of duties and responsibilities so as not judge him/her by inappropriate standards or by
personality traits unrelated to his/her work.

Item 3.1.10 is to be rated only for staff members whose duties involve supervisory
responsibility and items 3.1.11 3.1.13 only for staff members in management positions.

Concluding Remarks

4.1

If the average rating of the staff member is below standards, mark one of the
boxers provided below:

The staff member needs advice to improve her/his


performance and conduct.

4.1.2

The staff member should be transferred to another


post of similar grade.

4.1.3

The staff member should be demoted.

4.1.4

The staff member is incapable and his/her


dismissal is therefore recommended.

4.2

For a staff member on probation:

4.2.1

Since the average rating is satisfactory or better;


the staff member should be accorded a career appointment.

4.2.2

Since the average rating is below satisfactory


level but far better than below standard, the period
of probation should be extended for six months.

4.2.3

Since the average rating is below standard,


dismissal of the staff member is recommended.

After all sections above have been completed, the staff member must sign the original of the report
and be given a copy thereof. The signature merely acknowledges that the staff member has read the
report and received a copy; it does not necessarily indicate agreement with the evaluations contained
in this report.
