AFFIDAVIT OF LOSS
I, FRANCES MORELOS, of legal age, single, Filipino, and a resident of Unit 31, 12 The
Bellagio road, Bonifacio Global City, Taguig, Philippines, after being sworn in accordance with law,
hereby depose and state
1. I am the purchaser of condominium unit located at Unit 31, 12 The Bellagio road, Bonifacio
Global City, Taguig, Philippines with an area of Twelve square meters covered by
Condominium Certificate Title No. 34421 of the Reigstry of deeds of Taguig City as
evidenced by the Deed of Sale of a Registered Condominium unit which was executed in my
favor by ABC and thereupon acknowledged before Notary Public and docketed in his
Notarial Registry as Doc. No. 121; Page No. 22; Book No. 11; Series of 2016. A copy of
the said Deed of Sale of a Registered Condominium Unit is hereto attached and made an
intergral part hereof as ANNEX A.
2. Immediately after the Deed of Sale was executed, the original duplicate of owners copy of
CCT title No. 34421 was turned over to me by ABC. He likewise delivered to me the
possession of subject unit and since then i have openly exercised and maintained all rights of
ownership thereon.
3. I always carry the said CCT copy together with my other personal belongings inside my hand
bang when it was suddenly snatched from my possession while im traversing the streets of
Manila. All diligent efforts i exerted to retrieve it were in vain; hence for all legal intents and
purpose it is now deemed lost and beyond recovery.
4. I certify and confirm that the said unit covered by CCT No. 34421 was never mortgaged,
sold, conveyed, disposed of, or encumbered, to any other person or entity. I certify and
confirm further that said document was neither confiscated nor relinquished in favor of, any
government entity agency, or authority.
5. I attest to the truth of the foregoing statements to the best of my own personal knowledge and
basd on authentic documents or records. I am executing this Affidavit of Loss to support my
application for a new owners duplicate copy of Condominium Ceritificate of Title No. 34421
as well as for all lawful intents and purpose it may serve.

LEASE CONTRACT
KNOW ALL MEN BY THESE PRESENTS:
This CONTRACT OF LEASE is made and executed at the City of Manila, this day of 09 September 2016,
by and between:
ATTY. FRANCES MORELOS, of legal age, single, Filipino, and with residence and postal address
at 2 THE BELLAGIO, BONIFACIO GLOBAL CITY, TAGUIG, 3441, hereinafter referred to as
the LESSOR.
-ANDMRS. JIRENE KATIGBAK, of legal age, married to MR. NATHANIEL KATIGBAK, both Filipino
and with residence and postal address at 12 KALAYAAN AVENUE, Q.C. 7221, hereinafter referred to as
the LESSEE.
WITNESSETH; That
WHEREAS, the LESSOR is the owner of THE LEASED PREMISES, a residential property situated at
UNIT 10, 34 THE BELLAGIO, BONIFACIO GLOBAL CITY, TAGUIG, 3441;
WHEREAS, the LESSOR agrees to lease-out the property to the LESSEE and the LESSEE is willing to
lease the same;
NOW THEREFORE, for and in consideration of the foregoing premises, the LESSOR leases unto the
LESSEE and the LESSEE hereby accepts from the LESSOR the LEASED premises, subject to the
following:
TERMS AND CONDITIONS
1. PURPOSES: That premises hereby leased shall be used exclusively by the LESSEE for residential
purposes only and shall not be diverted to other uses. It is hereby expressly agreed that if at any time the
premises are used for other purposes, the LESSOR shall have the right to rescind this contract without
prejudice to its other rights under the law.
2. TERM: The minimum term of lease is for TWO (2) YEARS from 09 September 2016 to 09 September
2018 inclusive. Upon its expiration, this lease may be renewed under such terms and conditions as may be
mutually agreed upon by both parties, written notice of intention to renew the lease shall be served to the
LESSOR not later than seven (7) days prior to the expiry date of the period herein agreed upon.
3. RENTAL RATE: The monthly rental rate for the leased premises shall be SEVENTY-FIVE
THOUSAND PESOS (P 75,000) in Philippine currency. All rental payments shall be payable to the
LESSOR.
4. DEPOSIT: That the LESSEE shall pay an advance to the LESSOR upon signing of this contract and
prior to move-in an amount equivalent to the rent for ONE (1) MONTH or the sum of SEVENTY-FIVE
THOUSAND PESOS (P 75,000) and a deposit equivalent two (2) months of the rent or the sum of ONE
HUNDRED AND FIFTY THOUSAND PESOS (P 150,000). Wherein the two (2) months deposit shall be
applied as rent for the 12th month and the remaining one (1) month deposit shall answer partially for
damages and any other obligations, for utilities such as Water, Electricity, CATV, Telephone, Association
Dues or resulting from violation(s) of any of the provision of this contract.
5. PRE-TERMINATION: In case the agreement has been Pre-Terminated by the LESSEE in the payment
of the rent, or in any other form, the LESSOR at its option may terminate this contract and eject the
LESSEE. The LESSOR has the right to padlock the premises and may forfeit whatever rental deposit
or advances have been given by the LESSEE if only the latter is to be considered in default.
6. PETS: Bringing pets inside the leased premises is strictly prohibited. If found in possession of any
animal creature therein, whether intentional or accidental, will be paying a fine of not more than Two
Thousand Pesos (P 2,000) as may be determined by the management.
7. SUB-LEASE: The LESSEE shall not directly or indirectly sublet, allow or permit the leased premises to
be occupied in whole or in part by any person, form or corporation, neither shall the LESSEE assign its
rights hereunder to any other person or entity and no right of interest thereto or therein shall be conferred
on or vested in anyone by the LESSEE without the LESSOR'S written approval.
8. This CONTRACT OF LEASE shall be valid and binding between the parties, their successors-ininterest and assigns.

