According to North Page Research (2010) Patient Identity Management is a critical


function for healthcare-related federal and state government agencies (North Page Research
2010). There are five tips for Successful patient Identity management in government agencies,
including; dont reinvent the patient identity management Wheel, automate patient identity
management integration and delivery, align your patient identity management architecture with
your agencys needs, find and fix the sources of compromise to Patient data quality, and make
data integration the enabler of patient identity management (North Page Research 2010).

A. (2014) patient matching and identity management also has been a critical component of health
information management (HIM) since the beginning of the profession, when master patient
indices (MPI) were kept on 3x5 inch index cards, HIM professionals knows about the challenges
with duplicate health record numbers, and these issues have a cascade effect as duplicates within
a single organization are transferred downstream to other providers and health information
exchanges, but patient identity and duplicate record issues have only increased with electronic
health records, causing HIM professionals to reexamine how they can help prevent these errors
from occurring in their new electronics systems (Wiedermann, L. A. 2014). In this paper, I will
try to create a patient identity management policy including; the policy title, version number,
effective date, date ratified, ratified by, introduction, definition, policy, and general guidelines, in
that order. The Information that I reviewed is from this website called (Patient Access UCLH

with the Patient Access User Manual (Patient

patients who are waiting for treatments on
admitted, non-admitted or diagnostic pathways,
it covers the management of patients at all sites
where UCLH operates, including outreach
