4. Provide evidence of a visit such as group photos with the person in charge at the
company.

Presentation:
1. Each student is required to present within 3 - 5 minutes. Total time for group
presentation between 15-25 minutes.
Introduction
Company background, Nature of business, Address, Date of
visit, Evident of visit, PIC at company and etc.
Design/identify the facilities for occupational safety and health
that should exist on the site.

What type of hazards that may occur and how are they
avoided?
Chapter 5
(List the 5 types of hazard that may occur on site and their
preventive procedure, identification/evaluation, analysis of the
causes, consequence/hazards, remedies, prevention
procedure, tool/equipment required, etc)
The explanation should be supported with relevant technical
diagrams/drawings/ HIRARC Form.

What are the procedures and activities need to be carried out in
the event of an emergency/ injury / accident / incident has
occurred?
(Simulate the emergency procedures taken in 5 cases of
injury or accident- emergency evacuation, alarm,
rescue/ambulance/medical support, emergency shut-off,
isolation /production shut-down/ re-route operation, forensic
investigation, reporting, compensation and insurance claim)
