HEALTH SAVINGS ACCOUNT

At Mondelez Global, we like to keep it simple. Just as we strive for


simplicity at work, we believe selecting and understanding your
medical plan shouldnt have to be hard. Thats why we offer one
medical plan the Mondelez Global Medical Plan, administered by
Aetna.

Keeping it simple also means helping you pay for your health care expenses. But
thats just one piece of the pie. The Mondelez Global Medical Plan features a
Health Savings Account (HSA), a tax-advantaged account that you own, which
allows you to contribute on a before-tax basis.

The Plan provides access to a high-quality network of providers and gives you an
opportunity to earn money in an account when you take certain steps to maintain
and improve your health. With most preventive care services covered at 100%,
we make it easy to keep up with annual exams for you and your eligible covered
dependents. Heres a snapshot of in-network benefits in the Mondelez Global
Medical Plan:
In-network

With an HSA, you decide how much to save and when to spend it now or in
the future. The HSA can be used to pay for deductibles, co-insurance, prescription
drugs and other eligible expenses.
In addition to your contributions, Mondelez Global may also contribute to help you
pay for eligible out-of-pocket expenses:
1. Wellness dollars: You can earn up to $650 in 2014 for participating in the
Fuel4Life wellness program.
2. Automatic dollars: The company may also make an automatic contribution
to the HSA depending on base pay and coverage level.

Mondelez Global offers a dental plan administered by Aetna. The Dental Plan
provides:

Local plans are available to employees in California and Hawaii.

Prescription drug coverage, which is included when you enroll in the medical
plan, is also administered through Aetna. It includes both retail pharmacies and
convenient home delivery (mail-order) prescription service. What you pay for
prescriptions will depend on the type of medication you buy.

Preventive care covered at 100%, including routine exams and cleanings

$2,000 per person annual coverage limit

$2,000 lifetime orthodontia coverage

$1,200 Temporomandibular Joint (TMJ) Syndrome coverage

VISION COVERAGE
The Aetna VisionSM Preferred Plan offers routine eye exams, access to a broad
network of providers, low co-pays for eyeglasses and contact lenses and discounts
for vision materials.

unexpected and thats why we offer a line-up of comprehensive
life and disability insurance programs.

Thats why we provide you and your spouse or domestic partner with Fuel4Life,
a wellness program that provides access to interactive, easy-to-use tools and
resources to keep wellness top of mind, at no additional cost. Fuel4Life includes:

You are automatically eligible for the following basic insurance and short-term
disability benefits, which are fully paid by the company:

Personal health assessment and biometric screening to provide a


clear picture of current health status and a health action plan.

In Touch Care, Lifestyle Coaching and wellness challenges to


provide motivation to adopt or maintain a healthy lifestyle.

Tobacco Cessation Program, which offers personalized tools to help quit


tobacco, including weekly calls with a coach, a personalized plan for quitting
and up to eight weeks of Nicotine Replacement Therapy.

Interactive online tools, including nutrition and exercise planners,


calculators and a Personal Scorecard, available 24/7.

Basic Life Insurance provides a benefit to designated beneficiaries, equal


to two times annual base pay, to a maximum of $1 million.

Short-Term Disability provides income protection to you for up to 26


weeks (the first five work days may be compensated with available PTO days)
in the event you are unable to work due to illness or injury.

Business Travel Accident Insurance provides a benefit to designated


beneficiaries, up to four times annual base pay, to a maximum of $5 million.

Optional life and disability coverage programs are also available for purchase:

Group Universal Life Insurance (GUL) provides additional life insurance


coverage to supplement your basic coverage through the company. You may
elect between one and six times your pay, up to $2 million. Coverage is also
available on the lives of your spouse/domestic partner and/or children.

Employee Assistance Program: My Life Values

Inspiring trust is all about maintaining a human connection even when times get
tough. Emotional health is a big part of overall health, and no employee should
have to endure a difficult time alone. Mondelez Global offers support through My
Life Values to help with lifes challenging times.

Long-Term Disability (LTD) provides tax-free income after 26 weeks of


short-term disability. You may choose coverage of 50% or 60% of base pay.

Voluntary Accidental Death & Dismemberment (VADD) provides


additional coverage of up to ten times annual base pay or $1 million.

As a bonus, you can earn wellness dollars deposited into you HSA up to $650 in
2014 for participating in certain wellness programs year-round.

My Life Values is automatically provided to you and your family members, and
offers a variety of services including 24/7 confidential counseling and free
concierge services.

FLEXIBLE SPENDING ACCOUNTS
Mondelez Global offers two different Flexible Spending Accounts
(FSAs), depending on eligibility:

The Dependent Day Care FSA lets you contribute up to $5,000 before-tax
each year to pay for eligible child or elder care expenses.

The Health Care FSA allows you to contribute up to $2,500 beforetax each year to pay for eligible health care expenses, including medical,
prescription drug, dental, orthodontia and vision expenses. If you elect
Health Care FSA coverage or if your medical expenses can be reimbursed
under your spouses employers health care FSA, you will not be able to
contribute to an HSA.

Other Protection Programs


In addition to life and disability, you may purchase other insurance programs to
protect against a variety of unforeseen events that could occur:

Group Long-Term Care Insurance covers custodial care for you, your
spouse or domestic partner, parents, grandparents or dependent adult child
at home, an adult day care center or nursing home.

Group Legal Services Plan provides coverage for legal assistance,


including wills, real estate matters, divorce and more.

Auto and Homeowners Insurance includes renters and liability


insurance.

financial dreams, we keep it simple by providing one easy-tounderstand retirement plan. Its up to you to act like an owner
and make it work for you.

Leading from the head and the heart means building a workplace
in which everyone feels supported. To round out our benefits
offering, we provide several programs to help you find balance in
all areas of your life.

The Mondelez Global LLC Thrift 401(k) Savings Plan is designed to help you
prepare for your financial future. And, we make it easier for you to save for
retirement through generous company basic and matching contributions. With
this plan:

Mondelez Global offers a flexible Paid Time Off (PTO) program that gives you the
time you need to renew, refresh and to take care of other responsibilities in your
life. PTO is based on years of service reached in the calendar year:

Paid Time Off (PTO)

Generally, you can contribute up to 15% of pay on a before-tax or after-tax


basis, or a combination of both.

In Your First Year

The company automatically makes a basic contribution equal to 4.5% of your


eligible pay each payroll period.

If initially employed in:

Mondelez Global matches $1 for $1 on the first 1% of pay you contribute and
70 for every $1 on the next 5% of pay contributed thats an additional
4.5% of savings with a contribution of at least 6%.

You are fully vested in Mondelez Global basic and matching contributions
after two years of service.

Once eligible for the Plan (typically 30 days after date of hire), youre
automatically enrolled with a 3% before-tax contribution, invested in
the Plans default investment fund. You can increase or decrease your
contribution rate and change your investment election at any time.

PTO is prorated based on month of hire.

Adoption Assistance Program
The Adoption Assistance Program provides adoption
expense reimbursement of up to $5,000 per child.

Educating for the Future: Educational Programs


Through the Scholarship Program, Mondelez Global provides awards to selected
eligible children of full-time active employees. The company also offers the
Educational Assistance Program to provide financial assistance to full-time salaried
and eligible hourly employees for pre-approved courses.
