ANO was
detailed for this camp and attend from 11Jul to 13 Jul 2016. However, due to emergency at home
she had to be relieved from the camp .There were two Lady peons who were detailed from the Bn
for the camp. There were available 24*7 and stayed with the Girls. They have all done camps
previously and were competent to deal with the girls problems. Besides as is normal practice the
senior SW girls did also help in administrating the girl cdts.
Q19 What is the SOP for the conduct of the camp?
Ans
The camps are done as per DG NCC orders on the subject. The AJI of the camp was
issued which covers the adm aspects.
Q20 Why was GSSS Rewalsar chosen for the camp?
Ans
Himachal Pradesh does not have NCC Academy and hence most of the camps are
conducted at various colleges at Schools during their summer/winter vacations. Since almost all
of these are non residential schools, there are constraints of accommodation and toilets in these
places. GSSS Rewalsar was found to be the best locations amongst all the schools under the Bn.
The School had some new infrastructure and was fully secure. Another major issue is of
availability of water and at Rewalsar the water of Good quality was available in plenty. GSSS
Rewalsar has also organized many sports camp for children with a strength of 500- 700 earlier.

Q21 Why camp was not held at Shahtali (Baba Balaknath ) as camps were held earlier
there?
Ans. Sarai at Shahtali was not available during this pd.

Athletics/ sports camps for str of 450 950 have been held at GSSS Rewalsr earlier.

Q23 When lady ANO went back due to some problems did you ask for replacement? As
per others statement she had come back after 3 days what happened then?
Ans. The Bn has two lady ANOs. One had done the previous camp. The other was detailed for
this camp. She had joined at the commencement of the camp. However, due to domestic
problems she had applied for leave on 13 Jul. She then come back on 15 Jul to info that there
was a death in the family and hence she wanted further more leave.. Hence , she was relieved.
There was no replacement available for the lady ANO. Since two lady peons were available the
camp was continued.

Ans. Yes , the office peons have participated in the earlier camps and done a good job.
Moreover , the senior cadets were also responsible.

the camp?
Ans. The peons knew their charter of duties very well as they were experienced in the job.

Ans. I was staying at a home about 100m from the School location in the camp area. Col.
Manvinder was with me . Col HD Sharma was staying in a separate house about 5-6 KM away.
Q27 Can you produce bills for accn where you were staying?
Ans. No bills were charged to any of us by the owners since the hose belonged to the one of
the Army Ofiicer father. The officer is from Dogra Regt and is well known since the Bn ERE is
from Dogra regt.

What was the chatter of duties of various staff at camp?

The chatter of duties was as per the appts. The appts were given out in the Camp Orders.

Q29 What was the planning parameters for est camp especially for toilets for girls?
Ans. There are no planning parameters available . However , during planning all issues of
adm were taken into account incl accn, toilets, messing etc.
Q30 Why you did not hire a civ Nurse to give med cover?
Ans. There are neither any orders on hiring of civ nurses nor can it be recommended. How
can their competence be verified? There were health workers available during 1000h to 1600h .
Thereafter govt ambulance service and doctors were available at Ratti which is 18 km from the
camp.
Q31 Why cant the camps be held of lesser strength if adequate place is not available?
Ans. The Bn has a strength of 3690 cadets of which about 2000 have to undergo camps in a
year. A min of four camps are required for the bn of str 500-600. Majorly of the camps should be
held between 01 May to 30 Sep so as not to interfere in the studies of the children and
simultaneously select suitable cdts for TSC/RDC. Moreover, all the camps cant be held at
Shahtalai. Hence, majorly of the camps are held in Schools and College during their summer
break. Winters are not suitable time for holding camps in Himachal Pradesh due to cold

weather.Hence due to limited availability of time (May to Sep) due to the fact that cdts have to be
shortlisted for TSC/RDC, School/ Collage schedule, availability of space etc, camps cant be split
beyond a point. Camps of 200 cdts is not feasible.

I , No.12210020 F/O Leela Dhar was performing the duty of Camp Adjutant in CATC190 at Rewalsar hereby sabmit that on16-7-2016 arround 8-30 pm senior cadet of TSC
reported me that one girl cadet Kamna of GSSS Saigaloo was ill. I immediately called up 108 for
Ambulance. The ambulance arrived within 10 minutes. The said cadet accompanied by her
sister and Sub Balbir singh were sent to Zonal Hospital Mandi. While the ambulance was
administering the first aid, I informed the mother of cdt Kamna at 8.50 pm and her sister cdt
Anchal also spoke to her mother. I got a Call Back from her father at 8.55 pm and informed him
about the situation. The ambulance had left by that time. About 1150pm Sub Ashok Singh
informed me about the death of Cdt Kamna.

