Description of the patient & instructions to simulator: 43 year old female/male, demanding sick note for back pain
She is an IT Manager by profession having a desk job, happily married with two young
children.
Have recently had 5 episodes in past 6 months where she had to take 2-3 days off on
each occasion. She puts it down to her back pain but on exploring concerns she admits
to being bullied at work. There was no element of physical harm but just emotional
stress due to being criticised at work.
She has to now go to a meeting at work to explain her absences.
As far as her back pain is concerned it is non specific pain, which improves with simple
painkillers like paracetamol. It is not associated with any numbness in limbs/urinary or
bowel problems. She is normally fit and well, not on any medication. Her general mood,
appetite and sleep are not really affected by the pain but she has insomnia and anxiety
with her situation at work!
In short the real reason for taking the time off work was bullying at work rather than the
back pain
In the end accepts a sick note with diagnosis of stress for work.
Examination of the back just showed generalised mild tenderness on lower back with full
range of movements.
