Epidemiology: With a worldwide prevalence rate of 50%, H. pylori is the most common bacterial infection
in the world.
Clinical manifestations: Although the majority of patients are asymptomatic, different symptoms and
relevant pathologies may be caused by it, highlighting dyspepsia, peptic ulcer and gastric cancer. In fact,
H. pylori infection is the main cause of peptic ulcer disease and is an important cofactor in the
development of gastric lymphoma and adenocarcinoma.
Diagnosis and treatment: H. pylori eradication therapy allows the complete healing of peptic ulcer, which
has meant a real revolution in the gastroenterology. The proper diagnosis methodology, treatment
indications and the most appropriate therapeutic regimens for the eradication of bacteria are related in
the present guidelines.

En primer lugar, es necesario establecer un listado de indicaciones para llevar a cabo el diagnstico y tratamiento de la
infeccin (tabla 1). La dispepsia es, sin duda, la indicacin
ms frecuente de tratamiento erradicador. Las tres aproximaciones a la dispepsia no investigada que se han mostrado
ms eficaces son la realizacin de una endoscopia inicial, el
tratamiento emprico antisecretor y la denominada estrategia test and treat2. Esta ltima opcin consiste en la realizacin de una prueba no invasiva (que no precisa de una endoscopia, idealmente una prueba del aliento) para H. pylori y
el tratamiento erradicador consiguiente si se demuestra la
infeccin (fig. 1). La estrategia test and treat sera aplicable
nicamente a pacientes disppticos jvenes (menores de 55