Ans. At 0830pm on 16 June 16. This was told to me by one SW Cdt that she is having some

Ans. No. As per my experience of 8 to 10 Camp, the number of sick report cases were not high.

Ans. After the death of Cdt Kamna ,as there was no clear cut reason for death, some person
spread the rumor of food posing.

I am posted as Administrative Officer of 2 HP Bn NCC Mandi. I was attending Combined
Annual Training Camp CATC -190 at GSSS Rewalsar 11 Jul 2016 to 20 Jul 2016. The Camp
had 443 cadets from for SD/SW at JD/JW.

Along with the ATC, The Shimla Group team for TSC was also being trained which
comprised of 42 cadets.

HP/JW Cdt Kamna of GSSS Saigaloo D/O Sub Balbir singh was also part of the TSC
team. On 16 Jul 2016 Cdt Kamna Devi attended the .22 firing practice at 0800am under Col
Harsh Sharma . At around 0930 am she complained of uneasiness and was brought back to
the school where she was residing.

She was taken to the Primary Health Centre in Rewalsar at about 1245 pm. She was
given treatment and discharged by the PHC at around 3.15pm.

At about 0815 pm she again complained of uneasiness so we organized an ambulance
of the State Government and send her to Hospital in Mandi where she was attended on.
At around 11.15 pm we got the news of her demise from Mandi Hospital .

Ans. Cdt Kamna Devi looked stable when she came back from the PHC Rewalsar. She
immediately went to rest her room. Thereafter I had not received any complaint about her. It was
only in the evening at about 0830h that I got her complaint and the necesing medical assistance
was arranged.

The PHC Rewalsar was functioned from 1000h only, thereafter there were no medical
facilities available. Any cadet falling sick even for a minor ailement had to be evacuated to
Hospital at Ratti / Mandi in a ambulance.

During the course of training of NCC Cdt , firing of .22 Rifle was conducted at a
modified short range . The firing commenced at around 0930h on 16 July 16. JW Cdt were asked
to be part of the 1st detail to fire / practice.

Cdt Kamna Devi was part of the fist detail to participate in the firing. After
complation
of her firing, the Cdt Kamna Devi aproched me and complained of stomach ache and cramps .I
asked the UO [Under Officer ] Monika Thakur to accompany her to room as Cdt Kamna Devi had
requested that she wanted to rest in her room. UO Monika Thakur of 1 Bn (Girls) Solan

accompanied Cdt Kamna Devi and made the Cdt comfortable in the room , with the instructions
that in case of any increase in pain or discomfort, she should inform me.

Around 1230h after the conduct of firing when I returned to camp, I saw and was
informed by the Camp Comdt Col Piyush Agarwal that the girls Cdt Kamna Devi was in
discomfort and she was being sent to Primary Health Centre at Rewalsar .

During the game parade, I inquired well being from the senior girls Cdt UO Monika
Thakur regarding the well being of Cdt Kamna Devi and was informed that the Cdt was back form
the dispensary and was resting in the room .

Around 2030h, I was info that Kamna Devi was still not feeling well and hence was
being sent / evacuated to the Hospital at Mandi . Late at night around 2315-2330h the Camp
Comdt Col Piyush Agarwal informed that me Cdt Kamna Devi had expired in the Hospital .

assigned the duty of Sub Adjt. Our unit was detailed to conduct CATC-190 at GSSS Rewalsar wef
11 Jul to 20 Jul 2016. The camp strength was 443 cdts. On 16 Jul 2016,at 2030h cdt Kamna Devi
who was complaining of stomach ache was required to be transferred to civil hospital Mandi. I
was detailed to accompany her to the hospital in the Govt Ambulance when I arrived at Mandi cdt
Kamna Devi was in the ambulance and her sister cdt Archna was also sitting in the ambulance.
Ambulance medical assistance had given her glucose and oxygen as first aid. The distance from
camp location to civil hospital Mandi is about 27 KM. The parents of cdt Kamna Devi had been
informed & cdt Archana had spoken to them. We reached at hospital Mandi at 2230h. The parents
had already reached the civil hospital Mandi. The medical staff at Mandi hospital admitted her and
started treatment and we were asked to wait outside. At around 2330h I was informed that cdt
Kamna had expired. Cdt Kamnas parents on having the news broke down and I tried to console
them. I also inform to CO Col Piyush Agarwal and Sub Maj Jaswant Singh 2 HP Bn NCC Mandi
on telephone regarding the said demise.

am performing the duties of Training JCO. Our unit Camp CATC 190 was organized from 11
July 2016 to 20 July 2016 at GSSS Rewalsar. The strength of this camp was 443 cdts and out
of this strength , 42 Cdts were to be trained for TSC. The training was done by Col Harash
Sharma One of the Cdt from the TSC team, Kamna Devi reported stomach ache on 16 Jul 16.
She was taken from line area toM I Room in the camp at around 1230 hours.

