CERTIFICATION
Per marginal note by the Municipal Accountant Mr. Deo Rosales
attached to the voucher in payment for 1 piece Clear Glass (32x72) for
replacement of broken glass at the Municipal Hall 2 nd Floor, Palo Municipal
Building under the name of Payee DIMAT GLASS 7 ALUMINUM SUPPLY c/o
RICHEL D. DANDAN, this certification is issued to attest to the following facts:
1.) That the Local Government Unit of Palo held the Leyte Landing
Memorabilia and Art Exhibit on October 17-20, 2015 in line with the
71st Leyte Gulf Landings Anniversary Celebration;
2.) That during the said event, Mac Arthur Memorabilia and Religious
items were on display at the Second Floor Function Hall of the Palo
Municipal Building. Among the items on display was an old
vestment worn by priests during the Spanish Period which displayed
on a manikin stand placed at the lobby of the Second Floor Function
Hall close to the glass door which is the subject of inquiry;
3.) That the same manikin was tightly secured to the wood carvings
atop the glass door by means of tie wires so that it will not be
toppled down. Sometime in the evening of October 18, 2016, strong
winds unexpectedly blew which led manikin to hit the glass door
resulting to a crack which eventually developed into the breaking of
the glass;
4.) That the breaking of the glass was not due to any fault or
negligence of the organizers of the memorabilia and art exhibit
since it was caused by sudden strong winds despite the fact that the
manikin was securely tied to avoid toppling down;
5.) That Richel D. Dandan is the owner of DIMAT GLASS & ALUMINUM
SUPPLY and that no payment has yet been given to nor received by
her concerning the payment for the replacement of the said broken
glass;
6.) That this certification is issued to attest to the truth of the foregoing
and for any legal purpose this may serve.
