AFFIDAVIT OF STATUS
I, ROMULO B. SANCHEZ JR., of legal age, single, a Filipino citizen, and a
resident of Brgy. Dancalan, Bobon, Northern Samar, after having been duly sworn
to in accordance with law do hereby depose and say:
1 That I was born on August 21, 1987 from spouses Romulo B. Sanchez Sr. and
Flor B. Sanchez at Brgy. Dancalan, Bobon, Northern Samar;
2 That I am single and have never contracted marriage before;
3 That I do not possess any legal impediment to contract marriage under the
laws of the Republic of the Philippines;
4 That I am executing this affidavit to attest to the truth of the foregoing
statement of facts for whatever legal purposes this may serve.