Primary Health worker Smt Sujata looked at her condition and advised for taking her to
PHC Rewalsar. I immediately took permission from Col Piyush Agarwal , CO 2 HP Bn NCC and
sent her to the PHC in his vehicle along with Smt Sujata and Kamna Devi returned to the camp
location at around 1530 hrs. I inquired as to her condition and she replied that she was fine. She
again complained of stomach ache at around approx 2030 hrs. Third officer Leeladhar of GSSS
Rewalsar called ambulance and cdt Kamna was taken to Civil hospital, Mandi. Sub Balbir Singh
and cdt Archna accompanied her to the Civil hospital. At around 2330 hours, I came to know
Kamna from Sub Major that she was no more.

I, Cdt Archana am the elder sister of Kamna who was studying in GSSS, Siagloo. Kamna
was selected for Gp TSC team. She had done NCC Camp earlier also being a selected TSC Cdt.
She in one year younger to me and was quite active. We both were attending CATC- 190 w.e.f. 11
July to 20 Juy 16. Camp pgme were going on smoothly from 11 Juy to 15 Juy 16 but on 16 Jul 16
she complained of stomachache. She took some rest but her condition did not improve. Hence,
she was taken to Primary Health Centre where dome treatment was done but on return her
condition did not improve completely. Ambulance was called by the camp staff and I accompanied
her to Civil hospital, Mandi. My parents and Tayaji were informed on mobile and they had reched
at Civil hospital, Mandi. After sometime, I came to know that she is no more.

2015.
I have been performing the duties of Adm JCO. My responsibilities involves providing
administrative and logistic support to cdts. I was ADM JCO duty for CATC 190 also which was
conducted from 11 Jul to 20 Jul 2016 at GSSS Rewalsar. I was assisted by two NCOs for adm
duties. First Officer Leeladhar was also detailed to look after the adm and was performing the
duties of Camp Adjutant.
GSSS Rewalsar had adequate administrative support for accommodation and drinking
water to cadets. To prepare food, a cook party was hired and the camp site was cleaned before
the arrival of the cadets. Dry ration was purchased in bulk from Mandi. Packaged milk was
supplied daily by the Chakkar diary at the camp site location. Fresh to include vegetables and
fruits was purchased from Mandi once in two days. I took keen interest to ensure that the
preparation of food was healthy and hygienic. Cook party
alongwith Adm NCOs were
instructed to dispose of the surplus/extra cooked food to prevent its use for the next meal. Water
for cooking and drinking was from the regular source at school. This water is of good quality
and was used by the locals as well. The food was made as per the menu chart and distributed
centrally at two points during the prescribed timings only. The platoon JCOs used to eat the food
with their platoon cadets. Other officers, ANOs and JCO/OR used to also eat the same food in
the adjoining room.

Ans. Dry ration were bought from Mandi Bazaar and the fresh were bought every two days
rom Mandi Sabji Mandi and milk was supplied to us by diary daily.

Ans. On the order of the AO we used to dispose of waste food in a pit. No cooked food was
reused.

Ans. The menu was as per the menu of the day which I produce to the court.

Ans. The water was taken from a natural source. The same water has been used by all the
villagers and the school since long.

Ans. The thorough cleaning of the area was done before we established cook House and
dining hall. On daily basis the cook house was cleaned before and after making meals.
Simultaneously all the utensils were cleaned. It was also ensured that there were no insects in
the area after this . Periodic spary was carried out under my supervision.

for postmortem at 1230h I conducted the postmortem, after that I handed over the body to the
police along with Visera, Sample of blood and urain along with a letter to comical examiner for
bis cemical and histopathological examination. I also handed over the postmortem report.

about 1230h she cadet Kamna again come to me and told that she is still having pain. I imddly
look her to army auth to take her to PHC Centre at Rewalsar . I was told to go with her. I along
with cadet shalrani took cadet Kamna to PHC Centre in army van . at PHC centre she was treated
by doctor Mrs Stakshi Shavm , at about 1430h she was discharged from PHC. I along with cadet
Kamna come back to comprt as past. it took us opprox.10min.to reach camp. Officer coming to
camp she went to her room for rest at 1600h I left the camp area as my duty is from 9AM to 4PM.

gave her Injection DICYCLOMINE and infection PERINORM. at around 1430h we checked her
vitals again which were ok and she was not in pain. At 1430 we told her to go back to the camp
and to take rest.