CONTRACT OF LOAN
WITNESSETH
1. That the lender extends loan to the borrower in the amount of FIVE MILLION PESOS (5,
000, 000. 00), Philippine Currrency, which amount in full is hereby received in hand by the
latter;
2. That the loan is subject to the following conditions:
a.) The borrower shall pay the principal amount of the loan (P 5, 000,000.00) through
annual amortization of five hundred thousand pesos (P 500,000.00) each beginning the year 2017;
b.) The loan is subject to six percent (6%) annual interest and shall be paid to the lenders
account at the LandBank of the Philippines Espana, Sampaloc, Manila Branch;
c.) Unpaid interest shall form part of the principal amount of the loan which shall bear the
same interest as above-stated.
3. The the borrower shall faithfully comply with his above-obligations and that failure to do so
will give the lender the right of action for collection and damages before the proper court.

Dear Ms De Leon,
This has reference to your contractual obligations with our client Frances L. Morelos.
On September 10, 2016, you entered into a contract with him under which you are
granted a loan in the amount of FIVE MILLION PESOS (5, 000, 000.00), Philippine Currency,
payable through annual amortization of FIVE HUNDRED THOUSAND PESOS (P500, 000.00)
EACH, with six percent (6%) interest per annum, including surcharge of unpaid interest; the
amortization payment was to begin in year 2017, the year following the execution of the subject
contract.
It is with deep regret that up to this writing you failed to make good of your obligation
under the contract you have failed you pay your annual amortization despite the lapse of three
years. Our client has desperately failed to contact you by text messages and other means of
communication; your whereabouts could not be ascertained.
In view of the foregoing and on behalf of our client, we respectfully demand that you
settle your contractual obligations representing your unpaid annual amortization (plus interest) for
the last three years. Your payment should be made through our clients bank account with
LandBank as contemplated under paragraph 2 (b) of the contract.
We are giving you three month from the receipt of this demand letter within which to pay
your delinquent obligations. If you still fail to comply, we will be constrained to institute legal
actions before the proper court to protect the interest of our client.
We request that you seasonably act with dispatch in complying with your herein
obligations in order to avoid the inconvenience of an expensive counsel for the creditor/ lender
litigation.

- Atty. Dorado mentioned that staff member, Micah Tiburcios husband is ill and in the hospital.
MOTION to send a cash gift to Tiburcios husband, expressing the banks sympathy and support;
seconded and passed.
- Mr. Umbalin explained that a painting entitled A Self Portrait of Juan Luna is for sale for P
48,000,000.00. The president immediately proposed to purchase the painting for the bank. He said it
would add prestige to the bank as it hangs majestically on its lobby wall. The said painting is described as
follows:
- Acosta presented members with a draft of the reworded By-laws paragraph that would allow
members to conduct actions over electronic mail. Acosta suggested review and a resolution to
change the By-laws accordingly. Atty. Dorado suggested that she first review and verifies if the
proposed change is consistent with the local statute. Acosta agreed to accept this action and
notify
members
of
the
outcome
in
the
next
Board
meeting.
4. Other
business:
- Mendoza noted that he was working with staff member, Jacob dela Cruz, to help develop an
information management systems plan, and that two weeks ago he (Mendoza) had mailed
members three resumes from consultants to help with the plan. In the mailing, Mendoza asked
members for their opinions to help select a consultant. He asked members for their opinions.
The majority of members agreed on Lease-or-Buy Consultants. MOTION to use Lease-or-Buy
Consultants; seconded and passed.
- Umbalin announced that she had recently hired a new secretary, Karla Javier.
5. Assessment
of
the
Meeting:
- Tafalla noted that the past three meetings have run over the intended two-hour time slot by half
an hour. He asked members to be more mindful and focused during discussions, and suggested
that the Board President to take an action to identify solutions to this issue. President, Umbalin,
agreed.
