ENTRANCE QUESTIONNAIRE

If you wish to become a student in the Arcane School will you kindly answer the following
questions. All information given is regarded as strictly confidential.
1. State your full name (Mr., Mrs., Ms., or Miss) and address.

2. Give the date and place of your birth.

3. State whether married or single, also number and ages of children, if any.

4. What is your citizenship?

(a) Are you a native or naturalised?


(b) If naturalised when and from what country did you come?
(c) What is your racial background?
(d) What is the national background of your parents?

5. What is your occupation or profession?

6. What is your educational background? If academic, state colleges and degrees, if any. If selfeducated, along what lines?

7. What languages do you know? How well?

8. Is your health good? Have you any physical limitations?

9. How did you hear about the Arcane School?

10. Why do you ask to enter this group of students? Give your reasons after searching your motives.

11. Have you been or are you now affiliated with any occult organisation or esoteric school? Do you
belong to any church?

12. Have you done any definite study work along occult or metaphysical lines? Have you read any of
Alice Bailey's books? If so, which titles?

13. In what countries have you lived or travelled?


(a) Have you any racial prejudices?
(b) Have you any religious prejudices?
(c) Have you any ideological prejudices? Remember a prejudice can be either for or against.

14. What use do you intend to make of any knowledge you may gain in the Arcane School?

15. What first started your interest in occultism and the esoteric sciences?

16. What do you understand by the expression: the Soul and the vehicles through which it manifests,
the Soul and Personality, or -- as the occult schools sometimes word it -- the Constitution of Man?

17. Are you attempting to adapt your personality life to the higher impulses which come from the
soul?
(a) If there are hindrances to the adaptation, what do you think they principally are?
(b) What are the main handicaps to esoteric study presented by your circumstances and
environment?
(c) What are the conditions which offer you the greatest opportunity for growth?

18. Are you prepared to do the following things:


(a) Meditate regularly for 15 or 30 minutes (according to instructions) every day and report on it
every month?

(b) Study regularly a minimum of 30 minutes every day?


(c) Every day review the motives which have lain behind the actions and words of the past 24 hours?

19. Judging from the spiritual results and enlightenment which followed, what events do you regard as
having been the most important in your life?

20. Please state, if you can, your spiritual goal in life; what is your mental goal? What is your physical
goal?

21. Do you regard yourself as being:


(a) Mentally focused and on the line of the intellect? If so, why? If not, why?
(b) Emotionally inclined and therefore aspirational and devotional? Why?
(c) Introverted and basically introspective?
(d) Inhibited or suppressed?

22. To what extent does fear enter into your daily life?
(a) Have you always suffered from fear?
(b) Of what are you most afraid?

23. State which you feel to be the least controlled of your three bodies: mental, emotional or physical.
(a) What do you consider to be your major fault or faults?
(b) What do you feel to be your outstanding virtue or spiritual quality?

24. What do you understand by meditation? Please give definitions.

25. Have you ever meditated? In what way and with what results? Please be specific.

26. Have you ever had any supersensory, mystical or psychic experiences? What importance do you
attach to them?

27. Do you now or have you ever used any breathing exercises? If so, please describe.

28. Do you believe in the existence of the Masters? If so, why? If not, why?

29. Do you believe in the Law of Rebirth or Reincarnation? State why.

30. Religions teach the treading of the Path. What do you understand by that expression?

31. Have you counted the cost to the man who treads the Path? Are you prepared to face that cost? If
so, why?

32. Define "Service". What is your service to humanity?