I Jc 820 369 P Sub maj Jaswant Singh come on posting to 2HP NCC Mndi
on 27 Oct. 2014. After coming have I attended two Annual trg. Camps at truant
camp Pandoh and had camp at MLSM college Sundernagar. GSSS School
Rewalsar was fourth camp from 11July 2016 to 20 July 16. On 16 July 16. I went to
Mandi and when in evening I come back to Rewalsar camp I was told my Sub Anil
that cadet Kamna is not well and she is being send in Ambulance to Ratti Hospital
and Sub Balbir and her sister Archna is accompanying her. I asked in case the
parents come informed. I the Range up Sub Balbir and he told me that they are
going to Zonal Hospital Mandi and parents are told to come to Mandi at 1030PM
on 16Juy 16 Sub Balbir Singh told that patient have been admitted to ICU and at
1130 PM, I was info that the patient had died I informed the Co. Nest day Dy.
Director and Tehsildar came and they told that a order of DC the camp have to b
closed. Co told us that till order one received from Gp HQ till that time camp will not
closed. On 18 July 16 on order from Gp HQ the kids forwarded hereby placed as
on sent back and Rest I the children were sent on19 July 16 and camp close repot
was given to Co.

Ans. A lady ANO for their for two days after that she left ass she had some
peoblem at home after that she never come back.

Ans. There was one lady Health worker available at Camp site from 1000h
to 4PM, then after any med problems was sent to CHC Ratti through 108 Amb
service.

Ans. I do not know that we can levie a civ NA/Doctor for Camp.

Gitanjali was also admitted to Mandi Hoop my me along with another cdt on

Ans. As the str of cdts is 3500 and if we reduce the str then we have to do
approve 14 camp in yr. as per as the facilities were enough to also camp for
443 cdts.

I Mrs Rami Devi r/o Mandi, I am working as peon with 2HP Bn NCC at Mandi since last
three years.
I was detailed to go for NCC Camp at Rewalsar which was from 11July to 20July. I
and Mrs Gita Devi were told to look after the girls at Camp. We use to stay with girls and also
lookafter the officer ie gmy tea to office staff. I come to know from other girls that cadte Kamna
had some problem and she has been taken to PHC and then after and night to 20nd Hoop Mandi
at about 1140h I came to know that she has died.

attended many camps after be coming ANO. I was detailed for CATC camp at Rewalsar wef
01/ 07/ 16 to 20/ 07/16 when 108 Ambulance came in the camp area then it I come to kow
about illness of cdt Kamna Devi, thereafter next day 0600h during PT Prade it was informed
that she had died in the hospital, in this camp we were from ANO, during mon assigned
duties or 17/07/16 morning Tehsildar Balh and SHO Balh came in the camp. Therefore
camp was closed on 17/07/16 and we along with on cdts. Cdts. Come back to School.

I was informed about the camp tha Rewalsar wef 11 Jun to 20 Jun 2016 through email on School mail. I was called up on to attend the camp along with my School children
on 11Jun to 13 Jun 2016 at Rewalsar. I have been relieved by the Principal NSCM GSSS
Padhar for the camp on 11 Jun 2016. I was in the camp location wef 11Jun to 13 Jun 2016.
On 13 Jun due to my daughter illness I come to home after Cos permission for one day
leave. Thereafter, due to one of my relatives death I sought relieving from camp duty from
commanding officer Sir, through TV news I come to know about death of cdt Kamna in
camp and illness of other cdts. Simultaneously I rang info my Schools children to know
their well being.

Ans. Nobody come to me for any problem. I the binging of the camp ie first day all the
girls were told that anybody need solely pad can go to cdt Kamna, as she was given stock
to given to girls who need them, girls were also told that if they fever any peoblem they can
come to me, but till time I was in camp nobody come to me.

Ans. I was told that if any girls falls sick I have to report to the staff and girls to Health
worker who was in camp and if required they will be taken to PHC Rewalsar.
